Thank you, for choosing to continue your athletic and academic career with us. Walsh University’s Athletic

Department’s goal is to minimize all risk of injury while participating in your athletic endeavors. The first line of

defense in helping to provide you with a safe environment to compete in is by obtaining a complete medical

history in compliance with the regulations and guidelines that the NCAA has established. Failure to complete

this required information will result in denied participation in any athletic activities.

This PowerPoint is a step-by-step instruction guide to help you fill out the required information. If at any time

PRE-PARTICIPATION PHYSICAL

• Walsh University requires all new and transfer student-athletes to provide an initial intake physical exam

• Physical Requirements

• Must be less than 6 months old (prior to start date of athletic participation)

• Physical Exam administered by an MD, DO, or their designate

• Once the physical exam is submitted it will be reviewed by a member of the Walsh University Sports Medicine Staff. If there are any questions, you may be required to submit supporting medical documentation and consult with one of our team physicians.

• Note – future physicals will be completed by Sports Medicine Staff (at the University’s expensive)

SICKLE CELL TRAIT

Sickle Cell Trait is one of the leading causes of death in college

athletics.

• The test for this condition is a blood test that was mostly likely

done in your newborn screening. If you have access to this lab

work, we will accept this for this requirement. If not, we ask that

you have the test performed and the results forwarded to the

Athletic Training Department.

• This test is Mandatory and there is no waiver.
