Ted (71 years old) I have been attending Plenty Valley Disability Services for over 10 years now. I come 4 days a week and do a range of activities from Woodwork, Gardening, Exploring parks, yoga and others. I love going out in the community and making new friends. The photo below is of when I went on a train trip on the Puffing Billy. I got to meet the driver which was really nice as I have a passion for trains. If you would like to get more out of your day, I definitely recommend Plenty Valley Disability Services. Liam (21 years old) I've been coming to PIP placement for nearly 3 years now. I like all of the programs I participate in. They are, Numeracy and Literacy and Hospitality run at PRACE (PRESTON/RESERVOIR ADULT CENTRE OF EDUCACATION), Independent Living Skills (Volunteer work at a warehouse that provides food for the needy, Shopping and Cooking, learning self advocacy skills through VALID, and Drama). My favourite program is Melbourne Explorers because I catch the bus and train and I have the opportunity to visit different parts of this wonderful city.

Intake The Intake/ Assessment officer, PVCH Disability Services is responsible to undertake screening duties by providing first point of contact to participants. Assessing the NDIS Plan outline against PVCH service suite, scoping and matching services and staff, completing all intake documents and developing a Service agreement and scope of services with participants. The Intake/Assessment officer, is required to collaborate with other disability service providers/bodies and mainstream services in order to develop and improve the effectiveness of service coordination.
The NDIS is designed to give you more choice and control over how, when and where your supports are provided, and gives you certainty you will receive the sup-port you need over your lifetime. Here are some examples of how the NDIS could look to support Individuals: Getting out and about – Developing the skills and confidence to access your local community . Developing independence at home – Building the
practical life skills that allow you to make the most of your home environment. Staying connected in your community – Services and
activities to help you connect with your local community, supporting your involvement in recreational, cultural and sporting activities. Education and learning – Services that are designed to assist you with learning new skills throughout your life. Working and contributing to your community –
Preparing you for the skills required for employment, training or volunteer opportunities in your community.

Our services PVCH services are focused on supporting participants to learn new skills as well as maintaining current skills, like self-help and independence. We also encourage participants in learning how to maintain relationships, develop new friendships and engage in community activities. Whatever stage you are at in life, you or your representative can talk to us about how our services can help you achieve your goals.
