Jing Fang Treatment of the Infectious Pneumonia Coronavirus

February 8th, 2020.

As of February 5th (midnight), 31 provinces have reported a total of 28,018 confirmed cases

(34866 on 8th). 1,153 cases are cured and discharged, 24,702 cases are receiving treatments with

moderate symptoms, 3,859 cases are in severe and critical conditions and 563 have died (724 on

8th). There are currently confirmed cases in 28 countries around the world. It took the Novel

Pneumonia Coronavirus (COVID-19) 48 days to infect the first 1,000 people. It took SARS 130

days to infect the first 1,000 people. It took MERS 2.5 years to infect the first 1,000 people.

Important note: This information is not to replace the information given by the National

Health Commission of China but it is used as a reference for medical practitioners only. If

you have symptoms of cough or fever you must consult with your doctor. Please take care of

the usage of Ma huang in the listed formulas with patients with heart and kidney diseases.

In the Jing fang system from Zhang Zhong jing within the Hu Xi shu lineage, we must analyze

the constantly and rapidly changing symptoms with the 8 guiding principles to evaluate the

location and type of the pathogens in the body and the levels of blood and Zheng qi.

This novel pneumonia Coronavirus has an incubation stage and directly attacks the immune

system where blood tests will show low white blood cell count and low lymphocyte count. The

patients are contagious at the incubation stage. After the 2 to 14 days of dormant stage, the

symptoms evolve quickly and eventually cause organ failure of mainly the lungs, kidneys and

heart. Patients mainly die of respiratory or heart failure due to fluid retention. The virus has

also been seen to go back into a dormant stage after receiving treatment only to come back in

full force after a few days of observation. Both breath tests and blood tests confirm

contamination of the virus.

All hospitals are using integrated western and Chinese medicine to fight this virus and the

government issued the 5th edition of the treatment protocol for the pneumonia virus at midnight

February 5th. We have added it below for reference. Western medicine includes respiratory

assistance, CPR, and antiviral medication treatments given now consist of combining

Remdesivir (which is used against Ebola and Marburg virus), Ritonavir (which is used to treat

HIV/AIDS infections) and Zavesca (which is effective in breaking down certain lipids) with an

inhaled dose of nebulized alpha-interferon twice a day. They are also testing Aluvia, also

known as Kaletra, on patients. It is a combination of Lopinavir and Ritonavir.

In terms of Chinese medicine, we compiled important symptom differentiation, understanding

and recommended treatments from TCM doctors as well as Jing fang experts in Hubei treating

the virus now. We have identified different stages of the disease fitting different treatment

methods: Early, middle, severe, critical and recuperation stages. Within each stage, there are

varieties of similar symptoms also needing modified formulas. We have also added some

recommended formulas as a preventative step and formulas used in the beginning stage with

mild symptoms to prevent the evolution of symptoms.

The following are recommended formulas shared by three Jing fang experts which went to the

Wuhan critical care unit to help treat patients with the pneumonia virus. Dr. Tong Xiao lin (TXL)

usually practices at the Guang An Men Chinese Medicine Hospital in Beijing. Some other

formulas are recommended by Dr. Zhang Yue feng (ZXF) according to the Jing fang approach.

By evaluating the symptomes and pahtomecanism, it is determined that this pneumonia

viral infection is caused by an invasion of toxic damp cold which quickly transfers from

the external into the internal layers injuring normal organ function, and affecting the

lungs seen in cough, wheezing, and shortness of breath; affecting the stomach leading

to lack of appetite, glomus, and nausea; affecting the intestines with diarrhea or

constipation, as well as affecting the kidney leading to kidney failure. The dampness

also moves upwards affecting the mind leading to blocked orifices and runny nose in

mild cases and fainting in severe ones. The dampness stagnates quickly becoming

turbid leading to heat symptoms alternating with cold seen in the fever and sweating.

Even as the symptoms are similar we need to keep the Jing fang approach: “One patient

has one diagnosis and only fits one formula. These are only recommended formulas,

and only a qualified practitioner must make a clear diagnosis before administrating

them. As the virus evolves quickly, close observation of current symptoms is important

as the appropriate treatment for the early stage will be harmful during the later stages.

