budgeting/proposal purposes. does a tool like that already exist? Answer: It's difficult to provide a calculation for specific on-prem costs, because org's generally don't expose their on-

prem datacenter costs granularly enough to provide a true cost per VM calculation. As a rule of thumb, studies have shown an average of ~$90/month per VM (for small VMs) for just the HW and datacenter infrastructure associated with on-prem networks (just HW & DC operational costs - no SW licenses)

controller VM creation process as shown, but select your own VHDX file? Answer: Think of App Controller as a way to access or deploy VHDs in your VMM Library or in your Azure Library.

The VHD must first be loaded into the VMM library or Azure library. Once the VHD is there, it can be deployed again and again. So App Controller doesn't let you move from on-premises to Azure, but let's you deploy VHDs from either library.

their applications - it's really not cloud specific - it's the cost of business downtime for any IT solution whether it runs on-prem or in a public cloud.

individual's identity to prevent fraud/abuse. You may want to check with your school to see if they have access to Azure they can share, as an Educational facility they can apply for free Azure grants for faculty and students.
