1 Introduction
1.1 Purpose
This document provides the release notes for the release of NilRead (software version 4.0.49.83795).
Please see important note in Section 3.1 regarding feature licenses.
This release is for English in all deployment regions, and for the Europe deployment region, German,
Italian, French, Dutch, and Spanish. Simplified Chinese is also supported. For other languages, please
contact Lexmark.
1.2 Intended Use Statement In the U.S.: The NilRead software application provides desktop and portable access to multi-modality softcopy medical images, reports and other patient related information for conducting diagnostic review, planning, and reporting through the interactive display and manipulation of medical data, including mammography and breast tomosynthesis. NilRead also allows users to collaborate by sharing application sessions. Lossy compressed mammographic images are not intended for diagnostic review. Mammographic images should only be viewed with a monitor approved by FDA for viewing mammographic images. For primary diagnosis, post process DICOM “for presentation” images must be used. On mobile platforms, this device is not intended for diagnostic use. In Canada: The NilRead software application provides desktop and portable access to multi-modality softcopy medical images, reports and other patient related information for conducting diagnostic review, planning, and reporting through the interactive display and manipulation of medical data. NilRead also allows users to collaborate by sharing application sessions. NilRead is for use with MRI, CT, Ultrasound, Digital Fluoroscopy, Digital Angiography, X-ray, Mammography, DBT, and Nuclear Medicine Images. On portable (mobile) platforms, this device is not intended to replace full workstations and should be used only when there is no access to a workstation. It is the user’s responsibility to ensure the software is used on appropriate hardware and to verify display image quality is suitable for the clinical application. 2 Interface Requirements & Dependencies
2.1 Hardware The system resources required by Nil depend on several factors including the usage scenario (e.g. 2D,
3D, etc.), resolution of monitors and expected performance (e.g interpretation vs referring physician).
For minimum hardware requirements, please contact Lexmark.
2.2 Software The server should be equipped with the following software:
Windows Server 2008 R2 or later. (64bit, US English system locale)
IIS v.7 or later.
.NET 4.5 or later.
SQL Server or Express (.NET 3.5 is required for SQL Express installation)

3 Installation and Support
3.1 Installation Lexmark or Lexmark’s authorized distributor will install NilRead software v4.0 at the customer’s site.
Feature licenses: For NilRead versions 4.0.27.77745 and later, the following optional license features
have been added, and therefore, need to be requested when the license is generated if the
corresponding functionality is to be enabled:
Multi-tenant
Adapter feature licenses for Mach7, Teramedica. Feature license required to access non-Acuo
private data stores (TeraMedica, Mach7) using proprietary API.
Important note re upgrades:
When upgrading a site with NilRead (version earlier than 4.0.27.77745) connected to Mach7
or Teramedica, the license file will need to be updated in order for any tight integration with
Mach7 or Teramedica to continue to be supported.
When updating a multi-tenant installation, the license file will not require updating

Preset enhancements: Ability to define “Per User” or “Per site” Windowing presets for
compatible modalities. To access this feature, go to Settings->Preferences->Modality
Preferences page then select individual modalities such as CT, PT, etc. from the drop down.
The following preferences previously stored in a browser cookie are now stored in the
database:
• DesktopTouchUi - use touch screen
• UiScalingFactor - scale UI for high resolution screens
• ScalePatientDir - scale patien directory
• UseGrayscaleRendering - render images in grayscale
• AlwaysShowViewerControls - series panel and priors always visible
This means that once a workstation is setup it will always remember the settings, even if the
cookies are lost. This also allows new users of the workstation under a different windows
account or browser to have a configured initial set of preferences.
Mechanism for specifying the login information for multi-tenant sites has been simplified. A
"Site" field has been added to the login form, with support for type-ahead (configurable -
"EnableLoginSiteDiscovery?"). Domain names are automatically populated as soon as the
correct site name is provided.
Non-Dicom data import enhancements: Nil supports automated import and Dicom CDA
encapsulation of non-Dicom data as long as a markup file is supplied providing minimal
patient/study metadata. No transcoding of the non-Dicom data is performed (e.g. AVI to MP4).

