The Epidemiology and Biometry Research Program, DECA, NHLBI, seeks an echocardiography reading c e n t e r f o r a p r o j e c t i n which four f i e l d c e n t e r s w i l l r e c r u i t , examine, and fol low a t o t a l o f 5000 men and women (1250 i n each c e n t e r ) aged 65 t o 84 yea r s a t t h e b a s e l i n e examination i n a prospec t ive s tudy o f coronary h e a r t d i s e a s e s and s t r o k e . BACKGROUND

S t u d i e s have i n d i c a t e d t h a t when diagnosed e a r l y , and p r i o r t o c a p s u l a r invasion, t h e cu re ra te f o r p r o s t a t e cancer is p o t e n t i a l l y improved. I n a d d i t i o n , it has been r epor t ed t h a t tumor volume is a s s o c i a t e d with capacity t o metastasize. A t p re sen t , t h e r e is gene ra l consensus t h a t among imaging moda l i t i e s c u r r e n t l y a v a i l a b l e , ultrasonography o f f e r s t h e greatest p o t e n t i a l f o r e a r l y d i a g n o s i s and volume assessment of p r o s t a t e carcinoma. The a d d i t i o n o f known b i o l o g i c a l markers mayenhance t h e c a p a b i l i t y o f l a b o r a t o r i e s o r c l i n i c s with e x i s t i n g ul t rasonography c a p a b i l i t i e s are encouraged t o t a k e t h e l e a d e r s h i p i n response t o t h i s RFA. It is t h e i n t e n t of t h i s RFA t o i n i t i a t e network s t u d i e s among o r g a n i z a t i o n s f o r t h e purpose of e v a l u a t i n g ultrasonography i n diagnosing e a r l y p r o s t a t e cancer u s i n g uniform and s t anda rd ized approaches and t echn iques . According t o t h e Report o f t h e S e c r e t a r y ' s Task Force on Black and Minority Heal th (August 19851, t h e r e is a con t inu ing d i s p a r i t y i n t h e burden of d e a t h and i l l n e s s experienced by Blacks and o t h e r minori ty Americans compared with t h e popu la t ion as a whole. I n t h e United S t a t e s , s t r o k e is t h e t h i r d l e a d i n g cause o f dea th and a prime cause o f d i s a b i l i t y . A t least a h a l f m i l l i o n Americans each yea r s u f f e r an a c u t e s t r o k e . The o v e r a l l problem is even more imposing than annual incidence and

I n t h e Black populat ion, age-adjusted dea th ra tes f o r s t r o k e are almost twice t h a t o f t h e white popula t ion . Dramatic d i f f e r e n c e s between t h e e x t e n t o f t h e va r ious cerebrovascular r i s k f a c t o r s are thought t o e x i s t among minor i ty popula t ions , when compared wi th each o the r , and when each is compared with t h e popula t ion as a whole. Whereas some r i s k f a c t o r s , such as t r a n s i e n t ischemic a t t a c k s , are wel l known, t h e e x i s t e n c e of

'-o t h e r f a c t o r s and t h e i n t e r r e l a t i o n s h i p between va r ious pred ispos ing f a c t o r s remain unc lea r . Not enough is known about d i f f e r e n c e s i n t h e incidence of t h e va r ious k inds o f s t roke , a l though c l i n i c a l evidence sugges ts t h a t such d i f f e r e n c e s maye x i s t .

The St roke and Trauma Program o f t h e Nat ional I n s t i t u t e o f Neurological and Communicative Disorders and St roke (NINCDS) i n v i t e s g ran t a p p l i c a t i o n s f o r t h e support o f r e sea rch on t h e causa t ive mechanisms, prevent ion, and t rea tment o f dementia a s s o c i a t e d with cerebrovascular d i s e a s e . Although s t r o k e is t h e t h i r d l ead ing cause o f dea th and d i s a b i l i t y i n t h e United S t a t e s , t h e r e is a l s o a largenumber of v ic t ims of cerebrovascular d i s e a s e o r c e r e b r a l i n f a r c t i o n who experiencewidespread loss of neuro logica l func t ion t h a t may vary from minor memory loss t o d i f f u s e c e r e b r a l dysfunct ion but who do not show a t y p i c a l s t r o k e syndrome wi th t h e sudden onse t of f o c a l symptoms. The i n i t i a l complaint may be dementia with a s e v e r i t y t h a t depends upon t h e s t age , e x t e n t , o r anatomical s i t e of cerebrovascular d i s e a s e . The symptoms may inc lude minor f o c a l s igns , bu t t h e primary man i fe s t a t ion is a widespread loss of func t ion such as d i s o r i e n t a t i o n , loss o f motivat ion, apathy,i r r i t a b i l i t y , depress ion , loss of mental equi l ibr ium, and d e c l i n e o f memory o r s o c i a l withdrawal, a l l of which presumably r e s u l t from many small widespreadi n f a r c t i o n s t h a t i n d i v i d u a l l y cause only a s l i g h t d e t e r i o r a t i o n i n ove r t neuro logica l func t ion .

The u l t i m a t e o b j e c t i v e o f r e sea rch generated from t h i s program announcement is t o provide some form o f t rea tment e i t h e r t o prevent t h e d i s e a s e before it develops o r t o prevent progress ion once it has s t a r t e d . E f f e c t i v e t rea tment , however, must be based on accu ra t e d i agnos i s and t h e e l u c i d a t i o n o f t h e phys io log ica l , molecular, and c e l l u l a r causes o f t h e d i s e a s e .
