DES MOINES PUBLIC SCHOOLS MONUMENT SIGN UNIT PRICE QUOTE

Scope of Work QUOTE # Q7190 January 30, 2015

The Des Moines School District is working to install new free standing monument signs at several of its school locations. The District will award all signs to one firm based upon the lowest responsive / responsible bidder. The basis of award in indicated on the quote form. This unit price quoted in each round will be used as the basis of individual purchase orders for each sign on a per school basis in that round. The quoted unit price shall cover the period of time indicated in the scope of work and shall cover any of the signs listed in that round. The district at its sole discretion reserves the right to terminate this unit price arrangement at any time or modify the quantity of signs to be undertaken.

REQUIREMENTS:

1. Submit a signed quotation on the attached quote form . Quotes to be submitted not later than 3:00 PM on February 11, 2015. Submit proposal to the District at 1917 Dean Avenue, Des Moines, Iowa 50316. Faxed or emailed copies of proposals are acceptable. 2. Questions shall be directed in writing to [email protected] 3. It is anticipated that the successful firm will be notified on February 16, 2015. 4. A individual purchase order will be provided from the Des Moines Public Schools for each school

sign. Exclude all State of Iowa sales taxes and use taxes from your proposal. The District will provide a tax exempt certificate with the purchase order.

5. All monument signs shall be two sided. Internal lighting to be fluorescent with white Lexan behind letters. School name should be in font ITC Giovanni and Elementary School should be in Gill Sans. Lettering sizes may be adjusted slightly for each sign. Electronic display shall be Daktronics 20mm Galaxy 3500 with monochrome red letters and 4 lines of display with wireless remote capable of 1000’ signal. Include Venus 1500 V4 software and web seminar interactive web based software training. Include two DMPS logo signs mounted on the brick base. Brick base, foundation and electric will be by others, furnish steel columns early to be incorporated into foundation. If selected, provide shop drawings and schedule for approval. Any old signage shall be removed by the Owner.

6. Selected sign company to obtain and pay for the permit. Include the permit cost in your unit price.

7. The quotes shall include all field verifications and complete installation. 8. Final installations shall be complete and fully operational. 9. The District will pay 50% of the purchase order amount when the sign company pays the

message board supplier the down payment. Payment documentation required along with the invoice. The balance will be paid after the installation of the sign is complete and accepted.

10. The successful firm shall submit a current acceptable insurance certificate. Bid bonds are not required. Performance / payment bonds are not required unless the unit price per purchase order exceeds $25,000. If bonds are required, bonds will be required for each purchase order.

11. The successful firm shall guarantee each sign for a period of one year from acceptance of the sign. 12. The successful firm shall furnish an insurance certificate with the attached limits. 13. The successful firm shall sign the attached sex offender form for each purchase order 14. Attached are the standard conditions that are part of the Board of Adjustment approval. Quotes shall

incorporate these conditions / capabilities.

DMPS INSURANCE REQUIREMENTS Insurance by Contractor The Contractor shall purchase and maintain such insurance as will protect it from claims set forth below which may arise out of or result from the Contractor’s operations under the Contract, whether such operations be by himself or by any subcontractor or by anyone directly or indirectly employed by any of them, or by anyone for whose acts any of them may be liable. All such insurance shall be subject to the approval of the District for adequacy of protection, and shall include a provision preventing cancellation without thirty (30) days’ prior notice to the District in writing. ACKNOWLEDGMENT AND CERTIFICATION

__________________(“Company”) is providing services to the Des Moines Public School District (“District”) as a vendor, supplier, contractor or subcontractor and/or is operating or managing the operations of a vendor, supplier or contractor. The services provided by the Company may involve the presence of the Company’s employees upon the real property of the schools of the District.

The Company acknowledges that Iowa law prohibits a sex offender who has been

convicted of a sex offense against a minor from being present upon the real property of the schools of the District. The Company further acknowledges that, pursuant to law, a sex offender who has been convicted of a sex offense against a minor may not operate, manage, be employed by, or act as a contractor, vendor or supplier of services or volunteer at the schools of the District.

The Company hereby certifies that no one who is an owner, operator or manager

of the Company has been convicted of a sex offense against a minor. The Company further agrees that it shall not permit any person who is a sex offender convicted of a sex offense against a minor to provide any services to the District in accordance with the prohibitions set forth above.

This Acknowledgment and Certification is to be construed under the laws of the

State of Iowa. If any portion hereof is held invalid, the balance of the document shall, notwithstanding, continue in full legal force and effect.

In signing this Acknowledgment and Certification, the person signing on behalf of the Company hereby acknowledges that he/she has read this entire document, that he/she understands its terms, and that he/she not only has the authority to sign the document on behalf of the Company, but has signed it knowingly and voluntarily.
