HealthWorks: A Family Wellness Center is a concierge chiropractic practice.

This means that we’ve ditched the old-school insurance way of doing things for a better model — one where you are not limited by what your plan will or will not pay (hint: “will not pay” is far more likely, even if it appears you do have benefits). Our doctors (and patients!) were frustrated with this antiquated model and we have started from scratch with something that really works!

A concierge model means that we made our prices real, affordable, and predicta-ble. Your care plan in our office will include access to each of our services at exact-ly the time you need them. Custom care for real people who want to get and stay healthy for life!

During your third visit as a new patient, once we know you and are certain of

what you will need to get well, our doctors will prescribe a level of member-

ship that will suit your needs. Our care plans start at sixty per month for our

youngest members, please scroll through all eight pages to find the care

plan range for your age group.

All services will be applied to account balances at the full cost of care at time of service. Each item on this plan will be received as they are clinically recommended by the doctor. A no-show charge of $50 per visit will be applied

toward your account (not exceeding monthly plan fee) should you not reschedule within 24 hours; no reminder calls will be made. A 30-day written notice is required to properly close you case file if you wish to terminate care for

any reason. Payments will be automatically deducted from the account provided on the plan agreement start date and each month thereafter (ending on ___________________), at which time progress will be evaluated and rec-

ommendations for future care will be made.
