Re: Heparin monitoring

Please be informed that on or about November 15th, Torrance Memorial’s Laboratory department

will be offering the Anti-Xa test on all patients requiring heparin monitoring instead of the current

Heparin aPTT test. This is in corroboration with the Pharmacy department.

The Anti-Xa test, will be available two ways;

Anti-Xa, unfractionated

Anti-Xa, low molecular weight.

Both are measured by the same assay but will have different reference ranges.

HOW DO YOUR PATIENTS GRADE YOUR COMMUNICATION SKILLS? Questions asked of in-patients regarding physician communication are: During this hospital stay, how often did doctors treat you with dignity and respect? During this hospital stay, how often did doctors listen carefully to you? During this hospital stay, how often did doctors explain things in a way you could understand? Ratings include Never, Sometimes, Usually, Always. ALWAYS is the only answer that counts towards your score. The most recent physician score was 80% which seems pretty good except that score is at 34

Press Ganey percentile rank. That means that the physician communication at other facilities is scoring much better than Torrance Memorial. Please think about these questions when you are speaking with your patients and their families. We are currently working on a reporting methodology that will allow us to drill down on improvement opportunities.

STEM THE TIDE: ADDRESSING THE OPIOID EPIDEMIC

Background:

“Every year millions of Americans use opioids to manage pain. Doctor-prescribed opioids are appropriate in some cases, but

they just mask the pain. Reliance on opioids has led the worst drug crisis in American history. More than 40% of US opioid

overdose deaths in 2016 involved a prescription opioid.”1

What:

Torrance Memorial Medical Center has formed an Opioid Stewardship Steering Committee to raise the awareness and form

strategic goals to stem the tide.

First steps:

Limit outpatient fills for ER visits and surgical patients to 5 days (prescriber can modify as needed), starts 10/31/18

Use non-opioid pain medications

Provide alternative therapies, like cold/heat therapy and physical therapy

Identify at risk patients: Assess if recent opiate use (opioid experienced) or no history of opioid use (opiate naïve)

Standardize monitoring practices

Starting October 2nd prescribers are required to review the state database (CURES 2.0) for any for any controlled

substance for a discharged patient. Exceptions: discharge prescriptions less than 5 days for a surgical case, and less

than 7 days for an emergency room visit (per CA Medical Board) 2

Reduce the consumption of opioid containing medications

Educate patient, families, and caregivers about the risk of prescription opioids and safe ways to manage pain
