Helpful hints for filling out Officer Training Form, also the actual form to fill outTRANSCRIPT
It’s April, which means it is time to register your club for the leadership training sessions this summer. If your club has not elected your 2012-2013 officers, please pass this information to the incoming President and President Elect once your elections are completed.
PRE-REGISTRATION IS REQUIRED
To register please review the following guidelines: You must submit the enclosed form complete with a $30.00 payment for EACH participant.
If payment is not received WITH THE FORM, it will be sent back immediately. The registration form is located on the back of this letter. You can also find the form online
under the Leadership Training Icon. PRINT the information on the form clearly. Circle the appropriate officer position on the registration form that you will be holding
starting OCTOBER 2012. Mail the registration form to Civitan International, Attn: Stacy Sabo, PO Box 130744,
Birmingham, AL 35213-0744. You may also pre-register by emailing a completed form to [email protected] or by fax (205-592-6307) with a credit card.
A pre-registration packet will be sent with training materials and meeting location information once we receive your registration. Secure your materials in a safe place until the session.
