• Pre-filled catheters are NOT recommended for use and any patients currently prescribed these should be switched to one of the foley catheters in the formulary.

• Catheter trays should no longer be prescribed or used as they are not cost effective.

1. Indwelling catheters

• Short/medium term use (one to four weeks)

These are no longer recommended for use in primary care. Use is restricted to the secondary care setting only.

• Long term use (four to 12 weeks)

Prescribing advice: • Up to two catheters can be prescribed at one

time.

• They should be prescribed as required - usually every three months. If required more frequently, advice should be sought from the Ayrshire & Arran (A&A) catheter guidelines on AthenA or from your local continence advisers.

3. ISC cathetersIf the patient has been referred by the Continence Service, please continue with the catheter that has been advised. For all other patients, please contact the Continence Service for advice.

4. Catheter leg drainage bagsSterile leg bags contain one pair of non-sterile latex gloves and soft elasticised cotton Velcro straps. They are supplied in boxes of 10.

Prescribing advice: One box of 10 should be prescribed every two months only. Leg bags should be changed every five to seven days, as per manufacturers’ instructions.

5. Catheter night drainage bagsPrescribing advice: Single night use bags are recommended for use in patients who use leg bags.

No more than three boxes of 10 bags should be issued every month.

6. Drainable night bags Prescribing advice: For infection control reasons drainable night bags are not recommended for connection to leg bag systems.

7. Lidocaine and Chlorhexadine GelPrescribing advice:• Lignocaine hydrochloride 2%, chlorhexidine gluconate

solution 0.25% is a sterile local anaesthetic and lubricant for urethral mucosa. This must be used for catheterising both male and female patients. It may also be used for supra-pubic patients.

• Its use can prevent injury to the urethra and as a consequence reduces the subsequent risk of urethral damage.

• It also helps the reduction of pain on insertion.

• There is a slight risk of anaphylaxis from use of large doses anaesthetic gel in patients with cardiac arrhythmias.

• To be effective the anaesthetic gel must be left inserted for four to five minutes prior to catheterisation.

• Please prescribe the product by name - Instilagel

• To avoid wastage, this can be prescribed as an individual item.

8. Catheter valvesThese products should not be used without assessment of bladder function.

Contra-indications:

• Radical prostatectomy

Cautions:

• Reduced bladder capacity

• No bladder sensation

• Cognitive impairment

• Insufficient manual dexterity to operate the catheter valve

Prescribing advice: One box of five should be sufficient for one month supply. Valves should be replaced every five to seven days

9. Penile sheathsThese should be measured and fitted by appropriately trained staff. Measuring should only be done using the manufacturer’s measuring guide.

Prescribing advice: One box of 30 should be sufficient for one month supply.

Skin integrity should be regularly monitored. Sheaths should be changed every 24 hours.