Added ability to switch splitting/ordering of series from UI:
For multi-phase series, it is now possible to switch the way a series is split (position vs time). This
can be accomplished via the context menu for multi-dimension series. It is also mapped to the
"num-multiply" key on the keyboard
Add ability to load HP layouts with priors in the background.
When a hanging protocol needs to load priors, Nil will wait for 2 seconds (length of time
configurable via a WaitTimeSpanForPriorLoadMs? configuration setting). If the priors are not
loaded, the anchor study is shown and the viewports with prior studies are displayed a message
"loading priors...." until the priors are available. This applies to Acuo-direct and the adapters
such as wado and mach7 adapters.
Loading time improved for sites for direct access to Acuo VNA configuration: When "streaming" mode is configured, the load time has been significantly improved. Streaming is applicable for 'Core' and "Clinical' configurations. For non-streaming mode (e.g. for "Interpretation" configuration), loading time has been improved approximately by a factor of 2 (in our testing scenario)
Added lock status to Patient Search. For clarity, added separate column for study status (locked, reports) from study selection column.
Anonymization enhancement for zero-ing a patient name
Complete rules for local study worklists and folders Nil 4.0 brings better consistency between Patient Directory (PD) and Patient Search (PS) controls. The PD control is used to browse and perform actions on studies in the local database/cache. Most attributes in the database may be shown as a column in the grid. A common configuration offers column consistency between PS and PD - for the columns representing the same Study attribute, both grids should have the same caption and default configuration (such as size and visibility) as specified in the configuration setting AvailableStudyListColumns. Users may now display columns not specified in this setting via the customization button.

download to create a permanent anonymized copy of the data.

When in a collaboration meeting, a user can verify the reading environment only if the "verify
read environment" message is on the screen. Once it's removed or if it's not visible because the
environment has been verified recently, it's not possible for the user to verify the read environment
without stopping the collaboration session. If starting a collaboration session on a tablet, the user is
instructed to verify the read environment prior to starting the collaboration session (Settings->Reading
Environment Verification).

1.2.840.10008.5.1.4.1.1.7 Secondary Capture Image Storage (version 4.0.48.xxxxx and later) Video Playback: Nil relies on the HTML5/Flash-supporting browsers for H.264 MPEG4 videos to playback video content inside Nil viewer window. For other video formats (as well as non-Dicom media documents) the original document content is delivered to the client and opened in a native application appropriate for the mime type of the document. Nil supports multi-video playback mode on desktop browsers and single-video playback on mobile browsers. Viewports playing video are decorated with video controls to play/pause/seek video content, mute/unmute/adjust volume, toggle full screen mode. Streaming videos from VNA: Studies found in the Acuo VNA can be seamlessly reviewed in Nil including video playback. Video Storage in Nil: Nil can ingest (NilFeed or zero footprint upload) and store videos as Video Photographic Image Storage (MPEG4 only) or Encapsulated CDA Storage, based the configuration setting. Default - Video Photographic Image Storage. Note: Media Foundation component has to be enabled on the server. Use the Add Roles and Features wizard from the Server Manager. Skip through to Features and select Media Foundation. Known issues:
- No XDS support for streaming videos. Video from an XDS source need to first be copied to Nil storage. Streaming from Acuo XDS to be added in future release
- On mobile devices multi-video playback mode is not supported. This is a limitation of the mobile browser technology.
- Nil installer does not contain codec for video transcoding to H.264 MPEG4 format. Customer may provide their own transcoding solution, encoder path and parameters are exposed through Nil configuration.
- Video playback performance – video streaming is performed from the Nil server, not a dedicated media streaming server.
- in case video playback is heavily used (multiple users playing video files simultaneously) it might impact overall Nil server performance and client viewing experience if infrastructure not dimensioned appropriately.
