There is no supervision prior to 8:00 AM or after 2:50 PM. To ensure the safety of your child, your child should not be dropped off at school more than 10 minutes prior to the first bell at 8:10 AM. Students should be picked up within 10 minutes of dismissal.

Parent Teacher Organization The PTO is a vital part of the community and is comprised of parents,

school personnel, and other community members. It sponsors activities for parents in the evening and students within the school day in correlation

with the goals. The goals of the PTO are:

1. To establish an active PTO at Stone Lake 2. To support parent involvement

3. To support on-going student and school needs

We ask that you support the PTO by paying your dues, by volunteering to help with activities, by attending PTO meetings, and by supporting the

fundraisers that give money to school programs. The meetings will be held on the second Tuesday of each month starting at 4:30 PM in the Stone Lake Library. We look forward to and encourage all parents to become

involved in the PTO.

School Site Council

The primary role of the Stone Lake Elementary School Site Council is to assist in the planning, implementation, and evaluation of our Plan to

Achieve Bold Goals. The Plan to Achieve Bold Goals focuses on enhancing targeted areas of our curriculum and other key aspects of our

school environment. Through a collaborative decision making process, the School Site Council assists in determining academic targets for improvement, oversees the budget, and acts as an advisory to

the administration relative to school improvement.

The School Site Council includes the principal, teachers, parents and other staff. The Council meets regularly to review the budget, plan,

and evaluate school programs.

Parent Involvement We are thankful for active participation from our community and encourage this

partnership as we work together to ensure that every child reach their potential so that no child is left behind. Parent participation is vital!

We continually seek assistance from our parents, grandparents, and

community at large. The following are some suggested forms of assistance:

• PTO (officer, committee chairperson, members) • Working in your child’s classroom or doing work at home • Attending the various functions/events and activities held at the school • Chaperoning field trips • Serving on advisory committees such as the School Site Council, the English

Learners Advisory Committee, GATE Advisory Committee, and the District Par-ent Advisory Committee.

School Attendance

One of the most important elements of successful learning is regular, on-time at-tendance at school. If an absence is necessary, call the school office at 683-4096 the morning of the absence or send a note. The note should include the dates of

absence and the reason for the absence. STATE LAW permits the excuse of an absence for the following reasons:

• Illness • Quarantine, as directed by the Health Department • Medical, dental or eye services rendered • Attendance at funeral of immediate family member to the extent of ONE DAY in

California, and NO MORE THAN THREE DAYS outside California. Absences of more than 3 days (four or more) without a valid excuse constitute TRUANCY and are reported to the School Attendance Review Board (SARB). Tardiness can also interfere with success in school. The times for the opening of school are printed in several areas in this booklet. Students are expected to be at school before the tardy bell rings. Students must report to the office for a tardy slip when late, in order to be admitted to class. Please do not send or take your child to class without stopping by the office. The teacher cannot admit any student without an entrance slip.

Please Note: A student will not be permitted to leave school with a person other than his/her own parent or guardian unless we receive a note signed by the parent or guardian. Stu-dents cannot sign themselves out. Student Volunteers: Students (4th grade or higher) who are off track and have volunteered to work during their time off must have permission from the requesting teacher in written form. The teacher will notify the office of students who are volunteering on campus.

Administration of Medication at School Below is listed Education Code 49423. It sets

forth procedures which must be followed if school staff are to accept the responsibility for administra-tion of medication: Notwithstanding the provision of Section 49422, any pupil who is required to take during the regu-lar school day, medication prescribed for him by a physician, may be assisted by the school nurse or other designated school personnel if the school receives (1) a written statement from such physi-cian detailing the method, amount, and time schedule by which such medication is to be taken, and (2) a written statement from the parent or guardian of the pupil indicating the desire that the school district assist the pupil in the matters set forth in the physician’s statement. (E.C. 49423)

No medication (prescription or non-prescription - including aspirin, cough drops, etc.) will be dispensed to students or allowed on the school site without the appropriate form being completed and on file in the school office. If the physician requires a portion of a

tablet, pill etc., be dispensed, the parent(s) is responsible for dividing the tablet into the prescribed size. If you have questions, please contact the school nurse. Student requiring medications at school shall be

identified to the school by parent/guardian and physician. Students observed by school person-nel administering unauthorized medications to themselves will be reported to their parent/guardians. (A.R. 4141.21 (a)).

