IntroductionThe treatment of major osseous defects in orthopedic oncology is dominated by surgical techniques which focus on preserving the affected limb. Ablative measures (amputations and rotation plasty) as well as arthrodeses of major joints are avoided as far as pos-sible, if reasonable from an oncologic point of view.Surgeons may choose between different standard procedures: Besides the option of using allogene and autologous graft material, a tumor prosthesis represents a proven solution for the reconstruction of osseous defects. A com-bined use of allograft and prosthesis is often chosen. During the past few years, modular replacement systems have become more and more popular, since they offer the opportunity to fi ll defects of varying extents with respect to each individual case. Since 1992, MUTARS® (Modular Universal Tumor And Revision System), has been successfully used for the treat-ment of major osseous defects of the lower and upper extremities.Its modular design was developed in co-operation with Prof. Dr. W. Winkelmann (former director) and Prof. Dr.G. Gosheger (director), Department of General Orthopedics and Tumororthopedics at the University Hospital of Mün-ster, Germany. The system shows good functional results and allows an individual solution for major osseous defects, including arthroplasties of the adjacent joints.Until 2009 more than 10,000 MUTARS® prostheses have been implanted successfully. The co-operation with lead-ing European clinics has enhanced continuous further development of the implants and instruments, resulting in the proven instruments available today.This close collaboration enables us to always remain up-to-date and continue further development by using the latest experience in the tumor prosthesis sector by modifying our products according to this latest standard of knowledge. Since March 2003 the new PEEK-OPTIMA® lock is in clinical use and has shown excellent outcome and it is replacing the former PE lock now. Alternatively the system offers M-O-M components which are coupled by the metal-on-metal connecting mechanism which is successfully used in the Proximal Tibial Replacement for more than 15 years. It is recommended to use the M-O-M components in case of extra-articular resection.The non-invasive MUTARS® Xpand Growing Prosthesis could be developed together with a partner company. The fi rst implantations of the biological Growing Prosthesis MUTARS® BioXpand have shown promising results. More detailed information about it can be found in chapter F.1 of this brochure.

Since 2004 the range of humeral components includes inverse humeral implants. Now it is enhanced by a new Glenoid and eccentric Glenosphere and self taping stable locked cancellous screws which can be used to enhance the primary fi xation of the Glenoid. With the new proximal Ulna Replacement and the shorter Distal Humerus 30 mm the variety of treatments in the area of the upper extremity is extended. The Attachment tube is now available in two different diameters. Further progress could be observed with the clinical use of the silver coated implants (MUTARS®Silver). The anti-infective coating for salvage and prophylaxis against infections is established. The CE cer-tifi ed MUTARS®Silver components are considered as standard implants for the treatment of tumor patients in several major tumor centers throughout Europe. So far more than 2,000 MUTARS®Silver implantations have been performed successfully.Based on the femoral and tibial MUTARS® joint components the modular MUTARS® GenuX® Knee Revision System is implanted. With the GenuX® connecting modules an intramedullary connection between the hip and the knee joint can be assembled and the so called Intramedullary Total Femoral Replacements can be build more precisely than before. The cementless stems of the MUTARS® RS Hip Revision System offers now a HA-coating. A special targeting device is available to improve the distal locking of the RS stems. Currently the new MUTARS® RS Arthrodesis is launched, which allows the cementless and cemented fusion of the knee joint. Actually the team of developers is working on modifi cation of the Proximal Tibia and the development of a new revision acetabular cup and a modular partial pelvic replacement called LUMiC®.

The modular MUTARS® system offers the opportunity of a functional replacement in cases of major osseous defects, independent of the reason of the defect, a tumor excision, fractures or an infection-related explantation of a prosthe-sis.

Joint replacements may be carried out using the constrained rotational knee components, the constrained hinge joint for the elbow joint and the inverse components for the shoulder joint. In combination with regular joint components the system allows the hemi arthroplasty and the total replacement of the hip joint. Depending on the type of prosthesis, fi xation is achieved with or without cement. There are two types of intramedullary stems available: one designed for a cementless fi xation (TiAl6V4) and one designed for cementation (CoCrMo). The cementless stem has a microporous surface and is also available with a hydroxylapatite coating (HA).

