  • Procedure for filling Lokayukt Form

    Following are the steps for filling Lokayukt Form

    HR Module Empl. Services Lokayukt Lokayukt Form

    Lokayukt Form is to be filled by all employees. If you have already filled the information earlier,

    it will be available in Lokayukt Old Filled Data. You may view the same and refill again as per

    the new format.

    After clicking on Lokayukt form a screen will appear which will display the information of the

    employee.

  • It will also contain a list of various Forms with short description to be filled by the employee.

    The form has following features :-

    1. View This will help to view the contents.

    2. Fill / Edit This option will allow the employee to add the information and also to edit

    the already filled information.

    3. Save As Draft facility is given for partial filling of the form at a time.

    4. Once the employee is satisfied that all the information is correctly filled then only the

    employee should TICK ( indicated with black circle).

    5. After ticking the Save and Submit button will be activated.

    6. Once the Save and Submit button is clicked form is SAVED permanently and editing

    will not be possible.

  • On clicking Form-I a screen will appear having the details of Self and dependents. If all

    dependent children are not appearing then you may declare the children using Dependent

    Declaration screen in HR Module --- Empl. Services --- LTC --- Dependent Details ---

    Dependent Declaration. After approval by Establishment the Dependents will appear in this

    screen. In this screen employee can fill whether he/she is going to fill the details of the spouse

    and dependents or they are going to fill it separately. If husband and wife both are NPCIL

    EMPLOYEES EACH HAS TO FILL SEPARATELY. After filling the information employee

    can click on Save Form.

    Clicking on CLOSE Form will always take the employee to the 1st screen having List of forms.

    On clicking Form II following screen will appear.

  • For filling the information Cash In Hand may chosen and Amount can be filled. NIL may be

    chosen for no specific declaration.

    This may be done for all the dependents declared in Form-I.

    There are 6 screens available for filling various sub-heads in Form-II having the same features.

    Sr No. HEAD_NAME

    1 Cash and Bank Balance

    2 Investments and Deposits

    3 Personal Loans / Advances Given

    4 Motor Vehicles

    5 Jewellery

    6 Any Others Assets

  • For each head there are sub-heads as well.

    You may fill each sub-head for each dependent.

    If you do not have any details to declare then chose NIL, OTHERWISE system will force it to

    NIL FOR EACH GIVEN DEPENDENT.

    For updating any information you may click on symbol indicated by the circle.

    Then the information in the row will be available for modification. After modifications are done

    click on UPDATE button.

  • The Dustbin Symbol (indicated by black circle) can be clicked for Deleting the information in

    that row.

    The Last screen is for Other Assets.

    On completing the form employee can click on FINISH. With this it will go back to the 1st

    screen of List of Forms.

  • Form - III

    This form is similar to Immovable property.

    Already approved immovable property details having available information will appear in the

    screen. You may have to fill remaining details and then SAVE. Also you may add / remove

    property details, if any.

  • Form No. IV

    Loans/ Advances availed from NPCIL as well as from outside agencies may be declared in this.

    IMPORTANT POINTS

    1) Last date for submission of the form is 30th April 2015.

    2) The options in which the data is to be fed subsequently Nil is to be chosen against the

    option otherwise system will force it to NIL FOR EACH GIVEN DEPENDENT.

    3) In form II there are number of forms. Those forms are to be filled at a time.

    4) Form I is to be filled in first and Saved else remaining forms are not editable to input or

    edit.