Early Stage (within 1 to 3 days of the presenting symptoms):

Symptoms: Fever with aversion to cold, and or aversion to heat,

dry cough, dry throat, extreme fatigue, chest oppression, severe

glomus and obstruction at the stomach, nausea, loose stools,

tongue body is large and pale or pale red, coating is white and

greasy and pulse is soggy.

Diagnosis: Three yang Tai yin Concurrent Syndrome

(ZYF) Recommended formula: Xiao Chai Hu Tang + San Ren Tang.

Dosages: Chai hu 15-30g, Zhi huang qin 10-15g, Sha ren 15g, Sheng

gan cao 5g, Xing ren 10g, Bai dou kou ren 10g, Sheng yi ren 15g,

Ban xia 15g, Hou po 10g, Tong cao 10g, Hua shi 10g, Dan zhu ye

Case study: Treatment of Novel Coronavirus Infectious Pneumonia in Hainan

This case was treated and shared by Dr. Wang Gao an and Dr. Qiu Shi guang of the Second

Affiliated Hospital of Hainan Medical College which is the designated hospital for patients with

suspected coronavirus infectious pneumonia in Hainan province. On the second day, after

taking only half a dose of Chinese medicine granules there was significant improvement in

symptoms.

Case 1: Mr. Jia, 53 years old (from Wuhan, Hubei)

The patient flew from Wuhan to Hainan on January 24th, 2020. After arriving at Haikou, he went

directly to his home in Ding An county for isolation and observation. After developing a fever,

he was immediately hospitalized in Hainan’s Provincial People’s Hospital. A chest CT revealed

“exudative changes in both lungs”. The first, second, and fourth viral nucleic acid tests were

negative, and the third test was mildly positive. After receiving anti-infectious treatment and

not presenting with fever he was discharged. The fever recurred on January 31st, and he came to

our hospital for isolation and observation.

After admission, he suffered from continuous fever, his temperature began to rise in the

afternoon, the lowest temperature was 37.4°C and the

highest was 39.2°C. On February 3rd, the patient still had

a fever and his symptoms did not improve.

Symptoms: Continuous high fever, each antipyretic

medication caused profuse sweating, aversion to cold

after sweating, four limb soreness and weakness,

diffused bone pains and headache, dry mouth, strong

thirst for warm drinks, no bitter taste, cough with scant

white sticky phlegm, throat itching after coughing, poor

sleep, poor appetite, BM every 1-2 days, scant yellow

urine, slightly dark red tongue with thick yellow greasy

coating. The pulse could not be palpated due to

quarantine regulations.

Chest CT analysis:

Thorax is symmetrical bilaterally.

Multiple high density flake shaped

spots and strip shaped shadows can be

seen below the pleura of both lungs, the

lower lung lobes are prominent, with

unclear margins, lung vein texture is

normal, with no abnormal distribution.

Normal trachea and bronchus

morphology. No obvious masses or

enlarged lymph nodes in the

mediastinum. No pleural cavity effusion

or thickening was observed bilaterally. Confirmed severe infection of both lungs.

Jing Fang and TCM analysis:

Diagnosis: Tai yang Tai yin Yang ming concurrent syndrome with severe body fluid depletion.

Analysis:

Tai yang syndrome: Each antipyretic medication caused profuse sweating, aversion to cold

after sweating, four limb soreness and weakness, diffused bone pains and headache.

Yang ming deficiency syndrome: Dry mouth, strong thirst, scant yellow urine.

Yang ming damp heat syndrome: Sticky phlegm, BM every 1-2 days, throat itching after

coughing, tongue with thick yellow greasy coating.

Yang ming excess syndrome: Continuous high fever, slightly dark red tongue.

Tai yin syndrome: Thirst for warm drinks, no bitter taste, cough with scant white phlegm, poor

sleep, poor appetite.

Pathogenesis: Pathogenic dampness transforming into heat, which stagnated in the lungs and

stomach leading to a heat stasis and hindering lung and stomach functions.

Classical references:

Shang Han Lun, clause 26 states: “After giving Gui Zhi Tang, there is profuse sweating, severe

vexation and incessant thirst, with surging and large pulses; Bai Hu Jia Ren Shen Tang governs.”

