Fire Prevention and Control Fire at a construction site can endanger the lives of workers and others

who happen to be on the site. A fire during the course of construction also

can result in severe structural damage; destruction of machinery, equip-

ment or materials; and untimely delay in project completion.

It is essential, therefore, that an effective fire prevention and extinguishing

plan be developed before the onset of construction. The plan should be

closely followed throughout the course of construction. The fire protec-

duty. It is essential that the individual making the inspection be

competent in hazard identification as well as the corrective

measures required for elimination or control of all hazards noted.

The fire prevention inspection report should be written and re-

ceived by project management.

An important part of fire protection is proper maintenance of

equipment, particularly gasoline—or diesel-powered types. At-

tention should be given to fuel tanks and lines, carburetors, hy-

draulic reservoirs, and exhaust systems. For fixed equipment

such as power units for hoists, exhausts and muffler systems

should be clear of combustible materials. Equipment should be

regularly inspected and maintained by qualified personnel, and

complete records should be maintained. Give particular attention

to fueling methods.

Worker response in a fire-emergency situation is directly related

to training. All workers on site should be aware of the procedure

of reporting a fire and evacuating the worksite. These procedures

should be posted in prominent areas. All workers should be

trained concerning these procedures.

Emergency telephone numbers should be available to all work-

ers.

An alarm system should be provided to alert all workers of a fire

emergency. Designate a location where all workers will assem-

ble following evacuation and confirm that all personnel have

been safely evacuated.

Selected workers on each work shift should be trained in the

proper use of portable fire extinguishers. Training should ad-

dress the four classifications of fire, the type of extinguishers

suitable for each class and the proper technique to be used in

operating each type. Extinguisher suppliers or the local fire de-

partment may be willing to provide, or assist in, this training. In

the event of a fire, notify the fire department first. Then utilize

fire extinguishers by trained personnel for the initial small stage

of fire.

As many of you are aware the API 1169 certification is one that is gaining acceptance at an overwhelming rate amongst a vast amount of owner/operators within the oil and gas industry. In fact many of these owners are requiring 25% of their inspection staff to be certified now and 75% by the end of this year. What this means is that these owners are preferring individual inspectors who are certified over those who are not. This is creating a higher employment opportunity for those that are already certified. Near Misses Reported in October

Situational Awareness

When unloading pipe off a pipe truck, an employee was standing on the front of the trailer. As they were lifting the pipe, it would slide towards the employee.

Work was stopped and there was a talk with the employee about potential hazards. They got a lad der and secured it to the trailer to be able to get out of the danger zone.

Slips, Trips, & Falls

A contractor employee was walking down a slope (rather than walking down the ramp that had been cut by the hoe for that purpose) to enter the trench box via a ladder and slipped on loose dirt .

Spoke with this employee and made sure he knew there was nothing that was such a rush that he needed to take a short cut instead of using the ramp. The ramp was widened and an additional ramp was added as there were more than one trench box in the trench.

Communication

Proper communication is needed with the landowners when surveyors are on the ROW. Farmers were spraying a field with chemical and spray was carried with the wind.

Surveyors reported the incident and went home and showered then returned with no incident or injury.

Spotter

An employee was backing up a vehicle without a spotter. I stopped him and had one of the hands go over there to finish spotting the truck.

Contractors Lowboy was backing out onto a road without spotters/flashers. The road was a heavily used County Road.

The truck driver was stopped until a flagger arrived and a spotter was directing him. It was then safe to proceed. This issue was addressed with the entire crew the next morning at the safety meeting.

JSA

Conductors had been prepared for an insulation test. I asked for the JSA and noticed that this task was not on the JSA. We stopped the work until JSA was corrected with a tailgate meeting. Upon testing, the crew posted someone at the other end of the cables where they were exposed for the test and barri-caded the area off to prevent someone from electrical shock.
