MOGSC 14th Annual General Meeting
Wednesday, 26th April 2017 – The MOGSC 14th Annual General Meeting (AGM) was
held at Menara MATRADE, Kuala Lumpur and was well attended by MOGSC members.
At the AGM, the members:
1. Confirmed the minutes of the previous Annual General Meeting.
2. Received and confirmed the Executive Committee Report for the Session 2016/2017.
3. Received and confirmed the Auditor’s Report and Financial Statement for the year
ended 31st December 2016.
4. Reappointed Auditors for the ensuing year.
5. Approved the proposed amendments to the Constitution.

The MOGSC-MGA Industry Golf Tournament 2017
was held at Templer Park Country Club, Rawang on
Saturday, 20th May 2017.
All 120 golfers were on hand for the 7:45a.m.
shotgun start. We started the day with a nice and
cool weather that was ideal for a great day of golf.
Congratulations to Mr. Azrin Din for winning the
overall champion of the tournament.
The tournament would not have been a success without the generous contributions from

MOGSC Meet-the-Members – Presentation by SHELL“Delivering Malikai, Malaysia’s First Tension Leg Platform”
On Tuesday, 18th April 2017, Shell Malaysia presented “Delivering Malikai, Malaysia’s First
Tension Leg Platform” to MOGSC members. YBhg. Datuk Iain Lo, Chairman of Shell
Malaysia was on hand to give the opening remarks. The list of speakers were – Mr. Momas
Modon (Malikai Project Manager), Mr. Shankar Baht (Tendon/Riser Manager), Mr. Edmund
Adrian (Tension Leg Platform Manager) and Mr. Jilan Jenggi (Commissioning & Start-up
Manager).
The insightful presentation included on Health, Safety and Environment (HSE), Competitive
Scoping and Cost Competitiveness, Supply Chain Transformation and also Efficient Execution.
The presentation received the most compliments we have ever received. We thank Shell for a
fantastic display of support, openness and sharing.
MOGSC Meet-the-Members in Johor“Opportunities for O&M in Pengerang Integrated Complex (PIC)”
Johor Sector was fortunate to get PETRONAS to present on the Opportunities for Operations &
Maintenance in Pengerang Integrated Complex (PIC) on Thursday, 11th May 2017 at UTM-MTDC
Technology Centre II Technovation Park, Universiti Teknologi Malaysia, Skudai, Johor.
The event began with the latest Pengerang Integrated Complex project progress video, followed by
the presentation on Opportunities for Operations & Maintenance in Pengerang Integrated Complex
by En. Marzuki Daud, Head, Procurement Pengerang, Group Procurement, Project Delivery &
Technology (PD&T), PETRONAS.
PETRONAS also had representatives from Licensing & Local Capability Development (LLCD) to
present on PETRONAS Licensing & Registration. The speaker was Pn. Khalida Asri, Head,
Vendor Relationship Management (VRM), LLCD, Group Procurement, PD&T.

MOGSC-INSTEP Collaboration in Competency Development
On Tuesday, 20 May 2017, the MOGSC Competency &
Training Working Group, travelled to INSTEP to present
the MOGSC-INSTEP Collaboration in Competency
Development Proposal to Mr. Chandramohan, INSTEP
CEO and his management team.
The objective of the proposal is for MOGSC to become
INSTEP’ industry partner, develop opportunities for
members and enhance industry competency levels. In
return, MOGSC would promote INSTEP, increase
facility utilization and develop new courses.

Unit (DCU)
Summary: In February 2017, loss of primary containment (LOPC) of flammable fluid at
above auto ignition temperature resulted in a fire in Delayed Coker Unit (DCU). The unit was
immediately shut down.
The fire was extinguished by the in-house Fire Prevention Team and First Line Fire Crew.
There was no injury to personnel, no impact to the environment and no public complaint
recorded due to the incident. However, substantial asset damage occurred.

Summary: In March 2017, Deceased Person (DP) attended first day of BOSIET refresher
course and was certified “Fit for Offshore Work”.
During underwater Emergency Breathing System (EBS) familiarization, DP requested for a
break as he felt unwell and tired. DP’s condition suddenly deteriorated and was administered
first aid with oxygen and AED immediately by the medics.
CPR was performed continuously on the DP at the Training Centre and while being
transported to the nearest hospital. At the hospital, the DP was attended by emergency
department but they were unable to resuscitate the DP.

Summary: In March 2017, a fire incident occurred at a gas processing plant's wastewater
holding pond. An operator (Injured Person "IP") was driving a pick-up truck to attend to a gas
detector alarm when vehicle caught fire due to a gas cloud at the area.
The IP suffered burn injuries and the fire also caused damage to nearby facilities. Tier 1
emergency response was activated and the IP was sent to a medical facility for stabilization
and treatment. The fire was put under control and all clear alarm was activated.
Based on preliminary investigation, the source of hydrocarbon came from an open valve
which is supposed to be normally closed.

Summary: In April 2017, a major LOPC incident was registered when 37,000 liters of Diesel
was inadvertently drained from the Diesel Day Tank for Diesel Fire Water Pump (DWFP) to
the closed drain header. Low level alarm of the Diesel Day Tank alerted the Operation
Technician (OT), who then went to the site and found the Level Control Valve (LCV) at the
inlet of Diesel Day Tank and drain valve at the bottom of the Tank were in open position.
The OT immediately closed the drain valve and requested the on duty panel man to close the
tank’s LCV from the Distributed Control System (DCS). The drained Diesel was fully
contained within the closed drain system and the Corrugated Plate Interceptor (CPI).
Subsequently, road tankers were used to recover the diesel from the CPI.
