The larger community of partners including Area Agencies on Aging
ADRN’s are a collaborative effort from the national to the local level
that serves as “no wrong door” systems
ADRN’s are not limited by state programs, but include all programs

◦Person-Centered: SEPs will work together to ensure
extended coverage hours for North Central Ohio ADRN Availability
Monday-Thursday 7a.m. - 7p.m. Friday 7a.m. - 5p.m. Saturday 8a.m. – 5p.m.
Accessible by consumers Willing/Able to complete LTCQ on every
consumer/call that comes through their door. SEPs will provide a similar experience regardless
of which agency the consumer initiated contact with

Services & Supports A web based system developed by Harmony for Ohio Incorporates use of Trigger Questions & Algorithms' Identifies potential for Medicaid eligibility Determines:
the need for a level II Assessment for waiver services: or determines list of appropriate referrals to be made

LTCQ Summary:◦7 page questionnaire on line◦Developed to screen for multiple
Long term service & support needs◦Determines if Assessment is needed◦Determines potential for Medicaid
eligibility.

◦Once LTCQ completed=consumer gets linked with a support navigator (if needed) within the SEP
◦Support Navigators provide options counseling to consumers according to the outcome of their LTCQ.

Strategy CafeDirections:1. There are 5 questions located at 2 separate
tables each, through out the room.2. Participants are asked to brainstorm ideas
surrounding the question at their table with the other participants at the table.
3. Each table has a designated Scribe who will record the ideas of the group
4. When time is up please relocate to a table with a question you have not yet had a chance to brain storm
5. Once we have rotated 5 times we will take a break & tally common themes for report out.
