Can you believe we’re at thebeginning of another year?

At Caldwell Hospice andPalliative Care, we are takingstock of our accomplishments in 2012 and beginning the workfor 2013.

Reflecting on the changes to our Board of Directors, we aregrateful for the tireless dedica-tion of retiring members InezJones, Janet Wilson, and ParkerWilliamson, who accounted for a combined 67 years of Boardexperience.

We welcome Dottie Metzger,Charles Shell, and Linda Story to the board (see “BoardSpotlight” on page 11 for intro-ductions to each one), and we’reexpecting an explosion of newideas and different perspectivesfrom them, as well as a deepcommitment to maintainingCaldwell Hospice’s place as aproven, compassionate leader in end-of-life care.

We instituted a wellness programfor our staff, recently. All of usare learning to eat better, fitexercise into our daily lives, andreduce stress. It makes goodsense that we can give bettercare to our patients and theirfamilies when we feel strongerand more rested. Wellness alsohas a positive impact on “thebottom line,” as our insuranceprovider offers better rates whenwe have healthier staffmembers.

We completed months of prepa-ration for the re-accreditation

survey by the AccreditationCommission for Health Care(ACHC). The surveyor arrivedthe week before Christmas toreview documents, interviewpatients, families, staff, andvolunteers. Before leaving, thesurveyor complimented ourfacilities, our helpfulness, andour ability to keep a “warm,small hospice atmosphere” aswe grow. We were instrumentalin developing this method ofrating hospices for their qualityof work, and we were amongthe first hospices to earn ACHCaccreditation in 1998. We’vebeen re-accredited for eachthree-year period since then.

I’m looking forward to mysecond term as a Boardmember for The CarolinasCenter for Hospice and End-of-Life Care, the association forhospice providers in North andSouth Carolina. It will be agood opportunity to participatein strengthening hospicesacross the Carolinas.

As all of us make our “to-do”lists and get busy with our 2013goals, let’s never be too busy tomake time for good experienceswith our families and friends.

Caldwell Hospice Does Its “Homework”aring for hospice patients begins, most often, in their

homes because that is where mostpeople would prefer to spend theirfinal days and months.

When Caldwell Hospice’s teammembers (physicians, nurses,nurse practitioner, medical socialworkers, certified nursing assis-tants, chaplains, or volunteers)enter a patient’s home, theyexperience the patient’s situation;they enter as guests, and veryoften they leave as “adoptedfamily.” Finally, they prove the truthof Caldwell Hospice’s desire to be“companions for the journey.”

In the living room or at the kitchentable, team members listen tofamily conversations, pet the dogor cat or bird, and connect withthe family more personally than ina hospital or an office visit.

“Families want to welcome us intotheir homes, show us their familyphotos, tell us about their militaryexperiences,” Dr. Ray explained.“They can say, ‘This is who I wasbefore I became ill; this is my life.’”

“My years as a family practicephysician allowed me to understandpatients and their families as awhole unit,” added CHPC AssociateMedical Director Dr. DenniceHerman. “Visiting hospice patientsin their homes brings a morepersonal dimension to their care.”

For CHPC physicians, caring forhospice patients is a shift in direc-tion from what is taught tradition-ally in medical school. Drs. Rayand Herman aren’t trying to curetheir patients.

“One of the most important thingsI have learned is that patientsreally do want to know the truthabout their illness and prognosis,even if it is grim,” said Dr. Herman.“It is a natural reaction for familymembers, and even doctors, towant to protect the patient so asnot to destroy hope, but patientsseem more relaxed and comfort-able once the truth is out in theopen. They can express their lastwishes and hopes.”

One of the most important advan-tages of hospice care is thateveryone can take more time—to sit and visit, to listen and learn,to grow relationships with patientsand their families and their pets.Visits aren’t hurried, and patientsaren’t “numbers” or “cases.”

“What might seem to be ‘littlethings’ make a big difference in

people’s lives,” Dr. Ray said. Herecalls a caregiver who receivedconfirmation from Dr. Robert Belk,former CHPC medical director whoretired in 2011, that she was doingeverything right for her husband.

“Seeing the patient and the caregiverin their home,” Dr. Ray said, “allowedDr. Belk, as a medical expert shetrusted, to reassure the woman. Shecalled it a ‘blessing.’”

