I M P O R T A N TPLEASE READ CAREFULLY AND SIGN FRONT

Gulf Breeze Area Chamber of Commerce, hereinafter, Publisher, is authorized to insert advertising for the undersigned, hereinafter known as the Advertiser, and to run the amount of space for the time or number of issues indicated, at the price stipulated on the reverse.

Advertiser agrees to pay at the time copy is given to Publisher in a form approved by Advertiser for publication. Ad copy submitted shall be subject to Publisher’s approval and Advertiser is to have the privilege of seeing a proof, if so desired. If a proof is sent, and an approval is not given by deadline, the ad will run as is.

Artwork and special camera work will be paid for by the Advertiser when ordered by the Advertiser. Artwork created for above publication is only for that publication and is non-transferable to other publications.

Artwork created by Publisher is owned and copyrighted by Publisher.Advertiser may seek release of artwork and will pay a fee for release of artwork for

other publications. If original artwork or layout created by Publisher is used or recre-ated elsewhere, Publisher may seek damages including all costs, expenses and attorneys fees, for which Advertiser will be liable.

This contract may not be cancelled or terminated except by mutual consent. In the event of termination or cancellation, the Advertiser pays all expenses and costs incurred in the sale of this space and processing of the advertising to be used therein. Publisher will not be responsible for any space, copy, position, color, or other matters not otherwise noted in this agreement. Publisher will not be responsible for delays in publication.

A late payment penalty of $25.00 per month and 1.5% per month (annual percent-age of 18%) will be added to account balances 30 days past due. If payment is 60 days

past due, the balance of this contract will be due, and no further advertising will be run until the balance is paid in full including all late fees.

Publisher reserves the right to edit or reject material deemed inappropriate. In order to induce publisher to extend credit for advertising to the Advertiser and in consideration of Publisher extending credit to the Advertiser for publishing any adver-tising materials after this date at the request of Advertiser or its authorized agents, the undersigned guarantor hereby personally guarantees unconditionally and irrevocably prompt payment of any sums now or hereafter owed Publisher for advertising or relat-ed work performed at the request of Advertiser or its authorized agent.

Whether said sums are or will be due, Publisher shall not be obliged to notify the undersigned guarantor of the dates or amounts of such credit, and the under-signed guarantor waives demand, notice of default, and extension of time or other forbearance which may be extended by Publisher. Guarantor agrees and promises to pay a late charge fee of $25.00 and 1-1/2 PERCENT PER MONTH (ANNUAL PERCENTAGE OF 18%) on any unpaid past due balance.

This guaranty shall continue in force and be irrevocable for all payments, interest and penalties due on unpaid payments which unpaid payments are due prior to notice of cancellation of this guaranty by the guarantor.

This guaranty shall continue in effect until notice in writing sent by registered or certified mail, return receipt request, is received by Gulf Breeze Area Chamber of Commerce, 409 Gulf Breeze Pkwy, Gulf Breeze, FL 32561, Attention: Credit Manager. Said notice is to specify the date on which guaranty is to be terminated, said date not to be less than seven (7) days after the described notice is received.
