Description: This graph displays the number of non-deleted appointments that were scheduled during the fiscal year (July 1 – June 30) according to the visit type. It also displays the number of no shows according to the visit type. Source: Appointments

Program Enrollment:




Description: This graph displays the number of unduplicated patients who visited the practice for a specific program in the practice/health department during the fiscal year (July 1 – June 30). If a patient had multiple visits for different programs, they are grouped into the program according to the most recent visit. Source: Billing

Percentage of No Shows:




Description: This graph displays the percentage of no shows compared to the total number of scheduled visits during the fiscal year (July 1 – June 30) according to the visit type. Source: Appointments

Visits by Program:

Description: This graph displays the number of visits to the practice/health department during the fiscal year (July 1 – June 30) according to the type of program the visit was for. Source: Appointments




Average Visit Time by Visit Type:

Description: This graph displays the average visit time for an appointment by visit type based on the check-in and check-out time (only entries with a check-in and check-out time are included in the average). Source: Appointments




Average Visit Time by Provider:

Description: This graph displays the average visit time for an appointment by provider based on the check-in and check-out time (only entries with a check-in and check-out time are included in the average). Source: Appointments




Chronic Condition Percentage:

Description: This graph displays the percentage of patients with a specific chronic condition) as a percentage of the total number of patients that visited the practice during the fiscal year (July 1 – June 30). The chronic conditions for this graph are asthma, diabetes, hypertension, obesity, and smoking. Obesity is defined as patients who have a BMI greater than 25. Smokers are defined as those who smoke currently, either every day or some days. Source: Encounter note, Social History

Meaningful Use Performance Overview:

Description: This graph displays the names of each physician (blacked out in the picture) as well as sparkbars displaying how well each physician is meeting their meaningful use goals, and a link to jump to a page with more detailed information. Physician names have either green, orange, or red circles next to their names, indicating they have either met all of their meaningful use goals (green), failed to meet one to seven of their goals (orange), or failed to meet eight or more of their goals (red).




Meaningful Use Performance (Specific Practitioner):

Description: This graph displays how much deviance an individual physician (name blacked out in the picture) has from each specific meaningful use goal. Source: Meaningful Use Data Tables

Program Data Table:

Description: This table contains specific statistics for each program during the fiscal year (July 1 – June 30). On the surface level it displays a general overview of the enrollment for each




program and the number of visits to each program, however, the table can be further drilled down, as seen below. If a patient was enrolled in multiple programs, they are grouped into the program they were most recently enrolled.

Description (continued): This table is a drill-down into “Income by Federal Poverty Level” for the “Adult Health” program. The table displays a number of sub-categories for that drill-down, in this case specific thresholds based on the income of each patient. For each sub-category, the number of patients that fall into that sub-category are displayed, the percentage of all patients in that program are displayed, and specific numbers for each age group are displayed. Additionally, at the bottom of the table there is a total row that displays statistics for all patients in that program. Source: Encounter note
