1. When asked for his assignment, student reaches deep into his pocket and presents a piece of ragged paper folded sixteen times.

2. Student throws a paper wad at another student.

3. Student A refers to student B as (n-word) and student B finds the comment appropriate and even funny.

4. Student has the vocabulary words and definitions printed on the back side of her ID during the test.

5. Teacher tells students that he decided the worksheet was the best option for today but he actually did not have time to prepare the lesson.
