Project21SD is ready to tackle childhood obesity with a comprehensive community awareness, education and intervention campaign. Project21SD is a visionary campaign that promotes innovative approaches to address key health concerns and secure funding to support accessible services within the PPH area. It features a partnership between Palomar Pomerado Health (PPH) and the San Diego Chargers’ all-time leading rusher, LaDainian Tomlinson (LT) who wears jersey #21.

LT serves as spokesman for Project21SD and has a personal stake in the subject because his own family has been touched by obesity, diabetes and stroke. Specifically, the childhood obesity project will raise awareness about childhood obesity, educate kids and families on healthy choices, and provide screenings and services to treat childhood obesity.

“We see LT as a role model and cheerleader for physical fitness,” explains Sheila Brown, Chief Clinical Outreach Officer, who is in charge of implementing the Project21SD childhood obesity campaign. “We are targeting fifth graders, who are still young enough to change their eating habits, as well as first-time families. We want to work with families to prevent future obese children.”

Childhood obesity has tripled since the early 1970s. Being overweight exposes children to serious health problems, now and in the future, such as cardiovascular disease, cancer and diabetes, which account for two-thirds of all deaths in California and are largely preventable.

The 2005 Community Health Check conducted by PPH discovered that 13.9% of children between two and five years of age are overweight, while 20.4% of children six to 19 years old are overweight. They may be following their parents’ lead, as 44% of adults within the North County region served by PPH are considered overweight or obese.

Project21SD will implement coordinated activities to overcome childhood obesity. Guarded like the Chargers’ weekly playbook, specific details of the childhood obesity campaign will be revealed as the program gets underway within the next few months. As part of the American Public Health Association’s Healthy People 2010 Objectives for Prevention and Control of Childhood Obesity, Project21SD has adopted five goals for children and families within the PPH service area:

• Participate in more physical activity

• Spend less time in front of TV or computers

• Eat less fat and sugar in food

• Choose more healthy food

• Develop incentives that reinforce ongoing environmental and behavioral change

These goals will be addressed through coordinated activities including health screenings, community outreach activities and partnering with school districts to teach families such skills as role modeling and advertising.

“What excites me about the Project21SD childhood obesity campaign is that it presents a remarkable opportunity for PPH to embrace and enhance the scope of its mission,” says Nick Yphantides, M.D., Chairman of the San Diego County Childhood Obesity Task Force and Medical Consultant to PPH’s Health Development, the district’s grant making division. “We spend too much time focused on treating disease, illness and the consequences of human behavior. With Project21SD we have the opportunity to focus on what health care should be, which is wellness, health promotion and prevention of costly illnesses and diseases.”

Lactation SERVICES

Anxious to zip into your pre-pregnancy jeans? Breastfeeding can actually boost your effort to lose those extra pounds gained during pregnancy because you expend additional calories each time you nurse. Experts agree that nursing moms should wait at least two months postpartum to lose weight to give your body time to recover from childbirth and establish a good milk supply.

Before trying to lose weight, check with your physician or lactation consultant for sound nutrition and exercise guidance. Remember that your nursing baby will generally take all he needs nutritionally from your breast milk, which means your own nutritional needs could be short-changed if you try to cut corners with a quick weight loss plan. Aim to lose one or two pounds weekly by increasing your activity level while reducing your intake by about 100 calories per day.

Healthy Tips:

• Avoid “empty calorie” snack foods such as cake, candy or soda.

• Focus on eating foods that provide plenty of lean protein (poultry, fish, beans,) and calcium (low-fat milk, yogurt).

• Drink at least six to eight glasses of fluids daily to replace the water that goes toward milk production.

• Eat three smaller meals daily plus two healthy snacks in between. This will keep your energy level high and prevent hunger pangs.

• Engage in regular, moderate exercise such as walking.

• The food you eat passes along in breast milk within four to 24 hours. If your baby responds unfavorably to a certain food you have eaten, try to avoid that food for awhile.

Think of the NICU (Neonatal Intensive Care Unit) like a fire department.

“You don’t really appreciate that it’s available and ready to respond until you need it,” says David J. Golembeski, M.D., a board-certified Neonatologist and Director of Neonatal Services for Palomar Pomerado Health (PPH). “Having a NICU at both Palomar Medical Center and Pomerado Hospital should provide a sense of security to the community, knowing that we have two first-class NICUs to serve families in North County.”

The Community Level (tertiary) NICU at Palomar Medical Center (PMC) combines the latest technology with a commitment to family-centered, integrative health care to meet the special needs of infants requiring intensive care. Pomerado Hospital’s Intermediate Level (secondary) NICU treats premature infants and other newborns who do not require the higher level of care available at PMC. When necessary, infants may be transferred from Pomerado to PMC’s NICU.

“We have made many recent changes to assure that we can meet the growing demand for the best possible care for high-risk babies,” says Dr. Golembeski.

PowerfulNewEquipmentforTinyInfantsOne of the key enhancements is the addition of new “Bear Cub” infant ventilators. Infant ventilators breathe for babies when they are too sick to breathe on their own.

