    Summary:

    study aimed to identify the perceived problems of the

    student nurses in nursing health assessment,

    developed a proposed manual on patient

    assessment tool guide, validated and evaluated thecontents of the manual.

    study is developmental in nature as it developed a

    manual on Patient Assessment Tool Guide for Level

    IV Nursing Students of Notre Dame of the

    Dadiangas University.

    problems of the Level IV Nursing Students of

    Notre Dame of Dadiangas University

    112 students were used as respondents during

    this phase of the research.

    Instructors and NDDU BSN Level IV students on the

    proposed

    Manual on Nursing Health Assessment has a

    grand mean of 4.3 which means thatthe manual is highly comprehensive, acceptable,

    appropriate,

    and usable in conducting Health Assessment to a

    particular individual/patient.

    Conclusions

    Based on the result of the study, the Level IV Nursing

    Students of Notre Dame of Dadiangas University has factors

    that affect their ability to assess patients effectively;

    a Manual on Patient Assessment Tool Guide was developed

    as a suggested reference for the students in conducting Nursing

    Health Assessment;

    The Clinical Instructors view the manual as a good help for

    Level IV Nursing Students;

    The Level IV Nursing Students of Notre Dame of Dadiangas

    University view the manual as highly comprehensive,

    acceptable, appropriate, and usable.

    Recommendations:

    Advocate the use of the Manual during Related Learning

    Experience of Nursing Student of Notre Dame of Dadiangas

    University;

    Encourage the conduct of developmental research like this;

    Make the manual available to all Clinical Instructors and Nursing

    Students of Notre Dame of Dadiangas University for uniformity

    purposes

    students undergo improvement of the developed manual;

    Future researches similar to this nature of the study could also

    include the alumni and some of the nursing schools in the

    SOCCSARGEN area;

    future researchers should continue to assess and to study the

    skills, knowledge and attitude of Nursing Students.
