Ochsner needs YOUR help! The Holidays are fast approaching and we do not have a card to send to
family and friends!
Ochsner would love to feature your design on our holiday card this year:
• Students can create a card design as a class or individually. Only one
design from each third grade class to be submitted. Please include first name, grade, teacher name and school on the back of card.
• The design should be created on a white sheet of 81/2 X 11 paper • Ochsner will create the design into a folded, finished 5X7 card. • Designs will be on display in the hospital with patients, visitors and
staff choosing the winner • The winning design will be created into the official Ochsner

I understand that my child’s name, grade, school, and teacher’s name will be released to Ochsner Holiday Card. Furthermore I understand that the artwork will be on display in the hospital with patients, visitors, and staff choosing a winner. The winning design will act as the official Ochnser Northsore Holiday card for 2015 and will be duplicated for distribution.
