A difficult case of fluorosis or internal tooth staining ona 26 year old patient that required two in-surgery powerbleaching appointments one month apart. GC ToothMousse was recommended prior to treatment to reducesensitivity often experienced during this procedure and togive an improved final result.
In between appointments GC Tooth Mousse wasapplied twice daily.

Jason is a 34 year old electrician:• Generally healthy – no medications • Moderate heartburn/gastric reflux • Smokes 15 cigarettes per day • Drinks two cups of tea per day • Bike rides 40km six days per week • Insufficient water intake• Moderate consumption of beer and wine• Lower front teeth are often sensitive• Nightime grinding of the teeth in past• Some jaw clicking on opening
Diagnosis
Jason has considerable tooth wear. The main problem is not enoughsaliva being regularly produced.Saliva protects teeth and a lackof it will accelerate problems.Dehydration has made mattersworse and insufficient waterintake during the day hascaused a fluid imbalance.
Treatment
Applying GC Tooth Mousse twicedaily and using a high fluoridetoothpaste (5000 ppm) helpedstabilize Jason’s problems.Several of Jason’s teeth werethen filled and crowned toimprove looks and function.Future review and maintenanceare essential to ensure Jasoncontinues to benefit from hishome care treatment.

Sensitivity in teeth comes from open dentine tubules,and is particularly prevalent when the patient suffersfrom gastric reflux. The reflux causes repeated exposureto gastric contents which prevents any protectiveorganic or inorganic surface film from forming to coverthe tubules.
GC Tooth Mousse exerts a rapid desensitizing effectthrough immediate protein binding followed by thedeposition of calcium and phosphate compounds withinexposed dentine tubules.
This patient required treatment for severe dentalhypersensitivity affecting many teeth. She hadundiagnosed gastricreflux and considerabledestruction of the upperfront teeth had occurredfrom the erosive effectsof stomach hydrochloricacid. The lower frontteeth displayed gumrelated erosion lesionswhich also werehypersensitive.
After recommending anightly application of GCTooth Mousse and ahome care program usinga sodium bicarbonatemouthrinse to neutralizethe gastric acid, thesensitivity problemdiminished rapidly andthe hardness of theexposed dentineincreased. The teeth werethen restored with atooth coloured fillingmaterial. Three monthslater the teeth werereviewed – they were nolonger sensitive and thepatient was comfortableand able to eat normally.

In many cases of mild fluorosis, a single non-surgicaltreatment sequence of etching/microabrasion followedby GC Tooth Mousse can achieve the desired result. Thevisual effect occurs through a slow chemical reaction,and thus patients should expect to see changes over
several weeks rather than instantly.
Darryl is completing hisuniversity studies and hislifestyle poses a number ofchallenges for effectiveremineralization. He hasregular sporting involvements
with competitive rowing,which places him at risk fromdental erosion should his fluidbalance situation not be kept incheck. Darryl’s teeth have the
benefit of being formed withgood fluoride exposure (in his
case from fluoride tablets), and hehas remained caries free to this point.
Because Darryl’s enamel has higher acidresistance, he should be less vulnerable
to tooth wear from erosive factorssuch as dehydration and the
intake of acidic sports drinks,although he will, of course,still be prone to wear andattrition in the normal way.There is strong evidence thaterosion is less common inpatients who have goodfluoride exposure.
