7. SUMMARY OF CV

Dr. Matteo Trimarchi is senior physician at the Department of Otorhinolaryngology at San

Raffaele Hospital in Milan, and contract professor of the Specialization Course in

Otorhinolaryngology of San Raffaele University in Milan. He obtained his medical degree

from the University of Pavia with high honours in 1996, and specialized in

Otorhinolaryngology at the University of Brescia in 2001.

He has carried out training at various institutions in Italy and abroad (including S. Thomas

Hospital and Guy’s Hospital in London, the Department of Anatomy at Harvard University

in Boston, the Department of Otolaryngology at Mayo Clinic in Rochester and the Royal

National Throat Nose and Ear Hospital at the University College in London).

He is actively involved in several areas of research, and has a particular interest in

pathologies and endoscopic surgery of the nose and paranasal sinus, and the study of

tumours and granulomatous lesions of the head and neck. Of special interest are the

recent results from a study on midline destructive lesions from cocaine abuse.

He has authored more than 70 publications in national and international journals and 2

book chapters, and participated in creation of a data storage system for patient data and

images for neoplasms of the sinonasal tract (NSNT ver 1.0).

He is a member of the American Rhinology Society (ARS) since 2002, Società di

Otorinolaringoiatria Italiana (SIO) since 2007 and the European Rhinology Society since

2009 and honorary member of the Spanish Otorhinolaryngologic Society since 2011.

8. SUMMARY OF SCIENTIFIC ACTIVITY The scientific activity of Dr Dott. Matteo Trimarchi can be grouped into several main areas: rhinology, laryngology and neck surgery, vestibology and radiology.

1- Rhinology In collaboration with the departments of Nephrology, Anatomic Pathology, Radiology and Dentistry of the University of Brescia and the Thoracic Diseases Research Unit of the Mayo Clinic two groups of patients have been analyzed, one with cocaine-induced midline destructive lesions (CIMDL) and the other with Wegener’s granulomatosis. The most relevant finding was the positivity for the ANCA test (anti-neutrophil cytoplasmic antibody) in a high percentage of cocaine abusers; in 4 cases, the pattern of positivity was identical to Wegener’s granulomatosis. However, the ANCA test in cocaine abusers presented with some atypical characteristics compared to Wegener’s granulomatosis: prevalence of a perinuclear-ANCA (P-ANCA) pattern, association of the P-ANCA pattern with proteinase 3-ANCA (PR3-ANCA) and the complete absence of myeloperoxidase-ANCA (MPO-ANCA) despite the predominance of the P-ANCA pattern (D04, D05, D10, D13, D14, F01, F05, F06, F10, F11, F17).

In collaboration with the Mayo Clinic, it was possible to identify a typical ANCA pattern in cocaine abusers. In particular, an elevated level of elastase-ANCA (HNE-ANCA) was seen in cocaine abusers that was absent in a group of 604 control patients. The HNE-ANCA could discriminate cocaine abusers from those with Wegener’s granulomatosis, in contrast to conventional tests. (D21)

Apoptosis seems to be one of the processes implicated in the pathogenesis of destructive midline lesions. In vitro, cells cultured with cocaine showed an increase in time- and dose-dependent apoptosis. From histological preparations in 60 patients (30 cocaine abusers, 10 healthy subjects and 10 with nasal polyps and 10 with Wegener’s granulomatosis), it was demonstrated that apoptosis is frequent in nasal cells in cocaine abusers, which was not present in the other study groups evaluated (D23, F21, F22).

ANCA that bind to elastase (NE) and proteinase 3 (PR3) have been identified in cocaine abusers, although the aetiology and antibody specificity are not known. A study in collaboration with the Thoracic Diseases Research Unit of the Mayo Clinic in Rochester was carried out to determine the effect of ANCA on the enzymatic activity of elastase, to see if

these antibodies interfere with the physiological effect of secretory leukoprotease inhibitor (SLPI) and to evaluate the specificity of the antigen for both NE and PR3 ANCA in cocaine abusers. It was found that the functional effects of NE ANCA on SPLI could not be implicated in the pathogenesis of CIMDL. The response of ANCA included reactivity for PR3 ANCA in patients with CIMDL that was different from that described in patients with Wegener’s granulomatosis. (D26)

Another area of interest is the lacrimal pathways. Endoscopic treatment was used in revision surgery for failure of dacryocystorhinostomy using an external approach (7 patients). The results obtained confirmed the advantages of an endonasal approach, which in addition to being minimally-invasive; allowed identification and correction of anatomic anomalies and chronic inflammatory naso-paranasal alterations often associated with stenosis of the lacrimal pathways (D11, F04, F08, F16, F19). An endoscopic approach was also applied for treatment of 56 patients with obstruction of the lacrimal pathways using anastomosis between a nasal mucous flap with that of the lacrimal sac (F31, F33)

An endoscopic technique has also been used successively for treatment of rhinopharyngeal angiofibroma (D17, F18) and inverted papilloma (F02, F07, F09, F12). For the former, inclusion criteria for endoscopic treatment were optimal result of embolization, minimal vascularization of the internal carotid system and limited extension to adjacent anatomic sites (Stage I-II according to the classification of Fisch). For inverted papilloma, patients with massive basicranial erosion, diffuse involvement of the frontal sinus, intracranial or intraorbital invasion, or with significant scarring from former surgery were excluded. In a group of 21 patients undergoing intervention for inverted papilloma, no recurrences were observed, while persistence was seen in one patient among 15 treated for angiofibroma (D17).