This clause discusses a patient with depleted body fluids, internal excessive and deficient Yang

ming syndromes.

Shang Han Lun, clause 168: “Shang han syndromes, if after applying the vomiting and purging

[methods], the syndrome does not resolve in 7 or 8 days, there is heat binding in the interior,

with heat sensations in the exterior and interior, constant aversion to wind, severe thirst,

dryness of the surface of the tongue with vexation, desire to drink numerous sheng (200ml) of

water; Bai Hu Jia Ren Shen Tang governs.”

Shang Han Lun, clause 162: “After purgation was applied, Gui Zhi Tang cannot be given. If there

is sweating and panting without severe heat; Ma Huang Xing Ren Shi Gao Gan Cao Tang can be

given.”

Shang Han Lun, clause 62: “After sweat promotion, there are body aches and deep, slow pulses;

Gui Zhi Jia Shao Yao Sheng Jiang Ge Yi Liang Ren Shen San Liang Xin Jia Tang governs.”

From the understanding of these SHL clauses, Dr. Wang combined the 3 formulas with Da Yuan

Yin (Reach the Membrane Source Decoction) from the Wen Bing school. This formula was added

to expel internal turbid phlegm in the upper and middle, clear heat, expel toxic dampness and

nourish blood. Wu You ke 吴又可 states: “Bing lang can transform and disperse dampness,

expel pathogens, treat diarrhea and dysentery, while treating southern epidemics, (zhang qi);

Hou po breaks stasis and nodules; Cao guo is acrid and vigorously moves qi and strongly

www.iczo

expels pathogens, These three medicinals work together to expel the pathogens from the

internal membranes. This is where the name of the formula comes from. Heat injures body

fluids, Zhi mu is added to nourish yin [fluids]; heat also injures the ying qi, Bai shao is added to

harmonize the blood, Huang qin clears dry heat, and Gan cao harmonizes the middle; these last

four medicinals harmonize the formula.”

“Early stage Wen yi (Epidemic diseases), first there will aversion to cold followed by heat

sensations, in the following days there will be heat sensations without aversion to cold. The

early stage will last 2-3 days, the pulses are not floating or deep and rapid. There will be heat

sensations day and night, which will be more severe at dusk, with headaches and body aches.

At this time, the pathogen is pressing in front of the spine and behind the intestines, even

though there are headache and body aches, this is caused by pathogenic heat floating outwards

to the channel, this can not be mistaken for a Shang han external syndrome, using medicinals

such as Ma huang and Gui zhi to promote sweating. This pathogen is not in the channel, sweat

promotion will damage the exterior qi, and the heat will not resolve. Purgation method is also

inappropriate, the pathogen is not in the interior, promoting purgation will damage stomach qi,

increasing the thirst. Da Yuan Yin is appropriate.”

Formula Patterns:

FP 2: Bai Hu Jia Ren Shen Tang (SHL) Yang ming Tai yin syndrome

On February 3rd afternoon, the patient half a dose of the formula in

granules, reporting that his throat felt better as soon as he drank the

formula, he did not take antipyretic drugs again and his temperature

gradually decreased.

The next morning his temperature was measured at 37.2°C, which

was the lowest since he was admitted to the hospital, in the

afternoon it measured at 37.4°C, reporting reduction of symptoms

and his vitality improved greatly. His tongue coating was less thick

and greasy. Since the patient tested positive with the pneumonia

Novel Coronavirus, he was transferred to Hainan’s Provincial

People's Hospital for further treatment in isolation.

Summary

This latest pneumonia Novel Coronavirus falls into the Chinese Medicine category of epidemic

disorders, and can be sub-classified as a “damp warm” epidemic described in the Shang Han

Lun and the Wen Yi Lun. Dampness epidemics invade the lung, causing zheng qi insufficiency.

Once the zheng qi cannot overcome the pathogen, toxic pathogens invade the interior and

transform to heat, consuming body fluids, and ying qi while agitating the blood. If the

pathogens directly transmit to the pericardium, it puts the patients in a critical condition. The

pathological characteristics remain in the interior with “dampness, heat, blood stasis, toxicity,

and body fluid deficiency”. The treatment primarily aims to support the zheng qi and release

dampness and heat interior, while avoiding harming the stomach qi and zheng qi and mildly

addressing the exterior.

