1. Participation fee is inclusive of benefits as per stipulated.
2. Air ticket and airport transfer is on participants’ own expenses.
3. Registration form must be completed and returned to FMM by March 31, 2015. Payment should be made in favour
of “FMM Services Sdn Bhd” crossed “Account Payee Only”. Bank detail: Maybank 5142 0814 1636
4. FMM reserves the right to cancel or reschedule the programme should there are less than 20 participants. All
efforts will be taken to inform participants of any changes.
5. FMM reserves the right to cancel the booked seat if payment has not been received. Cancellation of registration
must be in writing to FMM. If cancellation is received after March 31, 2015, there will be no refund.