This new type of ventilator allows the infant to trigger a breath and then the respirator automatically synchronizes with the infant’s breathing pattern, rather than forcing the baby to take a breath. The ventilator has a digital display with audio and visual alarms that tracks, breath by breath, what is happening and alerts staff immediately if something goes wrong.

“Currently, there is a debate in neonatal medicine as to how much oxygen a pre-term baby should get at the time of delivery,” says Dr. Golembeski. “Our new resuscitation tables have the ability to blend oxygen with air in precise ratios, based on the amount of oxygen in the infant’s blood.”

Since maintaining the body temperature of preemies is critical, new incubators combine an open warmer that uses radiant heat

with a clamshell-like top that closes over the babies to keep them continuously warm with moistened air.

SkilledStaffProvideAdvancedCareTwo new neonatologists recently joined the NICU team and NICU nurses trained in advance life support (ALS) are available to assist with high-risk deliveries. “The ALS program is not present in every NICU, but has certainly been the foundation of our NICUs,” says Dr. Golembeski.

The multidisciplinary NICU team also includes specialized respiratory care providers, nutritionists, occupational, speech and physical therapists, lactation consultants, laboratory and radiology technicians, and maternal-child social workers. They continually participate in educational activities to be able to bring the latest treatment options to the NICUs.

“For example, highly trained technicians are now doing echocardiograms in the NICU, where in the past these tests were referred to cardiologists from Rady’s Children’s Hospital,” Dr. Golembeski says.

In addition to providing advanced neonatal services, PPH is dedicated to preventing pre-term deliveries by identifying high-risk pregnancies early. Last year, the Perinatal Testing Center opened at PMC. This program is offered through a partnership between University of California San Diego Medical Center and PPH. The perinatologists – physicians who specialize in the care of high-risk pregnancies – are full-time faculty at the UCSD School of Medicine who work by referral from obstetricians in the community to assess high-risk pregnancies.

“Although our NICUs are high tech, we are also high touch,” says Dr. Golembeski.

One practice is Kangaroo Care, in which parents hold their tiny premature infant skin-to-skin, which provides close natural contact and warmth. Studies have found that Kangaroo Care helps preemies grow faster and leave their incubators sooner, while providing parents with the confidence.

“Getting the family and baby ready for discharge is our goal,” says Kathleen Powers, R.N.C., B.S.N., Interim Director, Pediatrics and Neonatal Intensive Care Units. “We like for parents to spend as much time here as possible, learning to care for their tiny infant. That may include spending the night just before discharge, which gives the family an opportunity to provide overnight care and feel more comfortable about bringing their baby home.”

“I am proud to say that our NICUs are well equipped and the staff is highly trained to care for most pre-term infants and other critically ill infants,” says Dr. Golembeski. “These recent advancements to our technology and training are extremely helpful to parents because it allows us to keep some of the premature babies here that we would have transferred to Children’s Hospital in the past.”

With over 23 years of experience as an individual and family counselor, educator and therapeutic foster parent for 27 children, Shelley Cowper-Smith has learned a thing or two about successful parenting.

“Parenting is the most important and influential task in our society,” says Cowper-Smith. “Research shows that a child’s brain develops most dramatically during the early years of life. Through the Peaceful Parenting program, we educate parents, grandparents and caregivers about the important role they play in their children’s first years.”

Palomar Pomerado Health offers the Peaceful Parenting program in conjunction with Jewish Family Service of San Diego funded by the FIRST 5 Commission of San Diego County. It is designed to give parents of children up to age five the necessary tools to raise an emotionally healthy family. A free six-week series will be held at the new Jean McLaughlin Women’s Center for Health and Healing located in the Pomerado Outpatient Pavilion in Poway from 6:30 – 8:30 p.m. on Thursday evenings from January 24 – March 6 (no class on February 14.)

“Each of the free classes is a blend of instruction and discussion tailored to meet the needs of the parents,” says Cowper-Smith. “We cover lots of practical tips and skills as we work through the steps outlined in the book, Systematic Training for Effective Parenting. Lessons include developmental ages and stages, communication skills and positive discipline techniques.”

“Parents thrive with good support and effective tools that build their confidence,” continues Cowper-Smith. “Although our ever evolving society brings special challenges, it is possible to succeed and be a strong family in today’s world.”

Parents who complete the free program will receive a special kit for new parents featuring a DVD with helpful parenting information, a parenting resource guide, and information on important topics such as health, nutrition and obesity prevention, child safety, early literacy and finding quality childcare.

