Movement disorders: introduction
Movement disorders are broadly categorized by either an excess or a paucity of
movement on a nonvolitional basis—into hyperkinetic and hypokinetic movement
disorders, respectively. Regardless of the genetic or environmental infl uences
underlying the movement disorder, the common underlying etiology is usually an
abnormality of the basal ganglia and their connections.
The basal ganglia consist of deep nuclei with input from the cerebral cortex
through the thalamus and output to the thalamus/cerebral cortex and pyramidal
system. Within the basal ganglia are complex inhibitory and excitatory
connections allowing precise control over initiation and fine
control of movements. (See Fig. 9.1.)
Hypokinetic movement disorders
Hypokinetic movement disorders are characterized by rigidity and bradykinesia or
akinesia. Postural instability and gait disorder may be present. Parkinson disease
is the most common hypokinetic disorder; parkinsonism may be seen in a smaller
number of other hereditary, sporadic, and acquired disorders. Parkinson disease is
the prototypical hypokinetic movement
disorder.

Etiology of PD
Idiopathic PD
Increased risk of PD:
o Pesticides
o Exposure to manganese
o Rural residence, farming
o MPTP (1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine)—initially •
o synthesized and used as an illegal street drug
Decreased risk has been associated with cigarette smoking and caffeine •
intake.
Familial PD
Rare (often one family per mutation); multiple genotypes impacting
different proteins (or unknown products) with widely different function
resulting in similar phenotypes
Ten or more mutations identifi ed (e.g., PARK 1–10) on 5 or more
chromosomes
Autosomal dominant and recessive forms •
Parkinsonian features at young age, though may present late occasionally
Pathophysiology of PD
Hallmarks of PD are the presence of Lewy bodies + neuronal cell death in
the pars compacta of the substantia nigra.
PD does not develop until striatal dopamine (DA) levels drop to 20% and
substantia nigra (SN) cell loss exceeds 50%.
Functional anatomy involved in PD includes:
o Primary motor cortex
o Supplementary motor area
o Striatum (putamen and caudate)
o Globus pallidus
o Substantia nigra (SN)
o Subthalamic nucleus (STN)
o Thalamus
SN acts like an accelerator on the basal ganglia and damage results in
slowing.
STN is a brake and damage therefore causes excessive movement.
Clinical features of parkinsonism and PD
Diagnosis of a parkinsonian syndrome
Bradykinesia: slowness of initiation of voluntary movement with progressive
reduction in speed and amplitude with repetition, e.g., thumb and index fi
nger tapping. Plus at least one of the following :
Rigidity: increased tone; cogwheeling (a “catching” or ratcheting felt by
examiner when joints passively manipulated)
Rest tremor:
o May be the fi rst symptom in 75% of cases of PD.
o Twenty percent of patients never develop tremor.
o Some patients may in addition have a postural element to the
tremor—this is delayed in onset (“reemergent”), comes on a short
period after the posture is adopted.
Postural instability is not caused by primary visual, vestibular,
cerebellar,or proprioceptive dysfunction.
Features supportive of PD: ≥3 for defi nite PD

Other features of PD
Constipation: often the earliest symptom, beginning 1–2 decades in
advance of other symptoms
Anosmia: 80% PD patients have decreased sense of smell. If normal
consider PSP, CBD, or MSA.
Dystonia:
o Unusual in early disease—consider MSA
o More common after L-dopa therapy
Bladder and bowel symptoms:
o Mild urinary symptoms. Frequency, urgency, but rarely
incontinence may occur due to detrusor hyperrefl exia.
o In MSA these occur earlier and are more severe.
o Constipation is common.
Postural hypotension: mild but may be exacerbated by levodopa and
dopamine agonists.
Speech disorder:
o Hypophonia (monotonous and low volume)
o Tendency to repeat the fi rst syllable (pallilalia)
Sleep disorders:
o Restless legs syndrome
o REM sleep behavior disorder where patients act out their dreams
may precede other symptoms and diagnosis by a decade or more
Dementia. In the late stage, 20% of patients may have dementia:
o With memory impairment
o Fluctuating confusion
o Visual hallucinations
o Dopaminergic medication may compound the problem.