In collaboration with the Urology Department San Raffaele Hospital in Milan, the effects of sildenafil (Viagra), an inhibitor of phosphodiesterase type 5 approved for erectile dysfunction. Even if the drug is generally well-tolerated, it is associated with collateral effects such as headache visual disturbances, dyspepsia and nasal congestion. The effects of sildenafil on nasal airway parameters were evaluated in a double-blind study of 11 healthy subjects without respiratory disturbances. All individuals underwent evaluation by acoustic rhinomanometry and nasal endoscopy before and after administration of either sildenafil or placebo with visual sexual stimulation. The Kruskal–Wallis test was used. After

administration of sildenafil, the minimal cross-sectional area and endonasal volume showed lower values compared to placebo. Moreover, endonasal endoscopy showed a significant increase in the volume of the inferior turbinate with sildenafil compared to placebo. This preliminary study demonstrated that, together with sexual stimulation, sildenafil increases nasal volume (D25) In collaboration with the Institute of Laryngology and Otology, Royal National Throat, Nose and Ear Hospital, London, England, software for storing patient data with tumours of the paranasal sinuses was developed (D20). The software (NSNT v 1.0) includes a database for patients with neoplasms of the paranasal sinuses, and facilitates standardization of data collection and statistical analysis. The software was freely available

online via the Annals of Otology Rhinology and Laryngology during 2004.

2 - Laryngology and neck surgery The term laryngeal “leukoplakia” has clinical relevance, and can correspond to

several histological situations with variable biological behaviour. An essential requisite for correct histological classification is that the lesion is excised “en bloc” within healthy tissue and examined by serial sections. In one investigation, each case, vital staining with 2% toluidine blue was used to indicate the macroscopic limits of tumour excision. Histological analysis revealed: hyperplasia (4 cases), acanthosis (4 cases), keratosis (5 cases), parakeratosis (1 case), laryngeal intraepithelial neoplasia (LIN) I (1 case), LIN II (1 case), LIN III (4 cases), squamous carcinoma (20 cases) and verrucous carcinoma (3 cases). CO2 laser was shown to be an extremely useful tool that provides important diagnostic and therapeutic advantages when performing excisional biopsies. (D06).

Treatment of neck carcinoma in the upper airway remains a controversial problem, even if at present there is the tendency towards surgical intervention in N0 necks where the probability of lymph node metastasis is greater than 20%. In elective surgery, every effort is made to preserve lymphatic structures that are not involved in a positive neck. The presence and distribution of lymph node metastases at various neck levels has been evaluated in a cohort of 402 consecutive patients treated for squamocellular supraglottic carcinoma with the aim of defining the optimal strategy for neck treatment. Elective latero-cervical dissection (Levels II-IV) was recommended in T2-T4 N0 supraglottic carcinomas; bilateral dissection is indicated in cases in which the lesion is not strictly lateral. In the

presence of clinical, radiological or intraoperative positivity at any level, we carry out selective dissection of levels II-V. (D16)

3 - Vestibology We have carried out a study of the effects of hypotonic and hypertonic solutions (normal value 240 mOsm) in isolated labyrinth preparations from frog. Hypotonic solutions (60-180 mOsm) were obtained by reducing the perilymphatic content of NaCl. Hypertonic (300-420 mOsm) were obtained by adding a solution of normal perilymphatic NaCl, glucose, saccharose, glycerol, mannitol and urea. The results showed that any type of receptor activity was inhibited by hypotonic solutions. In contrast, hypertonic solutions produced different effects on resting and evoked activity. The resting discharge was, with the exception of urea, constantly increased whereas the evoked responses were constantly decreased by all hypertonic solutions. Studies of variations in osmolarity in isolated preparations of semi-circular canals can provide important information about Meniere's disease (D01).

Optokinetic postnystagmus (OPN) can be considered a posthumous reaction (“after discharge”) of central nervous structures that were previously activated by optokinetic stimuli. In a group of 28 normal subjects of both sexes with an age between 18 and 28 years, OPN was evoked on both horizontal and vertical planes. The maximum values of frequency and amplitude of the evoked OPN in both the horizontal and vertical planes was noted in the first 30 seconds of the postnystagmic reaction. Mean latency of OPN was greater after vertical stimulation, while the mean duration of the postnystagmic reaction was greater after horizontal stimulation (D02, D03).

4 - Otorhinolaryngological Radiology The close collaboration with the Department of Radiology at the University of Brescia

has allowed for better understanding of the radiological characteristics of patients with Wegener’s granulomatosis and cocaine-induced destructive midline lesions (D15).