All student medication must be in the original container clearly labeled with the student’s name

and will be kept securely locked in the school of-fice. Students may not carry medication on their

person, although exception may be made for students who need medication for potentially life

threatening conditions.

Such exceptions require physician, parent

and principal authorization.

Parents are required to provide written verifi-cation from physician to the principal of the school the pupil attends, or the school nurse, of any change in the medication or dosage. Physicians may fax forms to school sites to authorize medication changes.

The Authorization for the Administration of Medication by School Personnel must be updated annually or whenever any changes are made in the treatment plan.

At the end of the year, all medication must be picked up by the parent within five working days or it will be destroyed per safety regulations. .

Health and Nursing Service The district nursing services are administrated by a qualified school nurse who provides: • Vision and hearing screening • Updating medical information • Supervision of the maintenance of appropriate records for each student. No elementary school in the Elk Grove district has a resident nurse. Nurses are available on an on-call basis.

Illness and Attendance Children should not come to school with a fever or a rash, in fact, children should stay home one more day after the fever subsides to decrease the risk of reoccurrence of an illness. While it is important for students to return to school as soon as possible following an illness, they should not return too soon. If a student is not well enough to go outdoors at recess, he/she should not be in school.

Allergic Reactions

If your child has an identified allergy or reacts violently to insect bites, you should send a note to the office staff and to the teacher to inform them. All information should be filed at the beginning of each new school year. If medication for this allergy is necessary, a medical form must be completed and up-dated annually.

Aspirin & Other Non-prescription Medicine

The following information is designed to help you and your child comply with state law, and the district policy and procedures in relation to the administration of medication at school. 1. Accidents and Insurance

We try to do everything possible to insure a safe environment at school; however, acci-dents do happen. If an accident occurs, the school will make every effort to contact the parent or guardian. If that is not possible and there is a doubt about the seriousness of the injury, the school will contact emer-gency services. The Board of Education has authorized the principal of each school to send out information on a school accident insurance policy. Since the school district, by law, cannot pay for the medical and hospital expenses incurred as a result of an accident at school, we recommend this insurance policy. These policies are sent home with each student at the beginning of the school year.

Emergency Information In case of a serious accident, we make every effort to contact the parents or guardians. In order to do this, be sure the office and your child’s teacher has the following current information: » Parent/Guardian home and work phone numbers. » Emergency contact person’s name and phone number in case the parent or guardian can’t be reached.

Head Lice Policy

Head lice are tiny bugs that live on the hair and scalp. They are transmitted from one person to another by direct contact or on shared combs, hairbrushes, or hats. They can also be acquired from upholstered furniture and bedding. People can have head lice for a period of time and not know it. It takes about ten days for the nits (eggs) to hatch. If you think you or your child has been in contact with a per-son who has head lice, check their head for about 10 days. Check around the base of the neckline and ears. It is easier to see the nits in sunlight (nits are about 1/16th of an inch in size). Head lice cannot hop or jump! If your child is found to have head lice: 1. He/she will be sent home. 2. Your child may return to school once all nits (eggs) have been removed with proper treatment. 3. Your child may not return to class until having been cleared in the office first.

