Problem Identification & Resolution (PI&R)
PI&R program identifies, documents, evaluates, corrects, and
trends incidents, problems, non-conformances, findings,
recommendations, and areas for improvement
PI&R program provides a formal process to improve personnel
safety, plant and equipment reliability, event and error
prevention and promote continuous performance improvement
through organizational learning
PI&R Applicability
Establish responsibilities to identify, review, evaluate, trend PI&Rs,
and verify completion of actions
Classify PI&R’s by type and priority
Initiate automatic notifications based on specific criteria
Determine level of analysis based on the risk and uncertainty
Establish expectations and a methodology to identify precursors
and trends to improve site and organizational performance
PI&R Threshold
PI&R reports should be written to document an actual or suspected
problem or an actual or suspected hardware, process, or program
deficiency or non-conformance. PI&Rs may also be written for
conditions adverse to business or with negative economic impact.
The following is a list of examples for which a PI&R is appropriate. The
list is not all inclusive and should be interpreted conservatively:
