Northwestern Insurance Requirements
• Northwestern requires all degree-seeking students to have comprehensive health insurance
• Students holding a J-1 or F-1 visa are required to remain enrolled in the Northwestern University Student Health Insurance Plan (NU-SHIP) for the entire time they are at Northwestern
• International students must confirm NU-SHIP enrollment annually
• Students confirm the NU-SHIP via online Coverage Selection Form in CAESAR (“Profile” section in CAESAR)
How to get your insurance ID card

• Visit “aetnastudenthealth.com/northwestern”• Click “Get your ID card”• Use your student ID number found on your WildCard plus a “334” prefix• PLEASE NOTE! Your card will be available 4-5 business days AFTER you confirm
in CAESAR
“Covered” Does Not Mean “Free”

• Evanston and Chicago Northwestern University Health Services (NUHS) are freefor students enrolled in the NU-SHIP
• When seeing a Specialist outside the NUHS you are covered by the NU-SHIP, BUT:o You must have a referral from NUHS unless you need emergency services. o Without a referral you will be charged a $50 penalty fee (This includes going to an Immediate Care Facility during open hours at the NUHS)
o There is a $100 co-pay when you go to the Emergency Room and are not admitted to a room for overnight care
o Covered services outside of NUHS are subject to the plan benefits

Breakdown of Out-of-Pocket CostsSeptember 1, 2019 - August 31, 2020
Premium: The amount of money you’re charged to be enrolled in your health insurance ($4,050 on Student
Account)
Deductible: The amount you pay in claims before your insurance plan starts to cover your medical costs ($250)
Co-insurance: The percentage of your medical costs that you are responsible for paying after the deductible has
been met (80/20 for in-network!)
Co-pay: The set amount charged to the member for a particular service or item (Rx ONLY!)
Annual out-of-pocket maximum: The maximum amount of money you pay annually for medical costs before
your plan starts to cover expenses at 100%. ($2,000)
PLEASE NOTE! When seeing an out-of-network provider:o You are responsible for a $500 deductibleo You are responsible for 40% co-insuranceo There is no out-of-pocket maximum

• Visit “aetnastudenthealth.com/northwestern”• Click “Find a Network Provider”• Enter your location (zip code, city, county, or state)• Select “Open Choice PPO” under “Student Health Medical Plans• Search either by provider type or search for a specialist by name• The search results will indicate if the provider is in-network

• However, Northwestern University has contracted with Northwestern Medicine to provide Primary Care on the Chicago Campus in parallel to services offered on the Evanston campus at NUHS.
• Northwestern Medicine Immediate Care Centers are NOT part of the agreement, and services will be subject to the deductible, coinsurance, and out of pocket amounts.
