The Graduating Class of 1963 - Uni"I SOLEMNLY PLEDGE MYSELF TO CONSECRATE
MY LIFE TO THE SERVICE OF HUMANITY . .."Those opening words from the Declaration of Geneva sig
nalled the farewell of 128 seniors at the University of Minnesota Medical School in colorful graduation ceremonies heldJune 14th and 15th. Four years of intensive training in thescience and art of medicine ended for the Class of 1963 with thegranting of M.D. degrees and the conferring of special honors.
Senior Recognition Day exercises were held at the MedicalSchool before a rapt audience of relatives, friends, and facultymembers. Dr. Robert B. Howard, Dean of the College of Medical Sciences, presided. The Class of 1963, in traditional capand gown attire, marched in academic procession to the MayoAuditorium to receive their valedictory and hear a thoughtful, provocative commentary on medical practice by Dr. PercivalBailey, Chicago, Ill. His topic was "Medical Education - AFifty Year View."
As it graduated, the Class of 1963 swelled to more than6,200 the ranks of those physicians who received their M.D.

versity of Minnesota Medical Schooldegrees from the University of Minnesota since the first classgraduated in June, 1889. About 5,000 of these alumni arestill living.
The Class of 1963 included six women. Seventy percent ofthe class was married, including the first Negro couple tograduate in 74 years of Medical School history. The audienceon Recognition Day was filled with ample evidence that therewere many parents in the graduating class itself.
Twenty-seven members of Alpha Omega Alpha, honor medical fraternity, were cited for outstanding scholarship. H. DavidKnudsen received the Southern Minnesota Medical Association medal for "outstanding proficiency in medicine and sur·gery" from Dr. Edward Richardson, Jr., St. Paul, its president.
Eric Eisenklam was recipient of the Borden Award for "sig.nificant contribution to research during the student years,"and Mediclinics Prizes of $1,000.00 each were presented by Dr.Arthur C. Kerkhof (Med. '27) to Donald S. Masler and Don·aId W. Oines, Medical School juniors, for "exemplary scholastic and professional achievement."

Silicone Rubber in the Urinary Tract"JOHN A. BLUM, M.D.tMILTON P. REISER, M.D., M.S.!
Ureteral prostheses of silicone rubber and Ivalon® spongehave been used successfully to replace the lower ureter ofthe dog for two years. When a neoplasm surrounds or invades the distal ureter, a normal kidney may be lost. Thisneed not occur if prosthetic ureteral substitutes could beavailable. Complications associated with ureteral replacement with an isolated ileac segment include those associated with intestinal operation as well as stasis in the dilatedsegment, and even electrolyte imbalance if both uretersare replaced and the kidneys are diseased.
Vein, artery, fallopian tube, fascia, peritoneum, skin, vitalium, tantalum, glass, polyethylene, polyvinyl, dacronand nylon have been tried unsuccessfully as ureteral prostheses. There is a recent report from Czechoslovakia of adog alive and well seven months postoperatively with anureteral prosthesis of crimped teflon. Other investigatorshave not been able to duplicate this result.
There are several problems in the use of ureteral prostheses: surgical, chemical, and physiological. The surgicaltechnique requires an anastomosis of the prosthesis to therest of the urinary tract, and fixation to the surroundingtissue so as to prevent stricture formation and migration ofthe prosthesis. The chemical problem is concerned withthe tendency of urinary salts to precipitate on a foreignbody with the resultant obstruction to drainage and eventual destruction of the kidney. The physiological problemis concerned with the absence of ureteral peristalsis necessary for the propulsion of urine and for the prevention of

