1. The patient’s vestibular problems seemed to stem from the (labrum, labyrinth) portion of the ear.

2. Her vocal cords, situated in the (larynx, pharynx), appeared normal in size and shape.

3. Ms. Brown was diagnosed with nasal polyps after undergoing (rhinoscopy, pharyngoscopy).

4. The physician performed a lavage of the child’s ears to clear them of (serum, cerumen).

5. The (tympanic, tympanitic) membranes appear intact bilaterally with no evidence of infection.

6. Her (aural, oral) cavity had no masses or lesions, and her tonsils appeared normal.

7. My mother was diagnosed with a deviated (septum, sepsis) last year.

8. The patient’s hearing was evaluated using the (Weber, Wernicke) tuning fork test.

9. Mr. Hall was recommended to have a tracheostomy because his (glomus, glottis) was obstructing his airway.

10. The (tracheal, trabecular) rings in the throat were examined during inspiration and expiration and no clots were noted.

HISTORY: This is a 56-year-old woman who is referred for sinusitis and findings of abnormalities of epiglottis and lymph nodes on routine CAT scan. The CAT scan itself was reviewed. Physical examination reveals no palpable lymphadenopathy. The report on the CAT scan is multiple subcentimeter lymph nodes. These would not be palpable. They were not considered pathologic by the radiologist, Dr. Mark. Also, there is punctate calcification of the epiglottis. This is of no significance clinically. The patient is mainly complaining of left-sided face and neck and ear pain. She also relates that she is very sensitive to all odors and smells, and gets headache from smelling various substances.

PHYSICAL EXAMINATION: On physical examination, there is diffuse tenderness along the left neck and shoulder, left occiput and tenderness around the TM joint in the pterygoid muscle on intraoral palpation. The tympanic membranes are normal on physical exam. The patient also complains of noises in the ear.

IMPRESSION: This patient has a former TMJ syndrome more of a myofascial pain syndrome. The nasal exam showed moderate chronic rhinitis.

PLAN: The patient refused any medication for this. I would think that this patient would be just treated by neurological or orthopedic evaluation to make sure there is no cervical disc disease. She does relate pain running down the left shoulder and the arm. However, I see no ENT pathology that I would be able to treat. The patient refuses any nasal spray or medication.
