DIRECTORY

This booklet contains important information that will assist you in pre-registering your child for public schoolbus service for the 2015 - 2016 school year. The DOE strongly encourages current bus riders to pre-register for bus service next year by completing the school bus pass application form on Page A1. Pre-

registration application forms will be accepted through June  03,  2015 (new bus riders may register at theirconvenience).

Eligibility

Students in Grades K-5 must reside a mile or more from the school within their home attendance area andstudents in Grade 6-12 must reside 1.5 miles or more from the school within their home attendance area toqualify for regular school bus service. Regular school bus service is limited to transportation from thehome to school and return.

Students who do not attend the school within their home school attendance area; reside within the minimumqualifying distances as listed above; or desire other than home-school-home transportation  may receive “spaceavailable” bus service provided that there are unused seats on the bus and that accommodation will not result inadditional cost to the State.

Parents who desire space available bus service must submit a Request for Student to Ride School Bus on aSpace Available Basis (Form CS-101, available at the school office) along with their completed bus passapplication form (Form ST-70, located on page A1 of this booklet). Please submit both forms to the schooloffice. CAUTION: Approval/disapproval may take up  to four  or more  weeks. Your child will  NOT be permitted  toride the bus until your application is approved. Parents must make contingency plans during the interim.

PaymentPlans

All bus riders (except those who are eligible   and   approved for free bus service) must pay the prevailingtransportation fee. We offer three payment-plan options:

1. Annual Payment Plan – one-time discounted payment of $270 roundtrip or $135 one-way2. Quarterly Payment Plan – up to 4 discounted easy payments of $72.00 roundtrip or $36.00 one-way3. One-Way Bus Coupons – sold in sheets of ten coupons for $12.50 per sheet (cash only)

Students who intend to use the public school bus system as their primary means of getting to and/orfrom school daily throughout the entire school year are encouraged to select the ANNUAL paymentplan.

Students who intend to use the public school bus system as their primary means of getting to and/orfrom school daily but whose transportation needs might change from academic quarter to academicquarter are encouraged to select the QUARTERLY payment plan.

Students who intend to use the public school bus system only occasionally, and students who requirepassage on a bus route other than their assigned  bus route are encouraged to purchase One-way BusCoupons to mitigate the financial burden.

IMPORTANT: The DOE has a FIRM “No Refund” policy on all school bus pass purchases.

The DOE urges you to consider your child’s transportation plans carefully before remitting payment inrecognition of the no refund policy.

IMPORTANT: Please select your payment plan carefully!

Although you will be allowed to change your plan if/when needed, you will not receive a refund of the unusedportion of the bus pass under the DOE’s “No Refund” policy.

You can check on the status of  your child’s application form by visiting our School  BusTransportation website at http://iportal.k12.hi.us/SBT/home.aspx or by calling the school office.

Once your application is approved, you will be asked to remit your payment either in person or by mail at theschool office (please do not mail in cash payments). All payments are due prior to the first day that your childintends to use the bus service.

Bus Stop and Bus Route Information

The Department of Education is in the process of implementing bus routing software to design efficient schoolbus routes on Oahu,  the  Big  Island  and  Molokai. Previous  bus route  schedules  and/or bus stop  locations  maychange  as  a   result. Except   for  cases   involving  significant  safety   issues  or  special  needs  students,  curb-to-curb  service  will  not  be  provided.    Instead,  bus  stops  will  generally  be  located  at  the  intersections  of  public  roadways,    Again,  please  be  aware  that  bus  stop  locations  may  change  year  to  year  and  during  the  year.    Families who pre-register their  child for 2015-2016 school bus service before June  3,  2015 are  asked  to  check  with   their   schools  one   to   two  weeks prior to the   first day of school   for   the  2015-16  assigned  school  bus  stop   location  and  pick-up/drop-off  times. If you do not pre-register your child, you will be given your bus route information when youapply. As the school year progresses, additional changes to  bus schedules may become necessary. Parentscan expect to receive written  notification of bus  schedule changes a minimum of three days in advance.

New   riders   on  Maui   and   Kauai  may   contact   the   school   office   or   bus   company   office   for   bus   stop   times   and  locations.
