Here at the Njernda Medical Centre we are always looking at ways to improve how we deliver programs and information to our community. 715 HEALTH CHECK Have you booked in for your 715 Health Check?

A 715 Health Check includes an assessment of the patient’s physical, psychological and social wellbeing. Key elements will include: Information collection Overall assessment of the patient

Recommending appropriate follow up care

Advice and information to the patient

A record of the assessment Providing the patient, or their carer, with a copy of the assessment and

follow up plan. A Practice Nurse, Aboriginal Health Worker or Aboriginal Health Practitioner may assist with initial tests and medical history. A General Practitioner delivers the final assessment and recommends appropriate follow up care

A $20.00 Gift card is given to each patient who undertakes a Health Check.

CHSP OR AGED CARE PACKAGES SUPPORT Are you over 50 years , of Aboriginal or Torres Strait Islander descent and need some assistance to be able to stay living at home? There is a new way to register and request an assessment to see what supports you may be eligible for. A new online form is available on the My Aged Care website that allows you or someone acting on your behalf to register with My Aged Care The form can be completed in your own time as an alternative to calling the My Aged Care contact centre on 1800 200 422. Visit the My Aged Care website go to https://www.myagedcare.gov.au/assessmentfor more information and the form to apply for an assessment. on April 11th, 2020

The purpose of this market is to:

To celebrate and promote local indigenous arts and culture

To provide an opportunity for local aboriginal artists to showcase their skills in traditional arts, craft, music, dance

To provide an opportunity for indigenous and non-indigenous people to come together to bridge the gap and promote reconciliation

To create an opportunity for the local aboriginal people to sell their work and create income

THE ART YARN PROGRAM Will be held every Thursday from

10am - 1pm Njernda Medical Meeting Room

The Program will run over 10 weeks and at the end of the program we will exhibit the artwork created and one will be chosen as the new logo for Njernda’s Wellbeing Unit. The Program will be lead by Neva Takele, so come along and learn different styles of art while having a yarn. All materials are supplied along with a feed. If you are interested in participating, please contact Neva Takele on 0428 594 394 or you can put your name down at Medical Reception.

Beat the Boondah is a 1 hour quit/reduce smoking support group that is delivered across the whole Riverina area.

The program is designed to assist Indigenous people who are wishing to reduce/

quit smoking or would just like the tools and information for future attempts.

Njernda’s programs operate using a Charter of Rights and Responsibilities.

Compliments & Complaints If you wish to lodge a compliment or complaint regarding your service delivery, please discuss this with your case manager, receptionist, or program manager who will direct you on how this is done. Alternatively please feel free to leave suggestions in the ‘suggestion box’ located in each facility.

Client Survey We value your feedback as this helps us identify any issues and make quality improvements to our programs. Your program staff may ask you to complete a survey and are able to assist you to complete this. Thank you for taking the time to complete surveys and helping us improve.

OUR VISION Empowering our Aboriginal Community

We believe that:

• As the first peoples of this country we have a right to recognition, respect and positive support for our culture, heritage and traditions.

• To keep our cultural traditions strong we need to have a powerful Aboriginal community. Njernda is a focus for Aboriginal identity and it must be strong enough to stand up for our rights, present our viewpoints and signal to Echuca that we exist and that we have a unique culture.

• Njernda has a responsibility to satisfy the needs of employment and culturally appropriate housing, education and health services for our Community within Echuca so far as funding and policy directions allow.

• Our children need to learn the power of their cultural heritage within our mob and from their Elders and families. We believe that children who develop their identity through their culture will be able to move in and out of non-Aboriginal society and achieve their aspirations without losing themselves in the process.
