risk assessment of scanned charts
DESCRIPTION
How to make Risk Assessment of Scanned ChartsTRANSCRIPT
RISK ASSESSMENT OF SCANNED NAVIGATIONAL CHARTS USED FOR THE VOYAGE FROM BAHIA BLANCA, ARGENTINA TO SINES, PORTUGAL
Ship’s name – LPG/C Maersk JadeWork Activity being Assessed – Use of Scanned Navigational Charts BA 3135 // 3134 // 1870 // 3133 // 3636 (Reason – The Vessel was diverted, enroute to Houston, towards Sines, Portugal. At the time she had left Port and, with no means available to obtain paper Charts for the new voyage, employed the use of the Scanned copies)
Hazard No.
Description of Hazard Hazard Severity
Degree of Likelihood
Risk Factor
Control Measures in place
1 Scanned Charts may not be corrected to the latest NTM onboard // T and P corrections
Extremely Harmful
Likely Intolerable Risk
All scanned copies corrected to the Last NTM received on board i.e Week 33/06 // Additionally T and P corrections In force for the areas covered by these charts have been marked on the charts.
2 Distortion in charts due to scanning different areas at different times
Extremely Harmful
Likely Intolerable Risk
Passage through the areas covered by the scanned charts has been planned considering the distortion by increasing Safety Margins and plotting courses away from shallow waters or Land features which might pose a threat to the vessel’s safe passage.
3 Position on the charts may not coincide due to the Charts being assembled onboard by attaching the different pages
Extremely Harmful
Likely Intolerable Risk
Larger safety Margins especially when making Landfall by increasing range at which the vessel will pass the closest dangers. All means of position fixing employed and an error in position of at least 4 to 5 nm taken into consideration when calculating safety Margins
4 Reliability of display offered by the Paper charts once scanned
Extremely Harmful
Likely Intolerable Risk
Vessel using both the Paper charts and ARCS charts to navigate through the coastal transits with constant comparison of the Land and danger Features offered by both
5 The Officer on watch may Extremely Likely Intolerable A bridge team meeting was held on the 14/09/2006 with
misconstrue some of the information displayed on the Scanned copies as being reliable
Harmful Risk all navigating officers in full attendance and briefed as to the reliability of the Scanned charts and the importance of using all means of position fixing in these areas.
6 The possibility that a strong Current may push the vessel further towards any outlying dangers
Extremely Harmful
Likely Intolerable Risk
The currents flowing through the areas covered by these charts is the Canary Currents and best judged as South – SSW’ly flowing along the West African Coast at a rate of 0.5 Knots. This has been considered at the planning stage.
8 The positions plotted in one section of the chart may differ from the position plotted in the adjacent Joined section
Extremely Harmful
Likely Intolerable Risk
Position plotting Frequency increased on the scanned charts as well as between different sections of these charts.
Additional Control Measures in place – All the above points dealt with at the planning stage and Safety margins increased. A bridge team meeting held briefing all officers about the scanned charts and their usage A Further Bridge team meeting held discussing the effectiveness of these control measures.