Differential diagnosis of PD and investigation
Differential diagnosis
Parkinsonian syndromes •
For differential diagnoses, see Table 9.1.
Essential tremor (ET) versus PD
o ET 10 times more prevalent than PD.
o ET is a postural ± action tremor. A severe postural tremor may be
present at rest but is not “pill rolling.”
o Patients with ET may also have vocal tremor; head tremor (“no–
no” or “yes–yes”).
o In PD there may be jaw tremor; leg rest tremor.
Investigations
No diagnostic test for PD. Diagnosis is made on clinical grounds.
123I-FP-CIT SPECT scan (DaTscan). Ligand binds to the dopamine
reuptake transporter protein in the presynaptic terminals. Decrease
indicates loss of striatonigral neurons. Useful in differentiating ET from
PD, but not PD from MSA and PSP.
Exclude Wilson disease if onset <50 years: •
o Serum copper, ceruloplasmin •
o 24-hour urinary copper •
o Slit lamp examination for Kaiser–Fleischer rings •
MSA patients may have degeneration of Onuf nucleus—detected as
polyphasic potentials with increased latency on urethral or sphincter
EMG. Not routinely done for diagnosis.
False positives occur in patients who have had prostatic surgery and in
occasional patients with PSP.
Sphincter EMG has a sensitivity of 0.74 and a specifi city of 0.89. •
Autonomic function tests, if MSA is in differential. Similarly, a cognitive
assessment: dementia is unusual in MSA.
MRI (see Table 9.2). •
Drug-induced parkinsonism
Depletion of presynaptic dopamine stores:
o Reserpine
o Tetrabenazine
Dopaminergic blockers:
o Neuroleptic drugs: phenothiazines (chlorpromazine),
butyrophenones (haloperidol), thioxanthines (fl upenthixol), and
substituted benzamides (sulpiride)
o Prochlorperazine prescribed for labyrinthine symptoms and
nausea
o Metoclopramide for GI symptoms
Clinical features
Tremor and asymmetry as in PD
Patients may have a mixed movement disorder with untreated
parkinsonism coexisting with:
o Orofacial dyskinesia
o Stereotypies
o Akathisia
Parkinsonism may resolve within days of drug being stopped but may take
years, especially if depot preparations have been used.
Elderly patients may be left with residual signs.

Medical management of PD
Levodopa
Levodopa therapy remains the gold standard of treatment.
Starting dose is Sinemet • ® (levodopa/carbidopa) 25/100 with meals tid.
Side effects: nausea, vomiting, anorexia—often resolve spontaneously.
Consider domperidone, 10–30 mg tid, to treat GI symptoms.
Modifi ed release preparations (Sinemet • ® CR) have no benefi cial effect
in the prevention of motor complications; however, they are especially
useful for nocturnal hypokinesia and rigidity.
Bioavailablity of the CR preparation is 70% that of the immediate release
preparations.
Dispersible levodopa preparation useful adjunct in kick-starting
immediately on wakening or in the case of sudden “offs” or during
episodes of nonresponsiveness.
Motor complications develop in 50% of all PD patients after 6 years of
levodopa therapy.
Monotherapy with DA drugs is not associated with these complications;
hence the rationale for delaying the use of levodopa
therapies in younger patients if possible.
Long-term complications of levodopa therapy
Involuntary movements or dyskinesias:
o Peak dose choreathetoid dyskinesia
o Diphasic dyskinesia
o Dystonia (painful cramp)
Response fl uctuations:
o End of dose deterioration (wearing off)
o Unpredictable on/off switching
Psychiatric:
o Confusion
o Visual hallucinations
o Delusions
o Illusions
Dopamine agonists (DAs)
DA drugs (Table 9.3) act directly on postsynaptic dopamine receptors
without the need for conversion to dopamine.
DAs have a role as an alternative to levodopa as monotherapy, particularly
in younger patients, to delay the use of levodopa and its long-term motor
complications.