Registration of Students

Kindergarten: A child must be at least 5 years old, on or before December 2nd in the current year for which the child is being registered. The following items are required to complete the registration process: 1. Evidence of age (birth certificate, official notification of birth, passport, or affidavit of birth). 2. Immunization Requirements for Kindergarten-6th Grade: a. DPT– 5 doses total or 4 doses if the 4th dose is after 4th birthday. 3 doses meet requirement for ages 7-17 years if at least one dose was on or after 2nd birthday. If last dose was given before 2nd birthday, one or more dose and a 4th dose is required. b. POLIO– 4 doses total or 3 doses if 3rd dose after 4th birthday. 3 doses meet requirements for ages 7-17 years if at least one dose was on or after 2nd birthday. c. MMR– 2 doses, both after 1st birthday (4 day grace period if 1st dose given before 1st birthday). d. HEPATITIS B—3 doses total. e. VARICELLA—1 dose or Physician documented proof of history of disease. 3. Proof of Residence (copy of utility bill (PG &E or SMUD), which shows address and parent name, copy of purchase of rental agreement with a copy of a utility bill within 6 months if house is under construction/sale pending or 2 months for a rental agreement. At first grade entry, all students must provide the district with written evidence of a physical examination (by a licensed physician) completed no sooner than 18 months PRIOR to first grade entry. Before entering 1st grade all immunizations listed above must be completed. Proof of completion is required for entry into middle school.

Kindergarten students may be registered on Friday, December 3, 2010 for the 2011-2012 school year.

Important Policies Supervision of Students

BEFORE SCHOOL AND AFTER SCHOOL: There is no supervision prior to 8:00 AM or after 2:50 PM. To ensure the safety of your child, your child should not be dropped off at school more than 10 minutes prior to the first bell at 8:10 AM. Students should be picked up within 10 minutes of dis-missal. DURING SCHOOL: Yard supervisors are on the playground during all recesses. Teachers are responsible for all other supervision during the school day, either by an assigned schedule or by teacher selection.

Telephone Usage Students are permitted to use school phones for school business only. Special ar-rangements for after school activities should be made prior to school. Students will receive telephone calls during the school day only in an emergency.

Cell Phones Students are permitted to carry cell phones. However, cell phones must be turned off and remain off once a student enters campus. Cell phones are to be kept in the stu-dent’s backpack throughout the course of the day. Students may turn cell phones on once they exit the school grounds. Cell phones disrupting class or school activities will be collected by the teacher and held for parent pick up. The school is not responsible for broken, lost or stolen phones.

Riding Bicycles,Skateboards or Scooters

A bicycle storage area is furnished for students who want to ride bicycles, skateboards or scooters to school. IT IS THE RESPONSIBILITY OF THE STUDENT TO LOCK HIS/HER BICYCLE/SKATEBOARD/SCOOTER IN THE STORAGE AREA AT SCHOOL. There is insufficient personnel available to monitor this area, and the school cannot assume responsibility for these items. Please walk bicycles, skateboards, and scooters at all times on the school grounds. Riders must: 1. Obey all traffic control devices: i.e., stop signs and stoplights. 2. Not ride double. 3. Ride with the traffic (right side). 4. Not ride more than two side-by-side. 5. Not ride on sidewalks while on campus. 6. Park and lock items in the rack provided. 7. Walk their mode of transportation on school property, and while crossing in the pedestrian lane. 8. Wear a helmet for safety– this is a STATE LAW.

Lost and Found Most children keep track of their clothing. However, each year we end up with coats, lunch totes, etc. that are unclaimed. To help avoid a loss, please mark your child’s posses-sions with his/her name. At the end of each year, we donate unclaimed items to charity. Students or parents may reclaim lost articles from the multipurpose room.

Behavior

Playground Rules: Students will: 1. Speak and act courteously to others at all times. 2. Follow directions. 3. Stay in assigned areas. 4. Use school property and materials with care. 5. Freeze and squat when the bell rings and then walk to line. 6. Walk in straight, quiet lines. 7. Keep personal toys or balls off campus. 8. Not ride bicycles, skateboards or scooters on campus. 9. Take turns on equipment. 10. Not walk up or go down slides backward. 11. Not play tag. 12. Not fight, wrestle, or play fight. 13. Not play in the restrooms. 14. Not push others off the playground equipment. 15. Not chew gum. Lunchroom Rules Students will: 1. Walk in an orderly line in the walkways. 2. Stand quietly in a single line while waiting to be served. 3. Sit at assigned tables in the lunchroom. 4. Speak in lower voices while in the lunchroom. 5. Remain seated until dismissed by the yard supervisor. 6. Follow the yard supervisor’s directions for dismissal. 7. Carefully clean up their area before leaving the lunchroom. Character Education Program Our Character Education traits are: 1. Caring, Giving & Service: be compassionate, considerate, helpful and understanding of others 2. Justice & Fairness: be open-minded, unbiased and equitable 3. Leadership, Initiative & Teamwork: earn the respect of others; take the first step; work well with others. 4. Respect: show regard for self, others, property, and the environment. 5. Responsibility: be answerable and accountable for your actions 6. Trustworthiness: earn the confidence of others as a reliable and honest person. Each month a character trait is identified. The teachers and administration address the theme regularly and students are recognized at our Spirit Assembly.