Caldwell Hospice nurses andmedical social workers watch andlearn, during visits, how to help eachpatient live fully and pain-free, toresolve emotional, financial, orfamilial issues, and to prepare forthe end of life. They try to uncoverspecial wishes. Some requests canbe fulfilled as simply as calling onCHPC volunTEENS to do yardworkat patients’ homes or arrangingwith special volunteer hairstylists to do a shampoo and style.

Some requests take more effort.Social workers have arranged forpatients to speak with, and some-times visit with, family membersfrom whom they’ve been separatedfor years.

“We can identify problems soonerand solve them before they get toobig, provide assistance, reaffirm thevalue of what caregivers are doing,”Dr. Ray said. “We provide hands-oncare; we listen and support.”

“We gain a better idea of how toprovide care,” CHPC MedicalDirector Dr. Thomas Ray said. “Apatient’s independence or access to his community can be compro-mised by his condition. If we seethis during a home visit, we canorder medical equipment, such asa wheelchair, which will allow himto leave his bedroom. Or we canfind an agency or a communitygroup that might build a ramp andmake it easier for him to gooutside his home.”

Cathy Swanson Elected to The Carolinas Center Board

Cathy Swanson has been electedto serve, for the second time, onthe board of directors for TheCarolinas Center for Hospice andEnd-of-Life Care. Caldwell Hospiceand Palliative Care’s chief execu-tive officer will join three othernew board members: Lynn Hardy,Carolinas East HomeCare andHospice (NC); Jean Eckert,Hospice of the Carolina Foothills(SC), and Dr. Jeff Seymour,Hospice and Community Care (SC).

Named Caldwell Hospice’s execu-tive director in 1990, Cathy’sbackground as a medical socialworker is central to the organiza-tion’s focus on caring for thewhole patient, including familymembers. Since 1990, CaldwellHospice’s average daily censushas grown from 17 to 147, and the staff has increased toapproximately 110 permanentemployees.

“I look forward to active involve-ment on the board,” Cathy said.“We must maintain the highestquality of care, even in the midst ofincreased regulations and greaterscrutiny of the work we do.”

The Carolinas Center “representsover 100 hospice providers in Northand South Carolina and providessupport to local hospice programsthrough technical assistance, education, and advocacy in statelegislatures, state regulatory bodies,and Congress. Hospice care iscurrently available in every North and South Carolina county” (source:www.cchospice.org).

Cathy served on TCC’s board,previously, from 1996 to 1999.Her dedication to ensuring qualityend-of-life care also led Cathy toa role in creating a hospiceaccreditation program through the Accreditation Commission for

Health Care(ACHC).

ACHC is now a recognizednationalaccreditationorganization for medicalproviders. Itoriginated inNorth Carolinaand now reviews more than1,000 locations nationwide.Cathy served on the ACHC board,from 1995 to 1998.

Cathy earned a Master of PublicHealth degree from UNC-ChapelHill and a Bachelor of Science inSocial Work degree from WesternCarolina University; she holds aHospice Administrator Certificateand a certificate in Non-ProfitManagement from DukeUniversity.

Hospice and Palliative Medicine Certification

Caldwell Hospice and PalliativeCare announces the achieve-ments of two members of itsmedical team.

Dr. Dennice Herman, associatemedical director, and Dr. RameshKrishnaraj, weekend physician,earned Hospice and PalliativeMedicine certification, recently.The doctors sat for the exam inCharlotte on October 5 in agroup of approximately 20 physi-cians.

“The day-long examination wasvery difficult,” Dr. Herman said,“and it required considerablepreparation and study.”

Dr. Krishnaraj added that theexam was challenging for himbecause he works in hospice carepart-time.

Drs. Herman and Krishnaraj joinCaldwell Hospice Medical DirectorDr. Thomas Ray as Board-certi-fied Hospice and PalliativeMedicine physicians.

“The effort required of our physi-cians in gaining certification is astrong reminder that we focus onpatient care,” said CHPC ChiefExecutive Officer Cathy Swanson.“It solidifies our position as aleader in end-of-life care for thisarea.”