In patients already on levodopa who have developed motor complications,
DAs may be used with a consequent lowering of levodopa dosage.
Adverse effects
All DAs: nausea, vomiting, postural hypotension, confusion,
hallucinations, somnolence
Domperidone, 10–20 mg tid is useful for the GI side effects and postural
hypotension (peripheral effects).
Ergot-derived DAs: ankle edema, erythromelalgia, Raynaud,
retroperitoneal fi brosis, pleural effusions, cardiac valvular disease

Apomorphine
Apomorphine, a potent D1 and D2 agonist, has poor oral bioavailability.
Given by subcutaneous (SC) injection or continuous infusion.
Indications
SC injection of apomorphine may be used in assessing the dopaminergic
response, pattern, and distribution of dyskinesias in patients on long-term
levodopa therapy.
Intermittent injections are used as rescue for severe “off ” periods in
patients already on maximal levodopa and DA therapy. Helpful in painful
“off ” period dystonias as well “off” period sphincter and swallowing diffi
culty.
Continuous infusion: consider in all patients with refractory motor fl
uctuations that cannot be managed on oral therapy and require >6
apomorphine SC injections. This form of treatment should be considered
prior to surgery.
Temporary apomorphine therapy can be considered in PD patients
undergoing abdominal surgery.
Apomorphine challenge test to assess effect
Start domperidone 30 mg tid 36 hours prior to test.
No oral anti-parkinsonian drugs for 4–6 hours before challenge
Normal breakfast
Assess baseline motor function with motor examination subscale (Section
III) of the Unifi ed Parkinson’s Disease Rating Scale (UPDRS). See
Appendix.
Time to rise from a chair and walk 12 meters is measured.
Apomorphine 1.5 mg SC administered and motor response observed for
30 minutes.
Yawning may precede motor response.
If no signifi cant response, 3 mg is administered.
Dose increased every 30 minutes up to 7–10 mg.
Positive response is if there is an improvement in UPDRS score of 15%–
20% or 25% increase in walking time
Other therapies
Anticholinergic agents
Limited role and should only be prescribed in young patients with severe
tremor and dystonia
Trihexyphenidyl (benzhexol) (2–5 mg tid) is most commonly used.
Side effects are a major drawback especially in elderly patients confusion,
cognitive impairment, nausea, dry mouth, precipitation of closed angle
glaucoma, and urinary retention.
Amantadine
Previously used in early PD to delay the use of levodopa; with the advent
of DA drugs there is little use for this indication.
New role in the management of drug-related dyskinesias due to glutamate
antagonistic properties.
Dose 100–300 mg/day.
Side effects: confusion, hallucinations, ankle edema, livedo reticularis,
insomnia (second dose at midday)
Selegiline
MAOI drug has a mild symptomatic effect.
Used as adjunct therapy to levodopa
Dose of 5 mg bd
Side effects: confusion, hallucinations, insomnia (second dose at midday)
New melt preparation a given at a lower dose of 1.25–2.5 mg/day
COMT inhibitors
Increase the amount of levodopa reaching CNS
Entacapone (200 mg) prescribed with each dose of levodopa (dose range
400–1200 mg/day).
Side effects: excess dopaminergic effects, dyskinesias managed by
decreased levodopa, diarrhea
Surgical treatment of PD
New surgical procedures have developed as a result of a better
understanding of the pathophysiology of PD (Table 9.4).
Management of other problems in PD
Depression
Affects the quality of life in 40%
Although SSRIs (e.g., citalopram and sertraline) are fi rst choice, there is a
concern this class of drugs may cause a deterioration of parkinsonian
symptoms.
Mirtazapine (presynaptic A 2 antagonist)
Alternative drugs are the tricyclic antidepressants.
In severe cases electroconvulsive therapy may be an option.
Psychosis
Occurs in 10%–15%
Symptoms: mild illusions, visual hallucinations, and paranoid delusions
Underlying pathophysiology is combination of development of cortical
Lewy body dementia and medication effects.
