2015 start to develop the PRIME Physiobade

  • Goals of todays course

introduction of principle of the fascialearning of the basic techniquesuse and train these techniquesevaluating strategies for treatment

  • The traditionell view of fascias

We all learned some facts about the fascia:

It's a thin membran between the muscles.

It's a sleeve around the organs.

To understand the muscles, we have to isolate the muscle an remove everything else.

  • The modern view of fascias

The fascia is an important organ of the body.

    It designs 'muscle-chains'.

    It helps to transport informations.

    it absorbs and returns energy.

the fascia connects everything with everything.

newer studies show that the fascia can contract. It reacts to stress.

during isometrical work the fascia has a special job.

  • Some basics about the fascia

If you get hurt some strukture may be torn, ruptured or broken.

In surgeries we try to fix the broken parts.

    Nobody fixes the fascia!

  • adhesions of the fasciaIn a case of a disturbence or

adhesion of the fascia, impedences occur in the

    tissues and misinformation can happen.

Adhesions should be removed as soon as possible.

  • Why does nobody fix the fascia?

The structure of the fascia seems to be much like a 3D-version of a spiderweb.

Think about glue between your palms and pull the hands apart slowly. These fibres give you an idea of the flexible but breakable structure of a fascia.

  • Blading

Blading a variant of Instrument Assisted Soft Tissue Mobilisation (IASTM)

Blading follows the basic principles of the Gua Sha, a technique of the traditionel chinese medicine (TCM)

The main goal is to release adhesions of the fascia.

    Tools are used to

make it easier for the therapist

Special edges and curves are used to reach the area as good as possible

In traditional treatments spoons are used made of jade and horn

you can reach superficial deep layers of the tissue as well.

Different tools give different pros and cons.

We developed the Prime Physioblade to combine as muss pros as possible.

  • The Prime Physioblade

after using a lot of different variants of IASTM-Tools we took all the benefits of each of them and combined it in one tool.

    one tool for the most

Most of the tools are much more expensive and can deal with just a small amount of jobs. You need whole sets of tools to cover the whole range.

    different curves

    different points in different sizes

one side optimized for the superficial layers

one side optimized for the deeper layers

when the word DEEP and the logo is shown you are working deep!

    wide grip-holes

    Additional holes for stabilization

    Stainless steel

  • Kontraindikations

We highly recommend NOT to use the Prime Physioblade in following cases

Woundings and surgery scars which are not yet closed

    tumors

    active Implants

    Pacemaker, insulin-pumps for example

    acute Infections and imflamation

    Trombosis

    use of anticoagulants

  • Indications

We positive recommend to use the blade in case of

mechanical restriction of the Range of motion

Pain related restriction of the Range of motion

    Tissue problems like

    Fibrosis

    Adhesion

    Celulite

    old injuries

    Torn or ruptured muscles

Chronicle reduction of the range of motion

    Scars

Insufficient local suplience of the tissue

  • Effects of treatment

    Hyperaemisation

    breaking up crosslinks

Mobilisation of the different layers of the tissue.

(Re-)Activasion of the regeneration of the cells by causing controlled microtraumata

    Stimulating of mechanoreceptors

  • Some Infos for your clients

To avoid complication and stress with your clients, I recommend to inform your clients and do some paperwork.

    The treatment may be painful.

Ripping, sore or cutting feelings are common during the treatment.

The irritation of the skin may stay for a few hours.

Hämatome may occur but will go away in a few days.

a little pain or ill feeling like sore muscles may come up.

irritations of nerves can happen but won’t stay for more then a few minutes.

Talk to your client and make a clear codeword just in case of too much pain.

  • Blading- basic techniques -

We talk about 5 different techniques

    diagnostic scratch

    Point treatment

    Area treatment

    Sportsmassage

    Separating

    Lymphatic strike

If you are familiar with this basics you can treat nearly everything and develop your own strategies individually.

  • How to find the problemThe first step to solve a problem is to locate it.

Simelar to the standard protocol you can use the following

    Inspection

    Listen to the client

    feeling of stiffness

    reduced range of motion

Feeling of ripping off and pulling

Body language of the client (according to FDM)

    Palpation

    Diagnostic scratch

  • Diagnostic scratch

By slightly scratching over an area you will find some abnormalities

    Getting stuck in the tissue

    the skin turning red quickly

    Hear scratching

    Vibration of the blade

You use the blade in an angle of 45 degree to the skin. No pressure ist used,. The own weight of the blade is enough.

  • Point treatment

You work in a small area (like a coin) with one of the smaller parts of the blade

Variant 1: You hook into the skin an push into the stiff area, like a scar.

Variant 2: You use the blade to bring pressure on a small area like a trigger point.

  • area treatment

With one of the edges you can treat a larger area

    Pulling or pushing is possible

we recommend to use just one direction at a time.

  • Sportsmassage

you fix a part of the muscle in a shortened position (flexion of the joint) and pull the muscle through this fixation by moving the joint in extension.

In the classical version the movement is made by the therapist

According to myofascial techniques the client can make the movement.

  • Separation

the blade is inserted vertically between two parts of a muscle.

the blade is will be moved to separate these two muscles.

    Twisting is possible

We highly recommend this technique at the erector spinae.

  • Lymphatic strike

according to the area treatment you can use the blade to bring lymphatic liquid in motion.

we recommend to use the superficial side an pull with just little pressure

the direction of the strike is from distal to proximal towards the lymphatic knotts.

like the manual lymphatic pattern we recommend to start proximal and continue distal.

  • Special: Thoraco-Scapular-Joint

the blade can be used to work underneath the scapula.

we recommend to prepare the area by using different techniques around the scapula.

to get under the scapula you should have your client in a relaxed position. You put the Blade next to the edge of the scapula and pull the scapula over the blade by grabbing the shoulder and moving the shoulder and the scapular over the blade.

After that you can work under the scapular with different techniques.

  • Special: Intercostalneuralgia

The client should be in a relaxed postion.

thew client my be in in a stretched position of side of the chest you like to work on

    Work pointy between the rips

Area treatment over the rips is recommended

  • Special:Fasicia inside the Fossa olecrani

inside of the Fossa olecrani we find a Fascia, which is a problem during the extension of the elbow very often.

the elbow should be in a relaxed position in a 90 degree angle.Point treatment into the fossa olecraniiwe recommend the treatment of the triceps brachii in addition.

  • Thank you!

You are welcome to develop your own techniques and post videos, comments or fotos on our

    Facebook site.
