1. Fill in the quantities of up to 10 qualifying tires purchased from the list below between 03/01/20 and 09/30/20.

2. Submit online at www.GoodyearTireRebates.com ORAdditional terms and conditions: Limit one rebate per qualifying purchase, per visit. Cannot be

combined with other Goodyear rebate offers. Offers available only at authorized participating

U.S. Goodyear Retailers and websites. The invoice from an authorized participating U.S.

Goodyear Retailer and websites must clearly identify the tire retailer name, invoice number,

date of purchase, tread pattern name, tire size, customer or business name and quantity

purchased. Offer valid only for U.S. residents who are individual consumers with mailing

addresses in the U.S. and U.S. territories over the age of 18. Commercial fleets are not eligible

for these rebates. Not valid on previous purchases. This rebate form must be submitted online

no later than October 31, 2020. Allow 6 to 8 weeks for check delivery. Failure to provide

the required supporting documents, incomplete documentation, or illegible documentation

will result in the rejection of your claim. Late submissions will not be accepted. Please

ensure that you keep a copy of all forms including your receipt for your records. Fraudulent

submissions will not be honored and may be prosecuted. Sponsor is not responsible for

noncomplying rebate submissions or for lost, late, illegible, postage-due or undeliverable

mail. Noncomplying rebate submissions will not be honored, acknowledged or returned.