Management
Treat any infection (urinary tract infections, bed sores, etc.).
Correct any metabolic derangement, e.g., dehydration.
Reduce and withdraw anticholinergics, selegiline, amantadine, DAs, and
lastly, levodopa.
If necessary, balance between “mad and mobile” and “stiff but sane.”
Consider addition of a newer generation of antipsychotic drugs, e.g.,
quetiapine 25–50 mg daily to bid. Low-dose clozapine (6.25–50 mg, mean
25 mg) has been shown to be effective. Agranulocytosis occurs in 1.2% of
patients using clozapine.
Dementia
Features of Lewy body dementia include visual hallucinations, a fl
uctuating course with lucid intervals, and an exquisite sensitivity to
neuroleptic drugs.
Benefi t with the use of cholinesterase inhibitor drugs used in the treatment
of AD, such as donepezil and rivastigmine.
Sleep disturbance
Common problem due to combination of factors:
Stiffness and rigidity, making it diffi cult to turn in bed. Consider CR
levodopa preparations or cabergoline.
Bladder disturbance due to detrusor hyperrefl exia resulting in nocturia.
Oxybutynin and tolterodine may help.
Restless legs: CR levodopa or cabergoline at night
Rapid eye movement (REM) sleep behavior disorder (RBD) where
purposeful nocturnal motor activity occurs during REM sleep phase.
Clonazepam 0.5–2 mg is effective.
Excess saliva due to an inability to swallow
Can be treated with anticholinergic drugs but will have signifi cant side
effects
Hyoscine patches behind the ear
Instillation of atropine drops 0.5% on the tongue two or three times a day
Botulinum toxin injection or radiation therapy into the parotid glands if
unresponsive and problematical
“Freezing”:
especially in doorways: visual, patterned cues across doorway help. Use of
“laser cane” to step over beam.
Falls and postural instability
Occur late in the course of the disease and are unresponsive to medication
Multidisciplinary assessment with a physiotherapist and occupational
therapist to acquire walking aids and make appropriate adaptations
Gangguan pergerakan: Pendahuluan
Gangguan pergerakan secara garis besar dikategorikan menjadi hipokinetik dan
hiperkinetik. Terlepas dari pengaruh genetik atau lingkungan yang mendasari
gangguan pergerakan, penyebab umum gangguan pergerakan biasanya
merupakan kelainan dari basal ganglia dan koneksinya. Basal ganglia terdiri dari
inti dalam dengan input dari korteks serebri melalui thalamus dan output ke
thalamus/ korteks serebri dan sistem piramidal. Di dalam basal ganglia terdapat
hubungan yang kompleks antara eksitasi dan inhibisi yang memungkinkan kontrol
yang lebih tepat untuk memulai dan mengontrol gerakan dengan baik. (lihat
gambar 9.1).
Jaras Basal ganglia. Garis utuh dan garis putus-putus masing-masing
menunjukkan jalur eksitasi dan inhibisi. Kerusakan pada jalur tersebut
memungkinkan terjadinya ketidakseimbangan sistem, yang mengarahkan pada
gangguan pergerakan hiperkinetik atau hipokinetik. Striatum terdiri atas caudatus
dan putamen. GPe: bagian luar dari globus pallidus, GPi: bagian dalam dari
globus pallisus. STN; nucleus subtalamikus, SNr: substansia nigra pars reticular,
SNc: Substansia nigra pars compacta.
Gangguan Pergerakan Hipokinetik
Gangguan pergerakan hipokinetik ditandai dengan rigiditas, bradikinesia, atau
akinesia. Gejala lain yang mungkin timbul ialah instabilitas postur, dan gait
tertentu. Penyakit parkinson merupakan gangguan pergerakan hipokinetik yang
paling sering ditemukan; parkinsonisme mungkin juga dapat ditemukan pada
sejumlah kecil kelainan herediter, sporadic, dan kelainan didapat. Penyakit
parkinson merupakan prototipikal gangguan pergerakan hipokinetik.