A 1986 graduate of East CarolinaUniversity’s School of Medicine,Dr. Herman earned a bachelor’sdegree in nursing from theUniversity of North Carolina atChapel Hill in 1972. From 1998until she joined CHPC’s staff in2011, Dr. Herman practicedfamily medicine at South ForkMedical Park. She also practiced

emergencymedicine atCaldwell MemorialHospital for sevenyears and served asthe medical directorfor CaldwellPhysicians Networkand for ShaireCenter, in Lenoir.

Dr. Krishnarajgraduated from theUniversity of NorthCarolina at ChapelHill’s School ofMedicine in 1997.He holds aBachelor’s degreein Political Science(1991) and Masterof Public Healthdegree (1998) fromUNC-Chapel Hill. Heis a medical staffphysician in Geriatric andSpecialty Services at BroughtonState Hospital in Morganton, NC.

Dr. Ray earned his Hospice andPalliative Medicine certification, asa member of the first certificationclass after Hospice and PalliativeMedicine was accepted as amedical sub-specialty. His certifi-cation will extend throughDecember 2018.

Dr. Ray graduated from medicalschool and completed hisresidency at the Medical Collegeof Virginia. He practiced physicalmedicine and rehabilitation, priorto coming to Caldwell Hospice in2007, when he realized thatproviding hospice and palliativecare was “where I’m supposed tobe.”

Dr. Ray’s approach to caring forhospice and palliative carepatients combines his medicalskill in addressing physicalproblems and his compassionateconcern for them: “You may notcure, but you can alleviate painand distressing symptoms.”

“I enjoy serving the community,and Caldwell Hospice allows meto do that,” Dr. Krishnaraj said.“It is the most rewarding workI’ve ever done. To providehospice care, you have to haveempathy, not just sympathy.”

This is the last year the exam can be based only on workexperience in hospice care; in thefuture, Dr. Herman said, theAmerican Board of MedicalSpecialties will require one yearof additional formal training inaddition to work experience forHospice and Palliative Medicinecertification.

“The need for hospice and pallia-tive care specialists will grow inthe coming years, and the supplyof those specialists will not matchthe demand,” Dr. Herman said.“It is essential that we beprepared to provide end-of-lifecare and pain management.”

Drs. Herman and Krishnaraj’sHospice and Palliative Medicinecertification remain in effectthrough December 2022.

VOLUNTEER NEWS

VolunTEENS Bake!Sisters Megan and Ashley Lawsand Kristen Beach are among thededicated volunTEENS whobegan a new activity at theForlines Patient Care Unit(PCU) with patients a coupleof months ago. They come to thePCU’s family kitchen on Tuesdayafternoons and bake cookies withpatients who are able.

“These teens took the idea andran with it,” said VolunteerCoordinator Brittany Bonn.

Emma Adams, Landyn James,and Macie Barlow are othervolunTEENS who help patientsslice cookie dough into quarters,place onto baking sheets, wait forthe sweet scent to fill the room,and taste their success. Once the“baker patients” have had theirfill, volunTEENS carry plates ofcookies throughout the ForlinesPCU to share with other patientsand their families.

“Real sugar cookies made fromscratch” was a recent request. Thenext week, volunTEENS broughtsugar, flour, vanilla, Crisco, andeggs. The group got to workmixing dough, forming it intoballs, and baking sugar cookies.

The patient who wanted realsugar cookies didn’t feel well

enough to leave her bed, so thenurses and certified nursing assis-tants wheeled her bed into thefamily kitchen, and she helpedbake cookies.

“Hosting activities for patients isjust part of what volunTEENSbring to us, along with smilingfaces and lots of energy,”Volunteer Support SpecialistCrystal Burch said. “I am gratefuland proud that they are servinghospice patients and families.”

The volunTEENS need very littlesupervision, as they plan how tocelebrate holidays and otherspecial days: for Valentine’s Day,they will make iced cupcakes.

“Will we color Easter eggs againthis year?” a volunTEEN asked.

“Absolutely,” CNA Juanita Carltonanswered. “Hospice care is allabout doing the little things tobrighten someone’s day.”

Janice Stout, a Gateway Nursing and Rehabilitation Center licensedpractical nurse, was named Caldwell Hospice’s Fall 2012 Above andBeyond award recipient.

”She is very compassionate to patients at the end of life and theirfamilies,” wrote a CHPC staff member in nominating her. “When apatient is in distress, Janice calls to inform us about the changes.”

