“Permit to Work, discussed Points” Decommissioning Stage: Important Note: Spading for any equipment shall be done only after the completion of the Battery limits spading. Battery limits spading: One permit per unit (BA set is mandatory when working in Hydrocarbon,H2S and Flare lines) Equipment spading: One permit unit wise, Tag numbers for all equipment to be listed down in the permit, (Except for Reactors, a separate permit is required for the stacked reactors) Execution Stage: Man-way Opening: one permit unit wise. Tag numbers for all equipment need to be attached in the permit. A Single Entry permit including cleaning “by hand tools & manual cleaning” for each individual equipment. All disciplines can use the entry permit for internal checking and visual inspection only. (Everyone must register in/out in the log sheet next to the Man-way). Internal scaffolding: Permit for each individual equipment is required. Separate Hot permits for each activity like: Sandblasting, painting, welding/repair work, Inspection activities (NDT) inside the equipment (PA to ensure one activity at a time except Cleaning may be considered at different level after providing proper segregation without affecting people working nearby or underneath. “Permit to Work, discussed Points”

Decommissioning Stage:

Battery limits spading: One permit per unit (BA set is mandatory when working in

Execution Stage:

External scaffolding: One-permit unit wise except for the critical scaffolding like “extension

from platform or for the scaffolding having a height more than 45 meters which require

structural engineer approval”. For heat exchangers entry permit and box up are not required, unless there is request from

any discipline to check the shell internally then entry permit is required (entry/box up

permits to be raised for heat exchangers which are opened from one side only – Two sides

open shell does not need an entry permit)

Small vessels entry permit are not required. (entry permit is required only if a person wants

to put his head inside the equipment)

PSV’s: Two permits (unit wise listing all tag numbers) are required; one for

dismantle/servicing and the other for installing back.

Box-up individual permit for each equipment.

Miscellaneous like level gauges, motors, switchboards, motor coolers, UPS, battery

chargers, control valves, level switches, analyzers, and junction boxes – one cold work

permit unit wise for each equipment category for removal and re-installation.

Electrical Isolation permit: individual permit for each discipline working on the equipment is

required, the newly LOTO procedures shall be implemented (as per incident

recommendation from last shutdown).

General Guidelines: Operation & safety representative in permit team will handle all permit related to QP &

contractor scope of work.

Maintenance representatives in the permit team will handle the entire permit related to

contractor scope of work only.

QP scope of work permits, the members in execution team shall handle all permits related

to QP Scope (issuance/approval/tracking/closing). (To be decided after permit team

structure is approved).

Instrument permit related to QP & Contractor scope will be handled by respective

instrument representative in team. (To be decided after permit team structure is approved).

Permit working hours (validity) for the critical path (and near critical path) equipment /IATs /

PCRs activities will be around the clock (Critical Paths and near critical path activities shall

be provided by MCP/2). Also, Heat treatment, IRIS and water hydro jetting at yard shall be

around the clock and sent officially to permit team.

Representatives from all concerned sections will be available in night shifts.

The permit team to coordinate with site engineers, safety & operation in plant to expedite

the approval process. (To be decided after permit team structure is approved).

Contractor to provide the names and contact numbers for execution supervisor for each unit

for any clarification from the permit team. (To be decided after permit team structure is

approved).

Lifting plan approval mechanism to be agreed upon before shutdown start.

Permits shall be displayed at site.

Blind list & diagram for major equipment should be approved before shutdown and attached

with the entry permit. (The approved blind list and blind diagram shall be kept ready in

advance at least for equipment requiring confined space entry instead of Generic statement

“Spade close to the equipment” and struggling during the shutdown. This exercise will save

time during execution.)

All the spading shall have identification tag

One Vehicle Entry Permit per unit is good enough for entry and for small Rigging and

transportation activities inside a unit.

Separate Vehicle Entry Permit to be used for critical Rigging activities.

ALL Shutdown Permits should be Valid till the completion of that particular jobs and round

the clock (24/7 basis) i.e. NO NEED to make Extensions on each Permit as it could delays

the jobs. However, the GAS TEST for any HOT job on daily / shift basis must be done

regularly (if advised in HWPs).

All confined space related permits must be closed before equipment box-up and all permits

in the unit must be closed before unit hand-over.