pengkonsumsian kafein
PP familial
Jarang ditemukan (satu keluarga permutasi); genotip multipel yang
mempengaruhi protein yang berbeda (atau produk yang tidak dikenali)
dengan fungsi yang berbeda pula, dan menghasilkan fenotip yang serupa.
Lebih dari 10 mutasi sudah diidentifikasi (contoh, PARK 1-10) pada 5
atau lebih kromosom

Patofisiologi PP

dan kematian substansia nigra (SN) melebihi 50%.
Fungsi anatomis yang terlibat pada PP meliputi:
o Motor korteks primer
o Area motor suplementer
o Striatum (putamen dan kaudatus)
o Globus pallidus
o Substantia Nigra (SN)
o Nukleus Subtalamus (NST)
SN bertindak sebagai akselerator pada basal ganglia dan kerusakan
berakibat pada bagian ini berakibat perlambatan
NST merupakan rem sehingga kerusakan pada bagian ini berakibat
pergerakan yang berlebihan.
Gambaran klinis pada parkinsonisme dan PP
Diagnose sindrom pankinsonian
Bradikinesia: kelambatan dalam insiasi gerak voulenter dengan
pengurangan kecepatan dan aplitudo dengan repetisi, contoh, tapping ibu
jari dan telunjuk. Ditambah dengan paling tidak salah satu dari tanda
berikut:
Rigiditas: peningkatan tonus, fenomena cogwheel (“cahtching” atau
ratcheting yang dirasakan oleh pemeriksa sewaktu sedi dimanipulasi
secara pasif)
Tremor istirahat:
o Merupakan symptom pertama pada 75% kasus PP
o Dua puluh persen pasien tidak pernah mengalami tremor
o Pada beberapa pasien tremor baru muncul pada posisi
tertentu, hal ini merupakan onset yang tertunda
(“reemergent”), tremor akan muncul beberapa saat setelah
posisi pemicu diadopsi.
Instabilitas postural tidak disebabkan oleh disfungsi visual primer,
vestibular, serebelar, atau proprioseptif.

Konstipasi: biasanya muncul sebagai gejala awal, muncul 1-2 dekade
sebelum gejala yang lain muncul
Anosmia: 80% pasien PP mengalami penurunan fungsi penciuman.
Apabila PSP, DKB, atau MSA
Distonia:
o Jarang pada awal perjalanan penyakit-pertimbangkan MSA
o Lebih sering muncul pada terapi L-dopa
Simptom pada kandung kencing dan saluran pencernaan:
o Mild urinary symptoms. Frekwensi, urgensi, tetapi jarang
inkontinensia yang dikarenakan hiperefleksi detrusor.
o Pada MSA simptom ini terjadi lebih awal dan lebih berat
o Konstipasi sering terjadi
Hipotensi postural: gejalanya sedang, tetapi dapat dieksaserbasi oleh
levodopa dan agonis dopamine.
Gangguan bicara:
o Hypophonia (volume monoton, dan rendah)
o Kecenderungan untuk mengulangi suku kata pertama (pallilalia)
Gangguan tidur:
o Restless legs syndrome
o Gangguan REM sleep behavior dimana pasien merasa seolah-olah
bermimpi, gangguan ini biasanya muncul satu decade lebih awal
dibanding simptom dan diagnosis yang lain
Demensia. Pada tahap akhir, 20% dari pasien dapat mengalami
dementia:
o Dengan memori impairment
o Kekalutan fluktuatif
o Halusinasi visual
o Medikasi dopaminergik mungkin dapat memperberat demensia

Tremor esensial (TE) versus PP
o TE 10 kali lebih sering terjadi dibandingkan PP.
o TE merupakan tremor yang muncul karena postural ± action
tremor. Tremor postural yang berat dapat muncul pada saat
istirahat tetapi bukan “pitt rolling”
o Pasien dengan TE juga mungkin memiliki vocal tremor; head
tremor (“no-no” atau “yes-yes”).
o Pada PP juga mungkin terdapat jaw tremor; leg rest tremor.