“I love my patients all the same,” Stout said. “And I am surprisedbut glad to receive this honor.”

Caldwell Hospice employees and volunteers can nominate staffmembers of area long-term-care facilities who encourage qualityend-of-life care for resident-patients and go “above and beyond” tosupport resident-patients, their families, and Caldwell Hospice staffmembers. The award is presented each quarter.

CHPC Board Introduces Three New Board MembersDottie Metzger (pictured left in photo) retired from CaldwellHospice in February 2012 as itslongest-tenured employee,concluding a 27-year hospicehomecare nursing career. Herpatients, their families, andpeople throughout CaldwellCounty cared about her. TheNews-Topic readers voted herCaldwell County’s Best Nurse, in2005 and again in 2008. Herphilosophy of hospice care is “tokeep patients comfortable, givethem the dignity they deserve,and give them the opportunityto die at home.”

When Dottie went to a patient’shome, she might have been afriend dropping by, except thatshe gathered necessary medicalinformation and learned essen-tial details during the visit. She combined medical skill, aprofessional focus on patients’well-being, and the special qualities that made her visitsfeel personal.

“Dottie carries skill, respect, andher stethoscope on each patientvisit,” said Caldwell Hospice ChiefExecutive Officer Cathy Swanson,at Dottie’s retirement. “There isno way to measure her value toour patients or to our staff.”

After studying nursing at theLutheran Hospital School ofNursing in Baltimore, Dottieworked at the Lutheran Hospitalemergency room for five yearsbefore moving to Lenoir. Shecame to Caldwell Hospice as ahalf-time nurse, before extend-ing her hours into 27 years.

Dottie’s retirement focuses onvisiting with her four childrenand their families, includingthree grandchildren, bakingcakes—another of her talents—and crocheting, and teachingCPR to Caldwell Hospice staffmembers. When asked to partic-ipate on CHPC’s Board ofDirectors, “I was honored,” Dottie

said. So isCaldwellHospice.

Charles Shell(picturedcenter inphoto) hasbeen anemployee ofRPM WoodFinishes Group,Inc., for 13years; hismost-recentassignment is as the company’sController.

Charles and Melanie Shell, hiswife, believe in the importance of “giving back wholeheartedly,as God continues to bless us.”

He has known about hospicecare, personally, throughmembers of his family. He hasbeen aware of the mission andthe work that Caldwell Hospicedoes through other members ofFirst Presbyterian Church, Lenoir.When approached about servingon the Caldwell Hospice Board ofDirectors, Charles said,“I feltvery honored that you wouldthink of me.” As a Boardmember, Charles hopes to be apart of Caldwell Hospice’s “spiritof love and family that willcontinue for many years.”

When her husband went toVietnam in 1970, Linda Story(pictured right in photo) left ajob at the GE plant in Hickory fora job with the Town of GraniteFalls, for a very practical reason:to be closer to home. By thetime she retired, with nearly 40years in local government, theTown’s population had grownfrom 2,000 to 5,000; Linda hadbeen named Granite Falls’Woman of the Year twice (1983and 2009); she had served theTown’s best interests whilemeeting county, state, andfederal government regulations(not an easy task); she had

served on Granite Falls’ HistoryCommittee, which had come intobeing around the 1999 centen-nial observance; and, as part ofCaldwell County’s EconomicDevelopment Commission, shehad worked to bring in newbusinesses and retain jobs inhard economic times.

In retirement, Linda planned tospend more time with her threegrandchildren, read, travel withher husband, and volunteer forCaldwell Hospice. So far, shehasn’t found enough time forreading and going to the beach,but Caldwell Hospice has found away to utilize her talents—byelecting her to serve on itsBoard of Directors.

Linda’s strengths include 13years as Granite Falls’ FinanceDirector, management skillshoned during 26 years as TownManager, and problem-solving:she tries to see all sides andmake the best decisions for thelong term. Add in her passion to help people—she works withseniors groups at Dudley ShoalsBaptist Church and visits theelderly in the congregationweekly—and you see the contri-butions that Linda will make.

“I’m a person in the communitywho is willing to give time andtalent to causes I believe in,”Linda said. “At this time in mylife, it’s about helping others.”
