Service Area (inside red line boundary):

The St. Clair Shores Transporta!on Service provides residents of the City transporta!on to appoint-ments. Rides are scheduled first come, first served and space is limited. We will do our best to accom-modate, however same day service is not available. Please call 586-498-2331 before 3:00 p.m. Monday-Friday to make appointments. At least a 48 hour no!ce is required to accommodate a ride. Doctor appointments can require more no!ce to ensure prompt arrival !me. Monday—Friday: 8:00 a.m. -2:30 p.m. (the last !me available for pick up is 1:30 p.m.) Saturday (Shopping): 8:30 a.m.- 1:00 p.m. · All vehicles offer curb-to-curb service to medical appointments, grocery shopping, volunteer jobs, banking and other miscellaneous des!na!ons within the 8 mile service area (see map).

· Priority is provided to residents needing medical appointments. In order to ensure a return ride, the rider will need to be ready by 1:30 p.m. (Monday-Friday).

· The charge for the service is $1 each way within the City of St. Clair Shores. Appointments can be made for medical offices outside of the city within a 8 mile radius (see map) for $1.50 each way.

· If an appointment changes or needs to be can-celled/ rescheduled, please contact dispatch at 586-498-2331 as soon as possible. Please leave a message with your name and phone number if you are unable to get through to dispatch. Your call will be returned.

· Please allow a 15 minute window before and a+er your reserva!on when booking your ap-pointment !me. We want to ensure our drivers to get to you safely.

Thank you for your pa!ence and we look forward to ge'ng you to your appointments.

When making your appointment you will need to provide:

· NAME, PHONE #, ADDRESS, and TIME of where the driver will be PICKING YOU UP.

· ADDRESS, PHONE #, and TIME of appointment where they will be DROPPING YOU OFF.

· If you require a caregiver. Drivers provide curb-side pick-up; if you require more assistance you are able to bring a caregiver FREE of charge.

· Are you in a wheelchair or require the li+ to enter the vehicle? Drivers will need to make sure to al-low for more !me to secure passengers.

We will do our best to accommodate your request. Leaving a message does not secure an appointment.

Rehabilita!on Loan Program

The City of St. Clair Shores has established, as part of the federally assisted Community Development Block Grant Program, a financial assistance project en!tled “The Single Family Homeowner Rehabilita-!on Program.” The program is designed to assist qualified low and moderate income single family homeowners bring their home to a safe, sanitary, and livable condi!on, according to minimum hous-ing code standards. If you may be interested in this program and would like to see if you qualify, please contact our Rehab Coordinator at 586-447-3362 or by email [email protected] or you may pick up an applica!on at City Hall’s community develop-ment desk.

SAC LOCKER RENTAL/ RENEWAL INFORMATION

No!ces are being placed on lockers for expired con-tracts, and with the waitlist we are experiencing, please do not let your contract lapse. Locker rentals $30/ yr. with key or combo on file.

MEDICAL SERVICES & EQUIPMENT Helping Hands Coordinator has informa!on on med-ical services and equipment for seniors. Unfortu-nately, we do not have a loan closet, but we can rec-ommend resources for you to connect with should you need equipment or would like to donate equip-ment.

The Helping Hands program is designed to assist peo-ple in the community, age 50 and older. We can help you find programs that can assist you with snow re-moval, leaf pick up, grass cu#ng, chores, and much more. HOME CHORE SERVICES: GRASS CUTTING & SNOW REMOVAL ®

Macomb Community Ac!on Agency assists seniors with home maintenance and minor repairs. The pro-gram is available to persons 60 and older. Services include grass cu#ng & snow shoveling. Seniors re-ques!ng Chore Service are expected to share in the

cost of providing the service. For each service per-formed, the senior should make an appropriate con-tribu!on. No one will be excluded from receiving ser-vices if their income does not permit them to contrib-ute at the recommended level.

HOUSING Helping Hands has informa!on on senior housing available. If you, or anyone you know, is in need of assistance for housing in St. Clair Shores and sur-rounding areas, please call the 586-498-2339 for more informa!on.

Qualified senior par!cipants, 60 years and older, can receive coupons that are used to purchase Michigan grown produce. Please call to make an appointment.

bers of the Senior Ac!vity Center.

To be involved with the Senior Activity Center and programs, we require you to be 50 years or older. An annual membership fee and packet are required for participa-tion every year. Residents pay $25 per year and non-residents pay $40. Membership begins on the day of payment and is good for 1 year from the date registered. Pay-ment accepted: Visa, MasterCard, Cash, and Checks made payable to the City of St. Clair Shores. Office hours are Monday through Friday 9:00 a.m. to 4:00 p.m. Volunteers at the front reception desk will ask to see membership cards, even if they’ve seen you 100 times. We ask that you show your card to the front desk volunteers and the PM Fitness Attendant on all visits. If you do not wish to car-ry your card, or have forgotten it please take a photo on your Smart Phone to show upon visiting the Center. Thank you.

Membership Renewal Renewal dates are printed on the membership card along with the membership number. Come to the office to pay the annual member-ship fee by the anniversary date printed on your card. Your mem-bership number will remain the same, but you will receive a new membership card every year. When you sign up you will be asked to sign a release of information form along with a required survey from HUD. You may also make changes to your membership form to keep contact information up-to-date. You will not be able to par-!cipate in programs and services if

your membership is not current and your forms have not been properly filled out. Effec!ve June 1, 2014, all members are required to show their card and sign in when entering the Senior Center and par!cipa!ng in ac!vi!es. Handicapped Accessibility The Senior Activity Center and the City of St. Clair Shores actively seek and support participation by all people with a variety of interests and abilities in its recreational programs and services. The Senior Activity Center will operate its services, programs and activities so that they are readily accessible to, and usable by individuals with handicaps and disabilities. Please, let us know how we can serve you better. Hot Lunch Program The St. Clair Shores Senior Meal Program is funded in whole or in part by the Federal Older Ameri-cans Act and the Michigan Office of Services to the Aging through the Area Agency on Aging 1-B. The Senior Center complies with terms and regulations of the Title V of the Civil Rights Act of 1964 as amended and Section 504 of the Social Rehabilitation Act of 1973 and is an Equal Opportunity Employer program. Reasonable accommodation will be provided upon notification or request.

The Senior Activity Center lunch-eon contracts with the Macomb County Senior Nutrition, AAA 1-B, and the Michigan Office on Ser-vices to the Aging. In order to re-ceive subsidies per meal we are

required, by contract, to provide information on the participants of our meal program. A signed release of information is required, for re-porting purposes, that go to the state and federal agencies that pro-vide the funding for the program. Participants are asked to fill out a form that will be kept on file. These forms are updated annually. Partic-ipants must also sign their legal name each time they eat a meal. These sign-up sheets are our docu-mentation. Each week, sign-up sheets are put out for the following week! Per our agreement, meals must be consumed on site and may NOT leave the premises. Please help us to comply and con!nue to provide wonderful meals for our community! Thank you.

No membership is required for meals, however NON-MEMBER front desk sign-in is required. Requested donation: $4 (60 years+), $5 (under 60 years old).

Funding for Staffing and Programs The Senior Activity Center staff and operations receive funding in part from the Community Development Block Grant Funds (CDBG). The Senior and Disabled Transportation Program receives its funding from SMART Municipal Credits, SMART Community Credits and Michigan Department of Transportation Spe-cialized Services for staffing and operations. Due to our funding sources, information regarding de-mographics is required for mem-bership. Incomplete demographic information will result in refusal of membership.
