How to enter:I need 3 items: your name as it appears on your license, the state you are licensed in and your phone number. Incomplete entries will be discarded and will not be entered in the drawing. This information will only be used for the drawing, and will be responsibly discarded after the drawing is over. Only one entry per person, please. Dynamic Equilibrium is not responsible for entries that are not received, or not received on time. There are three ways to enter:1. Like PERGCE on Facebook and then message PERG in a private message on Facebook with the entry information. 2. Email [email protected] with the entry information. 3. Pertinent information:Entries will be accepted up to August 31, 2013. The drawing will be held September 1, 2013. Up to 3 prizes will be awarded; one prize will be offered for each 100 entries, up to a maximum of 3 prizes for 300 or more entries. The grand prize of $150 is guaranteed to be awarded; the two additional prizes are $100 each. Vouchers are retroactive in the form of a check paid directly to you with proof of completion of a course advertised in this issue of the PERG, or the voucher can be paid directly to the continuing education provider on your behalf if you have not yet registered or paid for a course at the time of the drawing. Each voucher must be claimed and used in its entirety before January 1, 2014. Each entry will be printed on a plain 8½X11 piece of white paper, crumpled, and mixed in a container suffi cient to hold all entries. A blindfolded person who is not employed by Dynamic Equilibrium and is not a massage therapist will randomly select the winners from all of the entries. If you would like to be present for the drawing, please let me know, and I will invite as many as want to come, space permitting.
