ASM Maritime

QUALITY HEALTH SAFETY ENVIRONMENT

What Happened?During Class annual inspection, Class Surveyor requested generator simulation test and during this test ship is blacked out. All cargo valve system shut down and vessel lost FBB communication and network system.

Preventive ActionAll engine crew is informed and warned, nobody can request any test or any other action during vessel cargo operation. I f this should be done CCR and Master must be informed.

Company CommentsIn case of any test is really necessary to be done only with Master approval.______________________________________________

What Happened?Vessel is berthed for discharging her cargo HMD. After cargo analyse (about 8 hours) and connection of the vapour return line for n2 supplying from shore, vessel commenced to discharge her cargo. During this time vessel wanted to discharge all 3 cargo tanks simultaneously, but suddenly one of cargo tank pressure failed down around -40 kpa and other two tanks pressure suddenly increased to 24 kpa. Immediately cargo operation suspended by ship. As you can see from attach sketch some pv line elbow parts which is connecting to the bw hatch was frozen. Due to this reason one of cargo tank could not supply shore N2 and other two tanks over pressured and all these two car-go tanks pressure to become waving.

There are two reasons regarding this issue:

1) Shore was supplying N2 and that N2 temperature was around 3-4 C lower than air temperature. For this reason all pv line elbows, UTI pipe and tank radar sensors are frozen when contact with cargo vapour inside the tank.

2) Shore N2 pressure was around 6-7 bar and if vapour return valve open fully then all cargo tank will be over pres-sured.

Preventive Action1) Before commencing of cargo operation all cargo line, PV line should be checked due to freezing of lines. That PV line elbow points should heat by steam manually.

2) Main vapour return line manifold valve should be open slowly while discharging commence for preventing of over pressure.

Company CommentsGood observation. Additional to preventive action: DISCHARGING ONLY ONE TANK at the same time should be planned and permitted. If more tanks are going to discharge at the same time then RA should be comple-ted and sent to office for approval.

What Happened?While vessel was supplying fw, aft peak centre tank fw shifted to technical tank by accommodation fw pump not to be aware of the ship crew. As you can see attach drawing vessel has two line for fw which is supplying to deck eye wash system and another one for deck general purpose line. That general purpose line is connecting to technical fw line on deck. When we want to supply fw from shore we are connecting shore line to manifold fw supply line. In this case shore is supplying that water to technical fw tanks via manifold line and also ship accommodation fw pump sys-tem is supplying fw from aft peak tank to again same deck line. In this case if deck line fw supply valve (Valve NR-2) will be open, all fw will go technical fw tanks.

Preventive ActionNow we will mark valve Nr 2 as ‘This valve should not be closed without permission from Chief Officer’.

We strongly encourage all crew to continue reporting near misses as they will contribute to improving safety culture within the fleet.

What Happened?After departure Bosun and Pumpman was instructed to secure deck for sea passage, extra lashings to be used for securing gangway as heavy weather expected. However, next morning both gangways found not proper secured. Railings support was unlashed and out from its mountings, butterfly bolts was not enough tight. It was easy to notice this from bridge, but OOW not report to Master. Inadequate lashings was noticed by Master.

Action TakenMaster took CON , alter course to make lee side , deck crew lashed both gangways in proper way.

Preventive ActionPlan of preparation for sea passage and heavy weather to be developed. Deck, stores, anchors to be properly check by Chief Officer.______________________________________________

What Happened?It was found by OOW that the Bosun was doing job invol-ving to maintain the forward Trunk Mesh and while he was carrying the job he was standing on the ladder which was supported by the main deck fish plate and he was without harness carrying out the above mentioned job.

Action TakenImmediately Bosun was made to stop the job, he was in-structed to wear harness to avoid slipping and tripping and lashing the ladder.

Preventive ActionDetailed RA to be carried out with the crew involving the job consisting of working aloft ,or ladders which may re-sult in unwanted injuries should the personnel involved in job has not taken care of proper PPE before the job is taken to. Emphasis of having the SAFETY FIRST attitude and proper Company Procedures should always be on the mind of every crew before they are assigned or take up their respective job, having said that Responsible Officer assigning the job should also monitor the Crew/Work Pro-gress by himself that the job/Procedures are being follo-wed diligently.

What HappenedRescue from enclosed space drill carried out. After the drill D-shackle was left connected to the davit and the hook was inside the pump room. The hatch was closed in this position. Next day during morning safety inspection found the hatch was not fully closed because of rope was passing from davit to hook and water tightness of the com-partment was compromised.