Investigation
Tidak ada tes diagnostik untuk PP. diagnosa dibuat berdasarkan bukti
klinis.
123I-FP-CIT SPECT scan (DaTscan). Ikatan ligand terhadap protein
transporter reuptake dopamine. Menurun
Kerusakan neuron striatonigral, berguna dalam membedakan TE dari
PP, tetapi tidak untuk membedakan PD dari MSA dan PSP.
Eksklusikan penyakit Wilson pada pasien dengan umur <50 tahun yang
memiliki:
o Ceruloplasmin
o 24-hour urinary copper
o Pemeriksaan slit lamp untuk cincin Kaiser-Fleischer
Pasien MSA mungkin memiliki degenerasi Onuf nucleus yang dideteksi
sebagai polyphasic potentials dengan peningkatan latensi pada uretra
atau spinkter
EMG. Tidak rutin dilakukan untuk diagnosa.
Positif palsu terjadi pada psien yang pernah menjalani operasi prostat
dan pada pasien dengan PSP.
EMG spinkter memiliki sensitivitas 0.84 dan spesifitas 0.89.
Tes fungsi otonom dan tes kognitif, apabila MSA adalah diagnosa
bandingnya. Tes kognitif dikakukan, karena demensia jarang pada
MSA.
MRI (lihat tabel 9.2).
Parkinsonisme karena induksi obat
Depletasi dari dopamin dari presinaptik akan mengakibatkan
penyimpanan:
o Reserpin
o Tetrabenazin
Bloker dopaminergik:
o Obat-obat neuroleptik: fenotiazin (klorpromazin), butirofenon
(haloperidol), tioxantin (flupentixol), dan benzamid subtitut
(sulpiride)
o Prokloperazine untuk simptom mual dan labyrinthine
o Metoklopramid untuk simptom GI
Ciri klinis
Tremor dan asimetri seperti pada PP
Pasien mungkin juga memiliki gangguan pergerakan yang lain,
disamping parkinsonisme yang tidak diobati, antara lain:
o Diskinesia orofasial
o Stereotypi
o Akathisia
Parkinsonisme mungkin membaik antara beberapa hari sampai
beberapa tahun setelah pemberian obat, terutama apabila sediaan depot
telah digunakan.
Pasien dengan usia lanjut mungkin akan memiliki gejala sisa.

Tatalaksana medis pada PP
Levodopa
Terapi levodopa masih merupakan gold standar pada PP.
Dosis awal adalah Sinemet® (levodopa/carbidopa) 25/100 saat makan
tid
Efek samping: mual, muntah, anoreksia-sering membaik dengan
sendirinya
Pertimbangkan domperidone, 10-30 mg tid, untuk mengatasi simptom
GI
Sediaan modifikasi (Sinemet® CR) tidak memiliki kelebihan dalam
mencegah komplikasi motor; meskipun begitu, sediaan ini berguna
untuk hipokinesia dan rigiditas nokturnal.
Bioavibilitas dari sediaan CR ialah 70% dibanding sediaan lepas cepat.
Sediaan levodopa terdispersi berguna sebagai tambahan awal yang
segera berfungsi untuk membangunkan pada kasus “offs” yang tiba-
tiba, dan episode-episode unresponsive.
Komplikasi motoris berkembang pada 50% pasien PP setelah 6 tahun
terapi levodopa.
Monoterapi dengan obat-obat DA tidak mengakibatkan komplikasi-
komplikasi tersebut; karena itu merupakan alasan untuk menunda
penggunaan levodopa.
Gunakan pada pasien dengan usia muda hanya apabila memungkinkan
Komplikasi jangka panjang terapi levodopa
Pergerakan involunter atau diskinesia:
o Peak dose choreathetoid dyskinesia
o Diskinesia difasik
o Distonia (painful cramp)
Fluktuatif respon:
o Deteriorasi akhir dosis (wearing off)
o Perubahan on/off tak terprediksi
Psikiatrik:
o Confusion
o Halusinasi visual
o Delusi
o Ilusi
Dopamin agonis (DA)
Obat DA (Tabel 9.3) bereaksi langsung pada reseptor dopamin
postsinaptik tanpa perlu dikonversi terlebih dahulu menjadi dopamin.