Lunch Program Lunch Payment: Families are encouraged to purchase meals in advance. The cost for meals is $2.25. Meals bought with cash must come in an envelope labeled with the child’s 4-digit PIN number and name. If you are paying for meals by check please make the check out to EGUSD Food Services. Food and Nutrition Services (FNS) now has an online service that allows parents to prepay for their child’s breakfast/lunch and ala carte items using their credit or debit card. Parents may enroll by going to www.myLunchMoney.com and click on ENROLL NOW. This service will allow parents to view student purchases: set spending limits, and even sends them an e-mail when account balances are low. Breakfast Program: Breakfast is served daily from 7:45 a.m. to 8:10 a.m. Your child can receive a well bal-anced breakfast for $1.25. If your family is eligible for the free and reduced meal pro-gram breakfast is free. Foster Students: Guardians must complete a separate, new meal application for each foster student. Charged Meals: The FNS lead (Jean Russell) will verbally inform the child if they are running low on their lunch money. Parents are to send their child to school with a lunch or lunch money as well as pay for any previously charged lunches. Please note that students may not charge meals during breakfast. For lunch, students may charge no more than two meals. Any student who exceeds this number of charged lunches will be given a complimentary “basic lunch” which consists of a cheese sandwich and white milk. Parents should be informed that if their child has charged, they are to send their child to school with a lunch or lunch money for the next day as well as money to pay for the previously charged lunch(es). Meal Applications: To qualify for free & reduced meals, a new application must be completed and approved at the beginning of each school year. This year the 2010-2011 free and reduced free breakfast and lunch applications are yellow. Last years light green applications will not be processed. Parents must have the new yellow colored application to be approved for the new school year. FNS Leads do not approve meal applications at the school site. They will however, receive applications and forward them to the District FNS where they will be scanned and checked for eligiblity and approved if they meet the guidelines. Parents must provide their child with a lunch from home or lunch money until they receive a written notification.

Back to School Nights (Fall) Our school will host a night for parents to visit the school and discuss with teachers the school program for the school year. BACK TO SCHOOL NIGHT IS FOR ADULTS ONLY.

Open House (Spring) Open House occurs in the spring. This is a time when parents and students can come together to visit the classrooms and teach-ers, and to view work. This is not a student conference time. Conferences should be scheduled individually with teachers at other times. Open House dates are: March 24 B & C tracks May 19 A & D tracks

Field Trips All field trips are planned to coordinate with the regular instructional program. Regularly enrolled students who meet specific academic and behavior standards may participate. Permission to attend may be denied if students fail to meet the state criteria. Field trips are normally funded by the par-ticipants and/or fundraisers, although no child will be denied access due to lack of money. Bus fares and entrance fees are included in the cost of the trip. Completed permission forms must be submitted to the teacher prior to the field trip for the student to be able to attend. Parent permission granted over the phone will not be ac-cepted since it is impossible to establish identity. Attendance is required on field trip days as classroom instruction will be provided for students who are unable to attend the field trip.

After School Activities Instrumental music, school choir, golf les-sons, and art lessons are featured after-school programs. Parents permission must be granted, and parents agree to provide transportation and to assume all costs associated with the activities.