vesicoureteral reflux. Absence of peristalsis in the prosthesis may produce a relative obstruction and so destroythe kidney.
Silicone rubber is a polymer which is tolerated by thetissues; neither autoclaving or aging changes its characteristics. There is less tendency for encrustation with urinarysalts than with other commonly used inert polymers. Silicone rubber was used as an ureteral prosthesis, and porousIvalon sponge was placed around it for anastomosis andlater fixation by fibrous tissue invasion.
In the first group of 12 dogs, ureteral prostheses six centimeters in length were used to replace the lower ureters.Three are alive and well- two with bilateral prostheses for18, and one with an unilateral prosthesis and contralateralnephrectomy for 26 months. All dogs have been followedwith urograms, cystoscopic examination, cystograms forreflux, and urine cultures; urinary infections and ureterovesical reflux are present. The vesical mucosa has healed without reaction around the protruding silicone rubber tip, andthe kidneys function well as evidenced by indigo carmineflowing from the neoureter at time of cystoscopic observation. Urograms of the surviving dogs demonstrate stasisin the upper collecting system but no dilatation. Five animals were sacrificed when the prosthesis migrated into thebladder. The design was then changed and no migrationhas occurred in the next seven dogs since that time. Twodeveloped pyohydronephrosis from anastomotic strictures, and two with normal upper tracts, died of unrelated causes.
A second group of nine dogs have had total ureteral replacements - two are alive and well over one year withunilateral prostheses and contralateral nephrectomies. Fivehave developed hydronephrosis due either to anastomoticstricture or to relative obstruction of an aperistaltic tubeand have died. In one, a calculus on an exposed surfaceof the Ivalon propagated down the Silastic® tube. Twodogs died of unrelated causes. When pyohydronephrosisoccurred the Ivalon sponge was always involved in the inflamation and appeared crumbly and disintegrating; formerly this was interpreted as a result of infection. Recentlyone animal died 13 months postoperatively from a reactionto intravenous contrast agent. Its urinary tract was normal,

but the Ivalon was disintegrating. With this in mind weintend to employ more durable substances for the outersleeve.

What Is Behavioral Science? '"DONALD W. HASTINGS, M.D.t
A new term, heard with increasing frequency in medicalschool circles, is "behavioral science." It seems to representa new scientific face rising in the medical school setting, but actually it does not. If the term is turned about andregarded as "any scientific endeavor which aims at understanding behavior," it is clear that few areas in sciencewould be excluded. Most of the biological sciences wouldqualify as well as all of the basic sciences traditional tomedicine - anatomy and neuroanatomy, physiology andneurophysiology, psychophysiology, pharmacology andneuropharmacology, psychopharmacology, b i 0 che m i stryand neurochemistry, genetics, statistics, and microbiology.
While the sciences related broadly to behavior seem mostimportant clinically to the field of psychiatry, they are ofconcern to all physicians with respect to psychosomaticinterrelationships. They also are important to research intothe diseases which so far have defied understanding butare thought, in part, to have an emotionally determinedetiology: asthma, peptic ulcer, many skin lesions, and perhaps even degenerative conditions of blood vessels.
Any science which investigates behavior utilizes hypotheses which can be tested by measurement. This distinguishes that science from philosophy about determinants ofbehavior.

THE MEDICAL BULLETIN
The years since World \Var II have seen a marked surgeof interest in the sciences of behavior. This has been stimulated mainly by two factors: (1) the national concern aboutthe size of the mental health problem, its staggering costsand wastage of human resources, plus the recognition ofhow little basic knowledge exists in medicine around whichto organize prophylactic and active treatment programs;(2) the urgent need for specialized data on the part of governmental agencies: psychological warfare, chemical warfare with respect to individual and mass effects on behavior, the space program, and so on.
The problem faced by medical education today is notwhether sciences exist which are interested in behaviorand which deserve to be called scientific. Such scienceswere established decades ago. Rather, the question is: inview of the need for much more basic data about mentalillness and health, about psychosomatic medicine, andabout the theorized emotional component of puzzlingdiseases such as atherosclerosis, how much expansion andconsolidation of effort should be made with the primaryresearch goal being the understanding of human behavior? What scientific areas, not ordinarily found in medicalschools, such as sociology, anthropology, certain subspecialties of psychology, and so on, should be incorporated?
No medical school as yet has answered these questions toits own satisfaction. Several have large pilot projects underway; others have made no move. Since no medical schoolcan possibly incorporate all of the sciences dealing in oneway or another with behavior, the ultimate pattern willhave to appear as a consensus on the part of medicaleducators as to how much behavioral sciences should betaught to medical students as part of their basic knowledge, and secondly, what is the responsibility of medical schoolsfor stepping up and accenting research in behavior at amuch faster rate than is now occurring?
While no uniform pattern will or should occur acrossthe country, it appears inevitable that expanded researchand teaching programs in behavioral science will occur inMedicine. This is the current development which has giventhe impression of a new science arising. More accurately, itis a matter of accent, expansion, and consolidation.