DA mempunyai peran sebagai alternatif levodopa sebagai monoterapi,
terutama pada pasien usia muda, untuk menunda penggunaan levodopa,
dan efek komplikasi jangka panjangnya.
Pada pasien yang sudah menggunakan levodopa dan sudah terkena
komplikasi motoris, DA dapat digunakan dengan cara menurunkan
levodopa secara bertahap.
Efek samping
Secara Umum: mual, muntah, hipotensi postural, confusion, halusinasi,
smonolen
Domperidon, 10-20mg tid berguna untuk meringankan efek samping
pada GI dan hipotensi postural (efek periferal).
DA derivat ergot: edema ankle, eritromelalgia, Raynaud, fibrosis
retroperitoneal, efusi pleura, penyakit cardiac valvular
Apomorphine
Apomorphine, D1 dan D2 agonis poten, memiliki bioavibilitas oral
yang buruk
Diberikan secara injeksi subkutan (SC) atau continous infusion.
Indikasi
Injeksi SC apomorphine dapat digunakan untuk menilai respon
dopaminergik, pola, dan distribusi dyskinesias pada pasien dengan
terapi levodopa.
Injeksi intermiten dgunakan untuk penyelamatan periode “off” berat
pada pasien dengan terapi levodopa dan DA maksimal. Berguna pada
periode “off” yang menyakitkan dan distonia, serta periode “off”
spinkter dan kesulitan menelan.
Continous infusion: pertimbangkan pemberian pada seluruh pasien
dengan fluktuasi refraktori motoris yang tidak bisa diberikan
penanganan melalui terapi oral dan membutuhkan >6 apomorphine
injeksi SC. Continous infusion ini harus dipertimbangkan untuk
diberikan sebelum operasi.
Terapi apomorphine sementara dapat dipertimbangkan pada pasien PP
yang menjalani operasi abdomen.
Tes coba apomorphine untuk memperkirakan efek
Mulai pemberian domperidon 30mg tid 36 jam sebelum tes.
Tidak ada pemberian oral obat anti-parkinson selama 4-6 jam sebelum
tes coba.
Sarapan sesuai jadwal dan porsi biasa.
Ukur baseline fungsi motoris dengan motor examination subscale
(Section III) dari Unified Parkinson’s Disease Rating Scale (UPDRS).
Lihat apendiks.
Ukur waktu yang dibutuhkan untuk berdiri dari kursi dan berjalan 12
meter.
Berikan apomorphine 1.5mg SC dan observasi respon motoris selama
30 menit.
Menguap dapat diukur sebagai respon motoris.
Apabila tidak ada respons yang signifikan, berikan 3mg.
Tingkatkan dosis setiap 30 menit antara 7-10mg.
Respons positif apabila terdapat peningkatan skor UPDRS 15%-20%
atau 25% pada walking time.
Terapi lain
Agen antikolinergik
Penggunaan secara terbatas hanya pada pasien usia muda dengan
tremor berat dan distonia.
Trihexyphenidil (benzhexol) (2-5 tid) sering digunakan.
Efek samping terutama pada pasien usia lanjut semisal confusion, dan
retensi urin.
Amantadine
Sebelumnya digunakan pada PP dini untuk mununda penggunaan
levodopa; pada saat ini dengan penggunaan DA, penggunaan
amantadine semakin sedikit.
Peran baru pada penatalaksanaan diskinesis karena obat, dikarenakan
sifat antagonis glutamate.
Dosis 100-300mg/hari.
Efek samping: confusion, halusinasi, edema ankle, livedo reticularis,
insomnia (dosis kedua pada saat tengah hari)
Selegiline
Obat MAOI memiliki efek simptomatik sedang.
Digunakan sebagai terapi tambahan levodopa
Dosis 5mg bd
Efek samping: confusion, halusinasi, insomnia (dosis kedua pada saat
tengah hari)
Sediaan cair diberikan pada dosis rendah 1.25-2.5mg/hari
COMT inhibitor
Meningkatkan jumlah levodopa yang sampai pada CNS.