Dr. Hu Xi Shu on San Ren Tang

Wen Bing Tiao Bian, clause 43: "Headache, aversion to cold, body heaviness and pain, white

tongue coating, lack of thirst, wiry, thin and soggy pulses, lusterless yellowish complexion,

chest oppression, lack of appetite, heat sensations in the afternoon, appearing as yin deficiency,

difficult diseases which resolve quickly; this is the damp warm [syndrome]. Deafness and loss

of consciousness after sweating, in severe cases, there is heaviness of the eyelids and lack of

desire to speak, purgation leads to severe diarrhea, moistening aggravates the symptoms and

prevent resolution, the treatment is the same in the summer, autumn, and winter days; San Ren

Tang governs".

Dr. Hu Xi shu commented on this clause, stating headaches with aversion to cold, body

heaviness and pain are similar to Shang han syndromes. However if the pulse is wiry and soggy

it is different from Shang han syndromes. A white tongue without thirst and a lusterless yellow

complexion are not caused by summer heat syndrome, but they actually occur due to internal

toxicity.

Chest oppression with lack of appetite demonstrates an obstruction of dampness preventing the

flow of clear yang. “Afternoon heat sensations, appearing as yin deficiency,” reflect a yin

pathogen, dampness, which invades the interior (yin areas) with a sticky and greasy quality,

preventing the natural function of the organs. This is different from cold pathogens which

resolve by sweating of the Shang han syndromes.

Normally, the external Shang han syndrome by promoting sweating which damages the internal

healthy fluids. However since these symptoms are caused by internal dampness stagnation, the

warm acrid exterior releasing medicinals will steam the internal dampness and leads to an

upward movement seen in the symptoms of chest oppression and lack of appetite, or affecting

the heart and head causing loss of consciousness. The dampness moving upward also affects

the orifices and causes deafness, heaviness of the eyelids, and the disinclination to speak. In this

patient, it has also caused shortness of breath, wheezing and lack of appetite.

Dr. Hu warns us not to strongly promote sweating without tending to the internal dampness to

prevent from aggravating the symptoms.

When encountering fullness of the middle with lack of appetite, practitioners will often purge

the middle. However, this damages the internal fluids and strongly inhibits digestive functions,

increasing the retention of dampness leading to explosive diarrhea which further depletes the

body fluids and weakens the patient.

In these cases, the sweating and diarrhea lead to internal dryness. The practitioner might give

moistening and nourishing herbs. This only increases the accumulation of internal dampness

and further stagnation in the body, mixing with the body fluids which further damages the

function of the organs.

Giving San Ren Tang in these cases can clear dampness from the upper and middle jiao, restore

the movement of the lung qi, restore digestion and transform the dampness by restoring organ

function. However, pathogenic dampness qi is formless and can reach all areas of the body, it is

difficult to identify and easily stagnates and becomes turbid and stagnant. The damp warm

syndrome occurs due to intermingling of pathogenic warmth with severe damp retention which

makes it difficult to treat and takes a long time to resolve. They were commonly called the

lingering syndrome in Dr. Hu’s hometown. Some patients will survive and some will not. It is

important to remember that since the syndrome occurs due to turbid dampness it will be

treated through the middle.

Dr. Hu On Infectious Pneumonia

After reading many cases of Dr. Hu treating and discussing infectious pneumonia, four

formulas repeat themselves:

1. Ma Xing Shi Gan Tang modifications: When the disease presents as fever with aversion to

cold, wheezing and shortness of breath, lack of sweating or incomplete sweating, night

sweating.

2. San Ren Tang modifications: When the disease has moved in the interior with dampness

blocking the lung, stomach and intestines. San Ren Tang not only dries dampness but

also expels fluids, used for treating water retention below the heart, with panting and

wheezing, chest and stomach fullness, cough and shortness of breath and is effective in

treating damp warm external syndromes without promoting sweating.

3. Xiao Chai Hu Jia Shi Gao Jie Geng Tang: When the disease has reached the half-external

half-internal layers, presenting as alternating fever and chills with excessive fever,

irritability, nausea, lack of appetite, sweating and constant sweating with chest
