SCHEDULE C-1 – QUESTIONNAIRE

Proponents should respond to the questions under each heading clearly and concisely. Responses should be limited to twenty (20) pages (not including Appendix).

The tables below are expandable.

Through the information provided in your Proposal, the City of Port Coquitlam expects to gain an in-depth understanding of a Proponent’s experience, capabilities, capacity to provide the Goods and/or Services outlined in Schedule A - Scope of Goods or Services.

The Evaluation Committee will evaluate the Form of Proposal based on the Evaluation Criteria outlined in the “Instructions to Proponents”, as seen in Section Error: Reference source not found of this RFP.

A detailed, clear and concise response from Respondents is required in order to facilitate the evaluation process. “Yes” or “No” responses will not be acceptable unless the City has requested only a “Yes” or “No” response to the specific item. Any requirement with no response will be presumed to mean it is either not included or not available.

MANDATORY CRITERIA

Proposals not meeting mandatory requirements will not be evaluated. The items listed below are mandatory. See also RFP Instructions to Proponents Section 5.3.

1. The proposal must be received at the Closing Location before the Closing Time as set out on the cover page and in accordance with Instructions to Proponents, 3.4

2. Proposal must be submitted in English in accordance with Instructions to Proponents, 2.2.3. Proposal must be submitted using one of the submission methods set out on the cover page

and in accordance with Instructions to Proponents, 3.34. The proposed Rescue must be fully compliant with the authorities and publications as

identified in ‘Schedule A – Scope of Goods or Services, under the Requirements section as seen in section three (3).

5. The specifications identified as Mandatory in the ’Requirements’ column as seen in Schedule A, 2.4 are mandatory. No substitutions are acceptable.

A) INTRODUCTION LETTER (1%)Responses will receive higher marks for this section that identifies:- a concise introduction letter that outlines their proposal and articulates the advantages of their

Q33. 5% Specification Review

USE OF BRAND NAMES: Unless otherwise provided in this solicitation, the name of a certain brand, make or manufacturer does not restrict (Proponents) to the specific brand, make or manufacturer named, but conveys the general style, type, character, and quality of the article desired. Any article which the City, in its sole and absolute discretion, determines to be the equivalent of that specified, considering quality, workmanship, economy of operation, and suitability for the purpose intended, shall be accepted. The Proponent should clearly and specifically identify the product being offered and to provide sufficient descriptive literature, catalogue cuts and technical detail to enable the City to determine if the product offered meets the requirements of the solicitation.

The City will be the final authority on what constitutes “equivalent” or meets acceptable thresholds of standards or quality.

The proposed vehicle must comply with all Canadian Federal and Provincial government regulation and requirements and all other authorities as identified in Schedule A – Scope of Goods or Services.

Whether specifically mentioned herein or not, all parts necessary to provide complete and efficient Rescue shall be furnished. Such parts shall conform to the best current engineering practices of the industry relative to design, structure, quality of materials and workmanship.

Proposals shall be based on the specifications as identified in ‘Schedule A – Scope of Goods or Services’, the Mandatory requirements identified throughout and as otherwise identified throughout this RFP document.

Specifications identified below in the ‘Requirements’ column marked as ‘Mandatory’ must be met. Proposals not meeting the mandatory requirements will not be considered for evaluation. Items identified as ‘preferred’ in the ‘Requirements’ column, allow for equivalent alternatives to the equipment specified and may be considered if full descriptive data on proposed alternatives is clearly indicated in the column pertaining to each and every section and paragraph on this form. The City reserves the right to determine, at its sole discretion, whether the alternatives are equal to the equipment specified. Should your proposed alternatives lead to cost savings or operational efficiencies, please elaborate.

ALTERNATIVES TO SPECIFICATIONSProposals shall be based on the specifications. Equivalent alternatives to equipment specified may be considered if full descriptive data on

The City of Port Coquitlam reserves the right to determine, at its sole discretion, whether the alternatives are equal to the equipment specified.

Any supporting literature provided for alternatives shall refer to the specification page number.

Each "NO" indicated shall be fully explained. If a section or paragraph contains an empty column, where the bidder neglected to indicate "YES" or "NO", it is assumed the bidder is not conforming to the requirements of this specification or paragraph. SCHEDULE C-2 – FINANCIAL PROPOSAL

Responses will receive higher marks for this section that identifies:- A lower financial cost- Multiple payment schemes or strategies, and processes for managing currency differences

Note: All pricing provided should be in Canadian funds and exclusive of Taxes

FEE PROPOSAL (35%):

1. Proposals should include a pricing spreadsheet that provides a breakdown of costs by major project activity/phase, including effort, relevant charge out rates/consulting fees, and total costs such as, but not limited to: software or hardware expense, travel costs, accommodations, optional services or components, etc.

2. Include a schedule for pricing of the Rescue, itemized schedule for the equipment provided and cost efficiencies as applied to the Rescue cost. All equipment proposed should be listed in a separate costing sheet that is easily accessible by the City.

3. Provide a final summary sheet providing an overall project cost.

FEES AND PAYMENTS:

1. The Respondent offers to supply to the City of Port Coquitlam the Goods and/or Services for the prices plus applicable taxes as follows to the City of Port Coquitlam Public Works Building, 1737 Broadway Street, Port Coquitlam, BC.

2. For each payment option proposed, provide a breakdown of the associated costs.
