REQUEST FOR PROPOSAL
Fitness and Aquatic Center Flooring Spring Lake Public Schools (hereafter referred to as SLPS) is accepting bids for the purpose of replacing or re-surfacing flooring at the Fitness and Aquatic Center in the following locations: .
• Weight room • Cardio area • Track • Multipurpose Room • Group Exercise • Competition Pool Seating Area
All areas will also receive rubber base. This work is to take place beginning August 6th with a schedule conducive to both parties. Bid Response All bidders submitting a response to this Request For Proposal (RFP) must provide the following Information on specified forms:
• Pricing Proposal Form • Notification of Bidder References Form • Affidavit of Bidder Form
Proposals must be received no later than 2:00pm on Thursday, July 12, 2018 at the following address:

General Terms and Conditions
1. Any document submitted after the due date and time will be rejected as not meeting the mandatory
requirements for the RFP. 2. Any cost for developing proposal documents is the responsibility of the bidder and shall not be
chargeable in any manner to SLPS. 3. SLPS expects that all supplies, materials or equipment offered by a bidder shall meet or exceed the
specifications set forth in this RFP. Further, it is SLPS’ intent that this RFP permit competition. Accordingly, the use of any patent, proprietary name or manufacturer’s name is for demonstrative purposes only and is not intended to curtail competition. Whenever any supplies, materials or equipment requested in the RFP are specified by patent, proprietary name or by name of the manufacturer, unless stated differently, such specification shall be considered as if followed by the words “or comparable equivalent,” whether or not such words appear. Notwithstanding the above,SLPS, in its sole and absolute discretion, shall have the right to determine if the proposed alternate/equivalent products/brands submitted by the bidder meet the specifications of this RFP and possess equivalent and/or better qualities. It shall be the bidder’s responsibility to notify SLPS in writing if any specifications or suggested comparable equivalent products/brands require clarification by SLPS prior to the date and time established in this RFP. SLPS reserves the right to accept or reject any and all Bids, either in whole or in part, to waive any informalities or irregularities therein, or to award the contract to other than the low bidder, in its sole discretion.
4. SLPS reserves the right to reject any and all proposals, to waive omissions, irregularities, or clerical errors not affecting substantial compliance with the bid specifications, and to accept any bid it deems to be in the best interest of the School District.
5. SLPS reserves the right to reject any part of a specific proposal for any reason. Bidders not willing to accept award of partial bid must so indicate as part of their proposal.
6. SLPS reserves the right to rebid all or part of this project at a later date. 7. SLPS reserves the right to accept any bid, even if it is not the lowest bid. 8. SLPS reserves the right to modify conditions and specifications, by mutual agreement with the
selected supplier, both at the time of acceptance of the bid and subsequent thereto. 9. SLPS reserves the right to increase or decrease quantities purchased, both at the time of acceptance
of the bid, and subsequent thereto. 10. The bidder is responsible for any applicable Federal and State taxes. All bids are to be made taking
this into consideration. The bidder shall be solely responsible for such payments and shall indemnify the owner and hold him harmless for any assessment and payment of same.
11. The successful bidder shall comply with all applicable Federal and State laws, regulations, ordinances, and codes, which are in effect on the cutover date and which place obligations on the bidder with respect to its performance under this agreement.
12. Bid prices quoted shall remain in effect for a period of not less than 1 year from the bid award date. 13. References: Bidders are required to list three (3) references. 14. It is understood and agreed that the delivery date and/or date of installation AFTER receipt of a
purchase order is the seller’s best offer. In its acceptance of any quotation offer, SLSP is relying on the promised delivery date and/or installation as material and basic to its acceptance, unless otherwise indicated. In the event of seller’s failure to deliver as and when promised, SLPS reserves the right to cancel its accepting order, or any part thereof, and seller agrees that SLPS may return all or part of any shipment so made, and may charge seller with any loss or expense sustained as a result of such failure to deliver as promised.
15. SLPS is exempt from applicable Federal and State Taxes, Michigan and Federal Tax-Exempt. All quotation offers and/or bids are made taking this fact into consideration.
16. Payment Terms: SLPS will negotiate specific payment terms with the successful bidder.

Bid Evaluation SLPS may make such investigations as deemed necessary to determine the ability of the bidder to supply the items and the bidder shall furnish to SLPS, all such information and data for this purpose as SLPS may request. SLPS reserves the right to reject any bid if the evidence submitted by or investigations of such bidder fail to satisfy SLPS that such bidder is properly qualified to carry out the obligations of the contract and to complete the orders contemplated therein. In determining which bid is the lowest responsible bid, the Board, in its discretion, may take into consideration not only the amount of the bid, but the following criteria as it deems appropriate and may give such weight thereto as it deems appropriate.
• The bidder’s financial ability to complete the contract successfully and on time without
resort to its surety;
• The bidder's prior experience with comparable or more complex contracts;
• The bidder’s prior history for the successful and timely completion of contracts;
• The bidder’s prior experience on other contracts with SLPS, including the bidder’s
demonstrated ability to perform its contract in accordance with the applicable contract
documents;
• The bidder’s compliance with federal, state and local laws, rules and regulations; and
• Depending upon the specific items to be furnished by the vendor, other essential factors.

A. Samples for Verification: Full-Size units of color and pattern of floor tile, and for each fluid applied floor provide sample of at least 2 inch sample applied to a rigid backing.

1.4 QUALITY ASSURANCE
A. Installer Qualifications: A qualified installer who employs workers for this project who are competent in techniques required by manufacturer for floor installation indicated.
B. Game Line and Markers: Comply with the requirements of National Federation of State High School Associations for Sport Activities indicated.
1.5 PROJECT CONDITIONS
A. Environmental Limitations: Comply with flooring manufacturer’s written instructions for substrate temperature, ambient temperature, humidity, ventilation, and other conditions affecting flooring application.
1. Do not apply flooring until spaces are enclosed, and all athletic equipment has been removed from the area, as well as any other projects in the area being completed.

B. Conditioning Period: Begins not less than seven days before the flooring application, is continuous through application, and continues not less than three days after application.
1. During the conditioning period, maintain an ambient temperature between 65 and
75 deg. F and not more than 50 percent relative humidity in the spaces to receive flooring.
2. After conditioning period, maintain relative humidity and ambient temperature planned for the building occupants.
C. Close spaces to traffic during the flooring application and for the period after
application recommend by manufacturer, but not less than required for the flooring system, including game-line and marker paint to cure.

3.1 EXAMINATIONS
A. Examine Substrates, with Installer (Applicator) present, for conditions affecting performance of flooring including substrate moisture content. Begin flooring application only after unsatisfactory conditions have been corrected.
3.2 PREPERATION
A. Prepare and clean substrate according to manufacturer’s written instructions. 1. Remove any curing compounds, form-release agents, dust, dirt, grease, oil, and other
contaminates using mechanical means. 2. Treat non-moving substrate cracks and conditions joints to prevent cracks from
telegraphing (reflecting) through flooring according manufacturer’s written recommendations.
3. Protect substrate voids and joints to prevent flooring resins from flowing into or leaking through them.
B. Protect walls, floor openings, athletic equipment inserts, electrical openings, door frames, and other obstructions during installation. Cover floor and wall areas at mixing stations.
3.3 APPLICATION
A. General: Mix and apply flooring components according to manufacturer’s written instructions. 1. At substrate expansions, isolation, and other moving joints, install continuous joint
of the same width through flooring.
B. Polyurethane Flooring and Resilient Base Mat 1. Adhesively Apply Resilient Base Mats to substrate according to manufacturer’s
written instructions. a. Base Mats must not be in compression. Leave gap of width recommended in
writing by manufacturers at butted base-mat sheets, walls, floors openings, athletic inserts, electrical openings, door frames, and other obstructions.
b. Roll base mats to set them into adhesive and eliminate air pockets. c. Repair ridges at seams, loose areas, and air pockets according to manufacturer’s
written instructions.

3.4 GAME LINES AND MARKERS
A. Mask flooring surfaces at the game lines and markers, and apply paint to produce sharp edges. 1. Where the game lines cross, break minor game line at intersection, do not overlap
lines 2. Apply game lines and markers in widths and colors according to sports association
indicated more aggressive course grit for the first pass.
3.5 PROTECTION
A. Protect fluid-applied sports flooring to ensure flooring and finish are without damage or deterioration until at the time of project completion.

B. Adhesives: Water-resistant, mildew-resistant, non-staining type to suit products and subfloor conditions that complies with flammability for installed carpet and is recommended or provided by carpet manufacturer.
C. Seem Adhesive: Hot-melt adhesive tape or similar product recommended by carpet manufacturer for sealing and taping seams and butting cut edges at backing to form secure seam and to prevent pile loss at seams.
6.- EXECUTION 6.1 EXAMINATIONS
A. Examine Substrates area, with Installer(Applicator) present, for conditions affecting performance of flooring including substrate moisture content. Examine carpet for type, color, pattern, and potential defects.

6.2 PREPARATION
A. Use trowelable level and patching compounds, according to manufacturer’s written instructions to fill cracks, holes, depressions, and protrusions in substrates. Fill or level cracks, holes and depressions 1/8 inch wide or wider, and protrusions more than 1/32 inch, unless more stringent requirements are required by manufacturers written instructions.
B. Remove coatings, including curing compounds, and other substrate that are in compatible with the adhesives and that contain soap, wax, oil, or silicone, without using solvents. Use mechanical methods recommended in writing by manufacturer.
C. Broom and vacuum clean substrates to be covered immediately before installing carpet.
6.3 CARPET INSTALLATION GENERAL
A. Comply with manufacturer’s recommendations for seam locations and direction of carpet: maintain uniformity of carpet direction and lay or pile. At doorways, center seems under doors in closed positions: do not place seams perpendicular to door frame, in direction of traffic through doorway. Do not bridge building expansion joints with continuous carpet. Carpeting in circulation areas, such as corridors and related areas, shall be installed with seams parallel to the long dimensions of the corridor or area. Seams perpendicular to the long dimension will not be permitted, unless approved. When corridor width is less than the width of carpet rolls, no seams will be permitted.
