Learning Levels To help you more efficiently plan your time, the content of the education sessions is categorized into two levels of expertise: • Level 1: Baseline knowledge of the subject area is required.

The information presented will be a primer on the topic and appropriate for a wide range of pharmacists and pharmacy technicians.

• Level 2: Experience with the topic is recommended, but advanced expertise or specialty practice in the subject area is not necessary. These educational offerings will focus on expanding the learner’s knowledge and application of the new and/or more complex information.

Activity Types Activity types are designed to give attendees an idea of the intended outcome of a continuing pharmacy education (CPE) activity. • Knowledge-based activities are designed to enhance or add

to pharmacists’ or technicians’ knowledge of a topic; mostly didactic sessions.

• Application-based activities provide pharmacists and technicians with opportunities to apply their knowledge, usually through case studies or other application activities.

• Practice-based activities are longer, more comprehensive activities of at least 15 hours that incorporate knowledge, skills, and performance. These activities are also known as certificate training programs.

CONTINUING PHARMACY EDUCATION INFORMATION

Clinical Patient CareCDC and VA/DoD on the Offense: How Are We Addressing the National Opioid Epidemic?1400–1500Baltimore 1-2Level 1Activity Type: Knowledge-basedACPE# 0202-0000-16-158-L04-PACPE# 0202-0000-16-158-L04-T0.1 CEUsPain affects more Americans than diabetes, heart disease, and cancer combined. Of those patients presenting to physician offices with noncancer pain symptoms or pain-related diagnoses, an estimated 20% receive an opioid prescription. Between 2007 and 2012, opioid prescriptions per capita increased 7.3%, with sales of opioid pain medication and opioid-related overdose deaths increasing in parallel. From 1999 to 2014, more than 165,000 Americans

died from overdose related to opioid pain medication. In an effort to address the opioid epidemic, the Centers for Disease Control and Prevention (CDC) developed the 2016 Guideline for Prescribing Opioids for Chronic Pain. Similarly, the Department of Veterans Affairs (VA) is undertaking an Opioid Safety Initiative and will soon publish, with DoD, an updated VA/DoD Guideline for Chronic Opioid Use. This session will discuss the epidemiology of opioid use and abuse in the U.S., review the CDC Guideline for Prescribing Opioids for Chronic Pain and the VA pain-related initiatives, and describe opioid risk assessment and stratification tools for use in patients on chronic opioid therapy.Speaker(s): Mitchell Nazario, PharmD, CPE, West Palm Beach VA Medical Center

Implementing Patient-Centered CareMedication Synchronization: Is It Time to Offer This Program?1400–1500Annapolis 3-4Level 1Activity Type: Knowledge-basedACPE# 0202-0000-16-167-L04-PACPE# 0202-0000-16-167-L04-T0.1 CEUsThe treatment of chronic illnesses is a major driver of rising health care costs, and—for many people—medications are the most cost-effective treatment. Sadly, patients often miss doses, fail to refill prescriptions on time, and even stop taking their medications without consulting a health care professional. In November 2014, the Pawhuska Indian Health Center Pharmacy leveraged its relationship with patients to improve adherence and provide quality care by developing and implementing a Medication Synchronization Appointment-based Model—the first of its kind for the Indian Health Service (IHS). The program is designed to provide patients a service that allows them to obtain their

ongoing prescription refills at the same time on a day convenient to them. This pharmacist-driven service eliminates the need for patients to request refills, enhances pharmacist–patient dialogue, and ensures that all prescriptions are in the hands of patients before they deplete their current medication supply. This session will discuss the medication synchronization program at the Wah-Zha-Zhi Health Center Pharmacy.Speaker(s): CDR Carl Murray USPHS; LCDR Donnie Hodge USPHS

Benefits of DOACs are their major pharmacologic advantages over vitamin K antagonists (e.g., warfarin), including rapid onset/offset of action, predictable pharmacokinetics, and elimination of the requirement for regular coagulation monitoring. Disadvantages of DOACs include concerns about medication adherence without laboratory monitoring; uncertainty about dosing in some populations (e.g., patients with renal dysfunction, advanced age, or obesity); higher drug costs compared with warfarin; lack of specific antidotes; and absence of assays to measure drug levels at most centers.Speaker(s): MAJ Victoria O’Shea MS USA

Building on basic pharmacy technician skills, a clinical pharmacy technician performs functions ranging from administrative tasks to direct patient care to data management. The Army is taking steps to solidify the important contributions of clinical pharmacy technicians by creating a standardized position description and adding a clinical pharmacy technician fact sheet and competency-based orientation to the Army Medical Home Roles and Responsibilities Reference Manual. This session will focus on the role, responsibilities, and contributions of clinical pharmacy technicians in the Army Medical Home.

While technology has contributed to rapid advancements in health care delivery, and adding pharmacists to the medical home team has resulted in safer and better quality care, a negative safety culture across the industry continuum acts as a barrier to transforming the health care system into a highly reliable organization. This session will discuss advancements in and barriers to achieving a positive patient safety culture, including the importance of human factors engineering and improvements in communication. Several decision support tools will be provided to model a positive patient safety culture.Speaker(s): Bruce McIntosh, PharmD, VA National Center For Patient Safety

Heart Association released new guidelines on the treatment of dyslipidemia. Recently, ACC also released an expert consensus statement discussing the role of nonstatin therapies. Untreated dyslipidemia increases the risk of cardiovascular disease by almost double. With more than 70 million American adults currently having elevated LDL cholesterol, appropriate treatment of dyslipidemia is critical. The role of statin therapy is firmly established; however, the role of nonstatin therapies and the new medication class, PCSK9 inhibitors, is not as clear, and the place in therapy of these agents is questionable. This session will provide attendees with data on nonstatin therapies and their potential role in therapy.
