    2010 PVREA RENEWABLE ENERGY REBATE APPLICATION.

    Section 1Instructions and Program Overview1.1Instructions: This form must be completed if you are applying for a PVREA renewable energy rebate. You must also

    complete a Poudre Valley REA Generating Facility Application For Residential (Only Residential?) Renewable

    Energy Net-Metering Systems in conjunction with this form and deliver both forms together, if you are applying for arebate. If you do not intend to apply for a rebate, you need only complete the Poudre Valley REA Generating Facility

    Application For Residential Renewable Energy Net-Metering Systems.

    1.2Program Overview: PVREA Renewable Energy Rebates are provided on a first come first service basis at this time

    We do not use a lottery system but are considering it for 2011 and beyond. Applicants are chosen that meet the Rebate

    Requirements (section 2) and will be offered a rebate reservation letter defining the rebate amount, construction

    completion deadlines and any other conditions required to connect your system to ours. Applicants that begin

    construction prior to signing a rebate reservation letter from PVREA will be disqualified from rebate eligibility. If you

    are not able to commit to using your rebate and installing your system, we will go on to the next eligible applicant. A

    your request, your application will either be either thrown away or re-dated with the date you tell us that you cannot

    proceed. If re-dated you will hold that place in line for future rebates. However, if new criteria apply, you will have to

    meet the new criteria. If you choose to have your application thrown away, you will have to reapply to qualify for any

    future rebates. Once you accept your reservation letter, you must complete a Poudre Valley REA Generating Facility

    Application For Residential Renewable Energy Net-Metering Systems.

    Section 2Rebate Requirements2.1Minimum Rebate Requirements:

    Applicants must be Poudre Valley REA customers. Application must be for Photo-Voltaic Solar systems. Wind and Solar thermal rebates are not available fromPVREA at this time. Applicants must have an energy audit of their home that includes a blower door test. The audit must be completed

    by a RESNET or BPI certified auditor. While no pass/fail criteria are in place at this time, we strongly encourage

    you to take the energy audit seriously and implement suggested improvements. Doing so may save you money by

    requiring a smaller PV system and energy efficiency improvements always show a better pay back than renewable

    energy system installation.

    Section 3Verifications and Commitments3.1 The Installer must complete and initial the following:

    3.2 The Poudre Valley REA customer must read and initial the following:
