• SOAP notes contain the patient’s complaints, the physician’s findings, and the physician’s assessment and plan for treatment.

• O—OBJECTIVE (the physician’s findings as a result of a physical exam or evaluation)

• A—ASSESSMENT (the physician’s diagnosis or impression of the problem)
