Introduction

Most health organisations face issues in healthcare data integration and two-way interoperability due to siloed approach of healthcare providers in maintaining clinical, claims, laboratory, and pharmacy data with every provider maintaining their version of Health IT Infrastructure. It’s a cultural change and new healthcare models are trying to break this barrier by integrating as many systems as they can.

It is very evident that analyzing and integrating this data collectively can drastically improve patient care and both clinical and financial outcomes, but how to actually collect, read, integrate, understand and leverage the data remains a broken process.

With providers using multiple EMR systems to store patient information it becomes very difficult for a Network of providers or Payers to collate this patient information in order to make any sense of the data. Each EMR system uses it’s own proprietary format to store data which adds to the complexity of the data and since most of them are “Black Box” models, modifications to the architecture is very expensive.

Datashop as a platform provides the support to integrate data from various black box EMR systems into a single standardized data-lake. From this data lake Provider Networks or Payers can use the data for the purpose of interoperability, transferring the data between the different EMR systems installed at the various providers in the networks.

Datashop Health Integrate connects with clinics, hospitals, physicians, payers and other healthcare providers onto a unified data platform. This involves setting up Datashop Client at each of the several geographically spread systems, across disparate and legacy EMR/EHR systems, for multiple types of datasets - ICD-10, DRG, CPT, LOINC, HL7, CCD, C-CDA, FHIR ADT, 834/837 etc files. The acquired data from the remote locations would be encrypted and transferred on an ongoing basis to a server located inside the client network via secure file transfer protocols.

Using Datashop Client frequency for the data process can be set, can be real-time, daily, weekly, depending on the requirement. The Datashop platform setup incorporates data pipelines, which allows detailed overview and management of the data flowing into the system, including the ability to raise appropriate alerts and notifications in case of failures.

Datashop is ideally suited to manage disparate and distributed healthcare data. Through a scalable client-server architecture, the onus of filtering and pushing data is managed on each of the disparate client side (each of different sources such as EMR, Lab, Payer etc.), while cleaning and integration happens at the server side. Unlike most solutions in Market, Datashop’s ability to build real-time data stream lies in the fact that data from each of the client side follows a “push” mechanism, instead of a centralized “pull” approach. This gives our customers flexibility to decide how frequently data needs to be streamed, control over what information is transmitted, better accountability in terms of log management, and better security by managing access locally at each of the sources.

Datashop maintains a single patient view post integration. We call it Enterprise Master Patient Index (EMPI) which comes with two in-built features.

2 Map Data Tables

Once the source is successfully connected, a data tables can be configured and mapped to the single unified data schema. Datashop handles multiple databases like RDBMS (Oracle, SAP, SQL-Server, MySQL), Flat Files, XML, JSON, HTML, CCDs, and Documents.

Intelligent Meta Data Management feature recommends mappings from source table to single unified data schema which can also be altered via a manual override. Medical datasets in Datashop are based on Normalized Medical Schema (NMS)and the data is loaded from various data sources to persist in Medical Data Repository (MDR). H-Integrator for HIE

Datashop platform can connect to external HIEs (private or state) using IHE XDS.b standards based workflow, which are go-to standard in the industry for ‘pull’ transactions between two enterprises.

The solution workflow includes:Datashop acting as XDS.b document consumer and pulling data from HIE.Datashop on demand document source allowing the source to pull data from Datashop.

Utilizes secure internet connectivity between HIE and Datashop without the need of VPN, but requires security certificate. All messages to include SAML token as per IHE cross enterprise user assertion profile XUA and secured via TLS per IHE ATNA secure node specification.

Two Way Interoperability

As in any network oriented approach, biggest challenge is that disparate clinical systems have different way of accessing the data. Some systems are new-systems with built-in interoperability, while as some legacy systems are not wired for data sharing and management. Some ways of accessing data from such systems are flat files, file system access, Direct Database access, or application-layer-access (HIE connectivity, HL7 feeds, CCD, CCLF etc.).

We solve this problem by building a very lightweight scalable platform to account for not only various types of data-access allowed/possible, but a platform which is source-agnostic. It means that Datashop’s client can be deployed at any EMR, Lab, Clinical Decision Support or other systems, and the mechanism for integration will remain the same, irrespective of the source.

Our Datashop platform enables forward looking healthcare organizations to meet the growing needs of seamless connectivity between providers within their facilities, across their network and in the community. Datashop is equipped to help the organizations to achieve the success with:

1. Workflow integration and delta views - Datashop has CCOW compliant EMR extensions to provide seamless access to the unified patient record for the provider without leaving the EHR at the point of care. Datashop provides a set of views which is important for the physician to see the data available in other EHRs to make the patient chart up to date. This makes it simple for the clinician and hence relies on Datashop for the complete patient history.

2.Community connectivity: Datashop uses standards-based connectivity routes to connect employed and affiliate clinicians. They just simply subscribe to have the patient information published in to their EHR, Datashop HIE does the rest behind the scenes by averting costly point to point interfaces.

3.Semantics harmonization: Healthcare organizations do not lack of data but the lack of standardization makes it impossible to exchange the data in ‘as is’ condition. Datashop normalize and standardizes the data in source-agnostic format enabling achieve easy interoperability with other health information systems. The semantic data organization also allows clinician to quickly understand the true meaning of data from multiple disparate systems. 4.Innovative platform: In the ever changing healthcare landscape to accomplish triple aim(Care, Health and Cost), organizations that are adopting emerging care delivery models(ACO, PCMH etc.) will be required to support complex clinical workflows requiring to use increased interoperability in the near future. Security and User Management

Datashop platform has powerful built-in security features. All data at rest and being transmitted is encrypted using industry-standard algorithms and transferred only via secure channels. Audit logs are maintained for every point of entry/exit and access of data. The platform has fine-grained role-based user-access controls for every feature and data cell, along with a dashboard can allows system administrators to see exactly what access the different users of the system have including turning PHI or PII masking on or off for a particular user. The server is housed within the host network. The system is regularly backed up securely so that the system can be restored from an incident. The system also follows guidelines laid down in the HIPAA Title-II for safeguarding of healthcare data.