Awards Trimester Awards: Each trimester there will be an awards as-sembly to honor student accomplishments in grades 4th-6th. Awards given include: •Perfect Attendance-Students who have no absences and have no more than two unexcused tardies. •Most Improved-Teachers select students who have shown the most improvement in citizenship and/or academics. •Honor Roll-Student’s who receive a 3.5 to 3.9 GPA. •Principal’s List-Student’s who receive a 4.0 GPA. •Physical Fitness Awards-This award is given to 5th graders who meet the criteria during the 3rd trimester.

Report Cards Report cards are standards based and is-sued three times during the year. Confer-ences may be scheduled by the teacher or parent as needed throughout the year.

Uniform Policy Student Dress and Grooming Guidelines: Stone Lake Elementary is a uniform school with a uniform policy and a dress code for those families who feel that they would prefer not to dress in uniform. Parents or guardians have the primary responsibility for appropriate standards of dress and grooming. However, as an elementary school in the Elk Grove Unified School District we have the responsibility to establish and maintain appropriate standards of dress and grooming for students who choose not to wear the uniform. The purpose of the dress and grooming code is to facilitate education, not to inhibit any person’s taste in attire or appearance. Students should be clean and neatly dressed in a manner that will be decent, not hazardous to the health and safety of the students and not disruptive or distracting from the educational program of the school. Tops for Boys & Girls: Solid white, navy blue Polo type shirts or blouses with a collar, sleeves; shirt or blouse with collar, buttons down the front, short or long sleeves, turtle-neck or mock turtle-neck. Bottoms for Boys & Girls: Solid navy blue, khaki (tan) slacks with or without cuffs, pleated, or plain, walking shorts-no shorter than mid-thigh, with or without cuffs, pleated or plain; Capri pants for girls. Bottoms for Girls: Solid navy blue, khaki skorts, (shorts with front flap)-No shorter than mid-thigh. Skirts-No shorter than mid-thigh (at least) pleated, gathered or straight. Tights/Leggings/Bike Shorts: Solid navy blue or white tights, leggings or bike shorts/work out type shorts can be worn under skirts, skorts, shorts or jumpers. Leggings and bike shorts can ONLY be worn un-der an outfit and cannot be worn as a bottom alone. Shoes for Boys & Girls: Plain white, navy blue, black or brown high or low top are encouraged; basic laced sneakers, oxford, or short ankle length boots. High athletic, high-tops with excessive col-ors and lights are not appropriate. High heels, platforms, sandals, or flipflops are not acceptable. Sweaters/Sweatshirts (outerwear) for Boys & Girls: Plain white or navy blue sweaters; cardigan, v-neck, or crew neck pullover; long-sleeve style pullover; ribbed or plain bottom edge. Dress Code Enhancement for 5th and 6th Graders Only: During the school year of 2010-2011, we developed a dress code enhancement for our 5th and 6th graders only. This enhancement is closely aligned with our current uniform policy. • Students can wear dark blue jeans, shorts, or skirts • Solid colored T-shirts (no logos) with or without a collar (navy, white, maroon or tan) • A Spirit Shirt in the two school colors with a hawk logo (maroon or navy blue) Track change days will be a non-uniform day for all students. If students choose, they may have a free dress day on track change days. All other days, students are expected to be in uniform or enhanced attire. Picture Day: On picture day only, free dress is allowed for only the tracks that are having their pictures taken. All other tracks must be in uniform attire. Uniform Assistance Option: A plan for parents needing assistance with implementation of the uniform policy is in place through the Clothing Closet. Distracting Materials or Items: Materials or objects that distract from instruction must be left at home. Teacher permis-sion can be given for special occasions. Unacceptable : 1. Make-up, pictures, trading cards, comic books 2. Artificial nails, however, clear or light colored fingernail polish is acceptable 3. Any electronic device or game such as headphones, IPod, DS, IPhone, PSP, etc. 4. Spray cans or spray containers of any type 5. Sports equipment (school furnishes all equipment) 6. Toys of any type 7. Sunglasses 8. Strong perfumes or colognes Helium balloons should be delivered to homes rather than to the school. Although they are fun and exciting for children, they are a distraction in the classroom .