The stems have a hexagonal cross section (Fig. 1) to provide for an optimal intramedullary fi xation. In the femoral area, the stem was designed to correspond with the physiological antecurvation of the femur. Hexagonal rasps are used to achieve a primary stability of 70 Nm. For an alternative way of cementless fi xation there are special tapered stems and the RS stems and the RS coupling device available to meet special bone geometries and conditions.

The toothed connecting faces of all components allow a precise intraoperative adjustment of the rotational angle ac-cording to the implanted fi xation stem. Thus, rotational malalignments can be avoided or corrected subsequently. The toothing also allows an exact intraoperative adjustment in 5° increments of the femoral antetorsion angle (Fig. 2).

In order to provide the correct implant length in each individual case, variable extension pieces are available for the reconstruction of the bone defect. The MUTARS® system allows an intra-operative adjustment of the length in 20 mm increments (10 mm increments for the total femoral replacement). On special request a 30 mm extension piece is available to adjust the implant length for the lower extremity in increments of 10mm.The article number contains the necessary information for the correct MUTARS® article. The implant description is chosen according to the localization and dimension of the tumor.Soft tissue refi xation or capsular reconstruction is performed using the MUTARS® Attachment tube (made of PET polyethylene terephthalate), which is fi xed at the prosthesis by means of a non-absorbable suture material, providing an effective protection against luxation of the shoulder and hip joint. As an additional prevention against allergic reactions, all components, which are made of CoCrMo alloy, may option-ally be fi nished with a titanium nitrite coating (TiN).If individual cases require special component features, the modular MUTARS® system can be supplementedby implants which are manufactured to specifi cation with a short delivery period, thus providing an optimal solution for each individual case. Please also refer to chapter F.3, which contains more information about special components.

MUTARS®Silver coating

The Silver coatingEarly and late infections represent the most severe complications of tumor arthroplastic treatments. Although local and systemic antibiotic treatments are considered, the scientifi c literature reports of infection rates of 5 to 35 percent. Reasons for these high rates are i.e. the long surgery time, the large incisions and the immunosupression due to chemo therapy and radio therapy as well as the increasing resistance of the bacteria against antibiotic drugs.The anti-infective effect of silver ions is known for centuries i.e. the disinfection of potable water is based on this princi-ple. This special property of the silver is used for the silver coated components of MUTARS® to build an intelligent protection against bacteria. Created by Brownian motion of the electrons of the silver coating, silver ions emerge on the prosthetic surface binding to electronegative groups of proteins, enzymes and the DNA of the bacteria.Their scientifi c expertise at the University of Münster let the investigators state that the energy transport of the cell, the translation and the transcription within the cell are inhibited, the metabolism is shut down and an adequate contact of bacteria to silver will consequentially lead to cell death. Another special chemical property of silver ions is the fact that they dissolve in an oligodynamic way and can release by dropped pH value like in case of infl ammation. There are always suffi cient silver ions on the surfaces for even late infections. Hence the silver coating is representing an intelligent shield against infections. By pre-clinical tests and during successful clinical trials the effi cancy of the clinical function of silver is ensured. The members of the research team at the University of Münster believe that: “ The silver coating represents an intelligent, bactericidal protection shield”.

Ongoing tests are performed to prove the impact of silver ions to the osteointegration and investigate to what extend intramedullary cementless stems of primary and revisions arthroplasty can be. Until now only non-articulating sur-faces and surfaces without direct bony contact are coated with silver. Silver coated plates in the PE-plugs of the Distal Femur M-O-M (see Fig. 5) assure that suffi cient silver is provided also in the area of the distal femur. In the catalogue information of this brochure you can fi nd the supplement *S indicating which MUTARS® components are available in silver coated version. The eight digit REF number receives an addition after the last digit (i.e. 5220-0020S).