Mommy & Me yoga is the perfect way to stay in shape and spend quality time with your child. Class begins with “baby-style centering,” songs sung just for them, followed by an active Hatha Yoga class incorporating poses using your baby. You’ll wind down with wonderful relaxation! Toys are provided for your baby – just bring a blanket! Dads, grandparents and anyone who loves your baby are welcome! Even at this young age, your toddler is learning that he is rewarded with positive attention — hugs, laughter, praise — when he behaves in ways that people around him think are “right,” and that he is punished or ignored when his behavior is “wrong.” The social interactions you’re most likely to witness now are basic — waving, smiling, playing peekaboo, and following simple instructions — but these are all first steps in establishing his personal social style that are learned through imitation. A 16-month-old is also able to initiate displays of affection and he’ll give back what he receives. If you show affection with hugs and kisses, he will likely, too.

Are you a first-time “dad-to-be” in need of some basic training? Your life is about to change drastically and if you don’t know what to expect, this is your opportunity to gain some informal, hands-on experience. This class is intended for expectant fathers in the second or thvird trimester of their baby’s pregnancy. It is a “fathers only” class featuring special guests – Veteran Dads (BT graduates) and their newborn babies. Learn what to expect during the last trimester of pregnancy, birth and the first few weeks at home. Plus, discover practical tips on diapering, feeding and soothing the newborn. Leave the class with a boost of confidence to be the best dad you can be!

Sudden infant death syndrome (SIDS) is the abrupt, unexplained death of an infant younger than one year old. While it’s impossible to imagine the unthinkable accidental death of a precious baby, SIDS is the leading cause of death in children between one month

and one year old, and claims the lives of about 2,500 children in the

U.S. each year. SIDS occurs most often in babies between the ages of two and four months, although premature babies seem to be at risk for a longer period of time.

Thanks to public awareness and preventive measures, the rate of SIDS has been reduced by half since 1983 and is at an all-time low. Still, fear of the mysterious, unexplained condition keeps new parents awake nights and checking on their baby frequently. While the cause is still not fully understood, health-care professionals recommend following these tips until your baby is at least 12 months old:

• Place your baby on his or her back to sleep, even for short naps. Infants who fall asleep on their stomachs should be gently turned onto their backs. Reserve “tummy time” for when your baby is awake and someone is watching.

• Use a firm sleep surface, such as a crib mattress covered with a fitted sheet.

• Keep soft objects and loose bedding away from your baby’s sleep area. Don’t use pillows, comforters or pillow-like bumpers.

• Keep your baby’s head uncovered during sleep.

• Maintain a comfortable temperature in the nursery – warm but not too hot.

Your pediatrician has just told you that your child is very sick and needs to be admitted to the hospital.

Naturally, you’re worried and unsure what to do next.

Since 2006, North County children and their families have benefited from a partnership between Palomar Pomerado Health (PPH) and Rady Children’s Hospital Pediatric Hospitalists. The affiliation provides convenient access to high quality inpatient pediatric care close to home as well as clinical expertise in pediatrics, for which Rady Children’s is known.

“In general, we care for children who require hospitalization for respiratory disorders, asthma, pneumonia, croup, dehydration, jaundice, urinary infections and fevers,” says Edward Epstein, M.D., Medical Director of Inpatient Pediatrics at PPH. “We tend to be much busier during the winter months when colds and flu are common.”

Family-CenteredCareKathleen Powers, RNC, BSN, Interim Director of the Pediatrics & Neonatal Intensive Care Units at Palomar Medical Center says “On average, we treat about six youngsters each day, which allows the nurses to spend more time with each child and family.”

Located on the fourth floor of PMC, the Pediatric Unit is close to the birth center and neonatal intensive care unit (NICU). Having all inpatient pediatric services at PMC in close proximity to one another encourages a strong collaboration between the nursing units and the physicians who care for the pediatric patients.

PediatricHospitalistsAlthough the term may be unfamiliar, this model of staffing assures that pediatric patients receive timely care from board-certified pediatric hospitalists from Rady Children’s Hospital. These pediatric hospitalists are actually on staff within PMC’s Pediatric Unit every weekday, where they examine young patients, order required tests and treatments, and manage the overall treatment plan. Local pediatricians cover the unit on evenings and weekends in order to ensure that continuous care is provided for the patients and for anyone needing hospital admission during those times.

“We also have a small cadre of pediatric nurse practitioners who are trained to provide inpatient care,” says Dr. Epstein. “There is always someone on site or readily available to provide prompt, responsive care.”

Having pediatric hospitalists working within the Pediatric Unit streamlines the care patients receive because their entire focus is on the hospitalized patients, rather than patients with office appointments. The national trend for both pediatric and adult patients is to have hospitalists manage inpatient hospital care while assuring close communication with the patients’ primary care physician.

“While pediatric hospitalists are available to care for all kids admitted to PMC, the child’s own physician is, of course, able to manage the inpatient care for their own patients should they so desire,” says Dr. Epstein. “When a community physician entrusts us with the inpatient care of their patient, we work closely with that physician to communicate treatment progress and always refer the young patient back to his or her physician when it’s time to go home.”

“What makes this unit special is the quality of the pediatric staff,” says Powers. “Our nursing and medical staff have great clinical and people skills. Having a sick child is upsetting to any parent, but knowing that we have such high quality care right here in the community is reassuring, not to mention more convenient for families.”
