What we tend to haveFull capacity to share information and documents: amongst hospitals and between specialists and GPs.
Capacity to rationalize administrative processes and make the information technologies “mission critical” in healthcare sector.
Identify an interoperability standard used in the hospitals and by all healthcare professionals and make compatible, possibly uniform, the informatics structure of Hospital Information Systems.
Capacity to promote some “networks of pathology”for main clinical areas.

CRS–SISS overview
The core of the project is an ‘Healthcare Extranet’, which links professionals, social services, organizations and citizens, tracking all the events in the process of patient care and providing value added services.
The project is based on smart card technologies, granting access to the Network to both citizens and operators by their personal smart cards.

Two joint implementations
CRS (“Regional Services Card”): the personal smart card allow every citizen to access all public administration services and enabling to access to private services.
SISS (“Health Care Information System”): the regional network connect all healthcare and social providers in Lombardy and provide e-Health services for ensuring continuous care.

Healthcare document managementOutpatient examination report, Letter of Discharge, Emergency Form: digital signature, notification, display, import in professional’s EPR.Issuing and maintenance of report are in charge of the health care company providing the service.

Network Providersthey are the intermediaries between users and the regional trust that are in charge for the project implementation;they are selected and certified by the Region Lombardy;
they provide the access to the SISS Network and to Internet;
they install the “front end” software and the “SISS programs” on the operator’s Work Stations;
they supply the first level “Help Desk” and the “on site”technical support for the SISS;
they are chosen on the market and paid by Hospitals, GPs and Pharmacists.

Lessons learnt
The cost of developing the entire informatics system is a small part of the total investments: we spent for that about 70 mil €;The main economical effort is to upgrade the Hospital Information System: we have already spent for that 250 million €; The security infrastructure cost is smaller then the “Privacy” respect: we spend 200.000 € for security and 20 million € per year for “Privacy”;The training cost is less that we could wait;At present, the health At present, the health professionals accept Regional Healthcare Information System and they are using it more and more.