The TiN coating for allergy prophylaxisAs the metallic components of total knee replacements, the articulating metallic parts of the MUTARS® system are made of casted CoCrMo alloy. In the late 70’s and 80’s of the last century, some of the Cobalt Chromium implants had a small Nickel content to add strength to the implant. Nickel is the primary cause for metal sensitivity, although some patients have shown to be hypersensitive to other metals such as Cobalt and Chromium. The use of titanium components can’t solve this problem, because the wear of the articulating polyethylene inlays will increase and so the survival time of the prosthesis is reduced. Since the end of the 1990’s TiN (Titanium Nitride coating) has been suc-cessfully applied to protect the body against metal ions that could cause allergic reactions. The metal ion release of TiN coated or TiNbN coated implants is reduced down to 10%. With the TiN coating the im-plants gain ceramic properties and show even better wear characteristics than CoCrMo components.Ceramic coatings have following properties: - increased hardness of the articulating surface - increased scratch resistance - lower friction coeffi cient - improved wettability - reduced wear of the counter surface - higher corrosion resistance - decreased metal ion release - enhanced bone ingrowth capacity - increased biocompatibility In order to prevent allergic reactions, certain parts of the prosthesis may be supplied with a ceramic coating (TiN). Since almost all components of the tumor system consist of titanium alloy, this only concerns those components, which are made of a cast CoCr alloy (CoCrMo). The REF-numbers of the TiN coated implants have the suffi x N after the last digit (i.e. 5720-0005N).

C.3 Attachment tubeIndication: The MUTARS® Attachment tube allows reconstruction of the capsule and refi xation ofmuscles and prevents dislocation thus it is used in proximal treatments of the humerus (rotator cuff) and femur. The MUTARS® Humeral ComponentsThe special system for the upper extremity offersthe opportunity of a proximal, distal or total humerus reconstruction. The modular humerus caps are available in three different sizes and allow an optimal glenoid contact. The small head piece and the slightly medialized geometry provide improved soft tissue coverage. As an intraoperative option there are inverse humeral components available including an inverse Hu-merus cap and glenoid replacement. The design of the extension pieces allows for an intraopera-tive adjustment in length of 20 mm increments. The modular construction allows the surgeon to treat the whole upper extremity with one universal system. If necessary, the implant may be extended intraoperatively to a total humerus prosthesis, thus providing an optimal solution to the individual requirements of each single case.

F.1 MUTARS® Xpand Growing ProsthesesThe endoprosthetic treatment of tumor patients of treatment of non adolescent tumor patients is a chal-lenging procedure for the surgeon as well as for the implant. The intramedullary fi xation of the prosthetic components leads to reduced growth of the treated extremity and i.e. leg length discrepancies. Most of the available systems include lengthening modules which are elongated mechanically during surgical proce-dures. In order to reduce the exposure of the patients to several surgeries and rising the risk of infection, theMUTARS® Growing Prostheses follow a new invention. Based on an idea of Prof. Dr. Baumgart (München) and in close collaboration with the company WITTENSTEIN intens GmbH (Igersheim) two new principles (mechanical and biological) of Growing Prostheses have been developed.The elongation of the Prosthesis, using the MUTARS® Xpand ProsthesisThe mechanical, non-invasive Growing Modules of the MUTARS® Xpand using a miniaturized, mechatronic actuator inside the prosthesis which is activated by a high frequency transmission (from Control Unit via transmitter head and receiver) from outside the skin. An additional surgery with opening of the skin is not necessary, the risk of infection is eliminated. After the lengthening procedure the Xpand components are replaced by regular MUTARS® components.

Remark: All components of the MUTARS® Xpand Growing Prostheses are customized implants which are planned, designed and manufactured for each patient based on a scaled x-ray.

F.1 MUTARS® BioXpand Growing ProsthesesIn the “biological” MUTARS® BioXpand Growing Prosthesis the initially implanted stem is used as a dummy stem replaced by a lengthening nail (FITBONE®) at the time when lengthening should be initiated. In this type of Growing Prosthesis the electro magnetical technology is used to lengthen the bone of the patient and gain bone stock.

The lengthening of the remaining bone, using the MUTARS® BioXpand ProsthesisThe “biological”, non-invasive BioXpand Growing Prosthesis for the proximal and distal Femur as well as for the proximal Tibia are suitable to stimulate the growth of remaining bone after tumor resection. Using the principles of callus distraction (Fig. 1 and 2) the interim stem is replaced by a motorized lengthening nail (FITBONE® ), an osteotomy is performed and the remaining bone can be lengthened. After the lengthening procedure also the MUTARS® BioXpand components are replaced by regular MUTARS® components.

In those delicate cases the use of standard stems may not be possible to avoid involving the joint.