Entacapone (200mg) diberikan dengan tiap dosis levodopa (dosis range
400-1200mg/hari).
Efek samping: efek dopaminergik yang berlebihan, diskinesis karena
penurunan levodopa, diare.
Treatmen Operasi pada PP
Prosedur operasi baru telah dikembangkan karena bertambahnya pengetahuan
terhadap patofisiologi PP (Tabel 9.4).
Penatalaksanaan masalah-masalah lain pada PP
Depresi
Mempengaruhi kualitas hidup sekitar 40%
Walaupun SSRI (misal: citalopram dan sertralin) merupakan lini
pertama, ada suatu perhatian terhadap kelas ini karena dapat
menyebabkan perburukan dari gejala parkinsonian
Mirtazapin (presinaptik A2 antagonis)
Obat alternatif yang lain adalah tricyclic antidepressants.
Pada kasus yang berat terapi electroconvulsive mungkin dapat
digunakan sebagai pilihan.
Psikosis
Tingkat kejadian 10%-15%
Simptom: ilusi sedang, halusinasi visual, dan delusi paranoid.
Patofisiologi yang mendasari ialah kombinasi perkembangan kortikal
Lewy body dementia dan efek medikasi.
Penatalaksanaan
Obati infeksi yang terjadi (infeksi saluran kencing, dll).
Tangani semua kelainan metabolik, contoh, dehidrasi.
Mengurangi dan withdraw antikolinergik, selegiline, amantadine, DA,
dan terakhir levodopa.
Apabila diperlukan, keseimbangan antara “mad and mobile” dan “stiff
but sane”
Pertimbangkan pemberian obat antidepresan generasi baru, contoh,
quetiapine 25-50mg/hari sampai bid. Clozapine dosis rendah (6.25-
50mg, rerata 25mg) terbukti efektif. Agranulositosis terjadi pada 1.2%
pasien yang menggunakan clozapine.
Dementia
Ciri pada Lewy body dementia termasuk halusinasi visual, rangkaian
fluktuatif dengan lucid interval, dan sensifitas bagus terhadap obat
neuroleptik.
Penggunaan obat cholinesterase inhibitor pada penatalaksanaan AD,
seperti donepezil, dan rivastigmin, akan memberikan banyak
keuntungan.
Gangguan tidur
Problem yang sering terjadi biasanya dikarenakan kombinasi banyak
faktor
Stiffness dan rigiditas, mempersulit pasien untuk membalik badan pada
saat tidur. Pertimbangkan sediaan lovodopa CR atau cabergoline.
Gangguan kandung kemih karena hiperrefleksi detrusor mengakibatkan
nokturia. Oxybutynin dan tolterodine mungkin dapat membantu.
Restless legs: levodopa CR atau cabergoline pada malam hari.
Rapid Eye Movement (REM) sleep behavior disorder (RBD) sewaktu
aktifitas motoris nocuturnal terjadi pada saat fase tidu REM.
Clonazepam 0.5-2mg efektif dalam menangani gangguan tidur.
Saliva berlebihan karena kesulitan menelan
Dapat ditangani dengan obat antikolinergik tetapi akan terdapat efek
samping yang cukup sgnifikan
Hyoscine patch dapat ditempelkan dibelakan telinga
Pemberian tetes atropine 0.5% pada lidah dua sampai tiga kali sehari.
Injeksi racun botulinum atau terapi radiasi pada kelenjar parotis
dilakuan pada kasus unresponsif dan problematis.
“Freezing”: terutama pada doorway: visual, isyarat berpola melalui
doorway. Gunakan “laser cane” to step over beam.
Jatuh dan instabilitas postural
Terjadi pada fase akhir dari penyakit dan unresposif terhadap
pengobatan
Penatalaksanaan multidisipin dengan fisioterapis dan terapis
occupational untuk rehabilitasi gerak jalan, dan adaptasi yang sesuai.
