Assessment Diagnosis Planning Intervention Rationale Evaluation Subjective: “Pakiramdam ko nanghihina ako.” as verbalized by the patient Objective: the patient is weak the patient has limited range of movements minimized movements stays in bed most of the time Impaired physical mobility related to decreased strength and endurance. Short term Goal: Within the shift the patient will have a much more wider range of motion than before and slight increase in endurance Long term Goal: After 4 days of nursing intervention the patient will achieve full restoration of ROM, strength and endurance. Establish rapport Reviewed functional abilities and reasons for impairment. Provided assistance for range of motion exercise. Kept necessary utensils within reach of the patient. Encouraged patient to do self care activities such as oral care, walking exercise. To gain the patient trust and cooperation during the entire procedure. Identifies probable functional impairments and influences choice of intervention. Maintains mobility and functions of joints alignment of extremities and reduces venous stasis. Keeping all in reach, can greatly reduce the risk of accident to the patient. This is to enable the patient to regain muscle strength and keeping himself clean, and will gain independence. Within 8 hours of the shift, the patient has a much more wider range of motion than before and slight increase in endurance After 4 days of nursing intervention the patient achieved full restoration of ROM, strength and endurance.
