How to Complete a Remediation Plan

1. Click on My Staff TKES/LKES Plans. 2. Click on selected teacher to initiate Professional Learning Plan for

Remediation & Additional Conferences. 3. Click on Professional Learning Plan for Remediation & Additional

Conferences.

4. Click on Remediation Plan Acceptance.

5. Scroll down past the Professional Learning Goal Setting and click on NEW beside Remediation Plan.

6. Fill out all required parts. a. Select the reason for a plan using the drop-down box. b. Select the Date and the Projected End Date. c. Select Performance Standard(s) needing remediation (no more than 4

standards per 8 week time period).

d. Add Targeted Areas of Improvement. e. Add Actions the Educator will take to complete plan. f. Add Support Resources and the Timeline which must include the date of

the remediation conference.

g. Add Data/Evidence for Review h. Add attachments if available. i. Click Save and Exit.

7. The teacher goes into his or her Platform and into the Remediation Plan Acceptance section.

a. Click New. b. Fill out all required parts. c. The teacher hits Save and Exit, then Accept.

If the Remediation Plan is completed (successful), please follow step 8. If the Remediation Plan is extended, skip step 8 and complete only steps 9-11. If the Remediation Plan is not completed (unsuccessful), skip steps 8-11 and go to step 12. Successful completion of Remediation Plan: Evaluator: 8. The evaluator goes into the Remediation Plan Status section. a. Click the Edit button. b. At Professional Learning Plan Status, click Completed. c. Click Save and Exit. Click Accept.

d. Return to the Remediation Plan Acceptance section, review the Timeline

and details, and indicate that the plan is complete and note the date. e. Click Accept.

Teacher: f. The teacher goes into Remediation Plan Status section. g. Click Accept, to acknowledge that they are accepting the Remediation Plan

as complete.

Extension of Remediation Plan: If Remediation Plan is extended, complete steps 9-11. 9. The steps below will be followed for the 4 week extension of the plan.

Evaluator: 10. The evaluator goes into the Remediation Plan Status section. a. Click Edit. b. At the Professional Learning Plan Status drop-down, click Extended. c. Click Save and Exit only. Do NOT hit Accept.

11. Return to the Remediation Plan Acceptance section. a. Click New. b. On Remediation Plan drop-down, select Evaluator Required. c. Select Beginning and Ending dates. d. Select relevant Standards. e. Fill out all required parts. f. Click Save and Exit.

12. Return to Remediation Plan Status. a. Click Edit. b. At Professional Learning Plan Status drop-down, click Not Completed. c. Click Save and Exit. d. Click Accept.

13. Return to Remediation Plan Acceptance. a. Click Edit. b. Enter details regarding the unsuccessful Remediation Plan into Timeline.

Note specifics regarding the teacher’s failure to meet the Remediation Plan expectations and criteria.

c. Click Save and Exit. d. Click Accept.

Teacher: 14. The teacher goes into Remediation Plan Status section. a. Click New. b. Fill out all required parts. c. Click Accept, to acknowledge awareness that Remediation Plan is

unsuccessful.