Action TakenD Shackle connected to the inner side “U” type ring of the Hatch, and the hatch was closed properly.

Preventive ActionAfter drill and training all equipment must be put in correct position.__________________________________________

What HappenedFire boxes around accommodation are lashed with rope; one end to the handle of the box another end to the railing. Not readily accessible in case of emergency.

Action TakenThe rope was removed from the Fire box immediately.

Preventive ActionIt should not be lashed by any other means except open handle in the box. Educated all crew and person in charge.______________________________________________

What Happened: (Chem Sirius)After very heavy weather during morning time safety round carried out, it was found stbd side anchor chain stopper wire sling almost parted.

Action TakenNew wire sling was prepared and fitted in place, chain stopper was tightened as far as would go.

Preventive ActionCondition of wire slings must to be inspected frequently, if any doubt replaced with new.______________________________________________

What Happened?Before LO bunkering operation it was found that LO bunker lines not marked by grade name, only cap was marked. During multiple grades LO bunkering, protection pipe caps can be mixed up with other caps and LO hose from barge could be wrongly connected.

Action TakenBunker lines were checked before bunkering to avoid mixed LO and properly marked for its grade.

Preventive ActionsAll LO lines and bunker lines to be positively marked with the grade for proper identification.

What happened?Duty deck rating was checking the cargo manifold pres-sure of 4 to 5 bars and reporting to CCR. It was observed that the rating did not put his gas/liquid tight googles on his face, but kept it just on the helmet, whilst staying in immediate vicinity of manifold connection under pressure.

Action TakenThe duty deck rating was told right away to put on his googles. He was briefed about the potential serious eye injury in case of sudden leak of cargo.

Preventive ActionsDeck crew advised on use of proper PPE at all times and to refer to the PPE matrix as per SOM Manual__________________________________________

What happened?While heaving up anchor the Chief Officer observed that the crew had not put the safety pin to lock the gear lever from moving. The gear if shifted due to the weight of the anchor chain while heaving the anchor would have allowed the anchor chain to slip back into the sea or could have caused damage to the machinery.

Action TakenHeaving was ceased till the safety pin was put in place.

Preventive ActionAll deck crew need to be advised on the precautions to take while anchoring operations and the correct procedu-res for using the windlass.__________________________________________

What HappenedThe dead man alarm key was found attached to the equip-ment making it easy to switch off by any crew member in engine room.

Action TakenThe key was taken off from the device and in custody of Ch Eng

Preventive ActionThe Key for the dead man alam system should always be in the custody of the Ch EngBest practise observed Chem New Orleans

Emergency stops fitted to both grinder and drill machine on Chem New Orleans.Both of the following crew members will be entitled with 1 additional flags for reporting the best practice and taking actions.

With this we would like to bring to your attention that as from February 2016 we are launching our new Encourage and Support Program with ASM Maritime. The main idea of this program is to create a long term commitment between the company and our outstanding seafarers.

Encourage and Support Program will allow promising crew members whose intentions are to stay with the company, to be considered for the company’s grant for their further education which will partially or fully cover their expenses.

With this we would like to outline the minimum selection criterias to be considered for the Encourage and Support Program:1) Minimum 6 months of sailing time with the company2) Positive evaluations from the previous contacts3) Willingness to develop and grow to a higher rank

We will closely assess all the crew members on board of our vessels and at home along with the Captains and C/E’s whom will help us to identify crew members meeting the minimum pre-selection criteria.

Selected crew members will be subject to the final interview and final approval by the Crewing Manager.

Those approved crew members will receive required grant for their education in full or partially and will be bound by a term agreement with ASM Maritime, agreement will be tailored individually to suit both employer and employees needs and expectations and reviewed in case-by-case situations.

We hope and expect that our new Encourage and Support Program will help our crew members to follow definite path in their career and we look forward to fruitful collaboration.

CHEMICAL BLOODTEST

ASM Maritime has started a new procedure in which crew members need to attend additio-nal medical check after signing off from the vessel to make sure that they were not exposed to any dangerous cargoes. It is coming from sincere responsibility to our seafarer’s health. This check includes only a regular and normal blood check. This check can be done at any approved maritime doctor. The expenses of these checks will be reimbursed by the company upon next joining of the crew member with the proper and official receipts. It is important to emphasize that this procedure is for the benefit of the seafarer to make sure that his body wasn’t exposed to any toxic materials.
